Commissary Kitchen Replacement Equipment Purchase

1) Scope of Work Replacement of 2 existing Rational Combi Oven which have become too expensive to continue repairing and they are not in proper working condition. The requested equipment are necessary to provide cooked foods for the Visn2 sites which includes 8 medical facilities totaling 3000 patients and 2 million meal per year.

2) Objectives Required to purchase 2 replacement Combi Oven with required accessories which help prepare and cook recipes for customer sites. Bids will include removal of existing Rational Combi Oven, factory authorized installation, installation of two water filtration system, professional training, and any warranty work on new equipment as needed for a specific period of time.

It is the sole responsibility of the owner/operator/purchaser of this equipment to verify that the incoming water supply is comprehensively tested and, if required, provide a means of water treatment that would meet the compliance requirements with the manufacturers water quality standards published on the product spec sheet. Non‐compliance with these minimum standards will potentially damage this equipment and/or components and VOID the original equipment manufacturers warranty.

INSTALLATION

PHASE 1

1. Installer will run a water line over the ceiling from the back side of the combi ovens to the opposite wall where we will install the RO water filtration system.

2. We will also mount a rail to protect the RO system from being knocked into.

PHASE 2

1. Installer will remove and dispose (2) Rationale roll‐in combi ovens

2. Installer will receive, deliver, un‐crate and set in place

a. (2) Alto Shaam roll‐in combi ovens

b. (2) reverse osmosis filtration systems & (2) booster water pumps.

3. Installer will now install the above‐mentioned equipment to properly sized existing utilities such as water, drain and electric

4. Installer will now install the filtered water from the newly installed RO system

5. Installer will test and make ready for use.

6. All labor will be provided on two scheduled Saturdays 8:00 A.M. to 4:00 P.M.

7. Mechanical startup to be performed by local authorized service agency upon completion of the installation during NORMAL working hours.

8. The pricing reflects “NON PREVAILING” wages.

9. Any required permits are to be supplied by the Saint Alban's VA Medical Center.

10. Any required upgrades to accommodate the equipment are to be completed by the Saint Alban's VA Medical Center.

11. The quote does not include any unforeseen price increases, labor and materials, upgrades, which may be required should problems arise.

5) Travel/Parking

The contractor will be responsible for their travel. As stated earlier, due to other construction projects and limited parking available at the site, contractor must coordinate any parking needs with the assigned COR and the Police Service. Details of any vehicles entering the site must be provided in advance. (Driver’s Full Name, Driver’s License Number, Vehicle Description, Vehicle License Plate Number)

6) Government-Furnished Property

Contractor will not need any government furnished equipment or property.

7) Education and Training Requirements

The Vendor shall be responsible for ensuring that vendor employees providing work on the required equipment are fully trained and completely competent to perform the required work. Specify what training and education is required for the clinical staff to be competent in using/operating the supply.

9) Limited Source Justification/Recommended Source

TriMark/Gill Marking is the govt. representative for Rational Ovens which we require as replacement.

10) VA Patient Privacy and Network Infrastructure

None of the equipment listed within this procurement package will access the VA Network or store any sensitive patient information. Any contractors that will come on site for install will not need access to VA network. All contractors must comply with VA regulation and complete the following training: “VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access or use to VA Sensitive Information.”

· Reference the attached Handbook 6500.6 Appendix A - Checklist for Information Security in the initiation phase of acquisitions.

· The system does not transmit or maintain electronic Protected Health Information. A Manufacturer Disclosure Statement for Medical Device Security (MDS2) form in not applicable.

· A VA Directive 6550 Pre-Procurement Assessment form is not applicable.

11) Recalls

The Vendor shall immediately notify the Visn2S Commissary designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification:

· A complete item description and identification (to include lot and serial numbers where applicable);

· Administrative identification data including order numbers and order date;

· Reasons for recall, removal, adjustment, or modification; and

· Instructions for appropriate corrective action.

12) Defective Items/Warranty

Warranty will be no less than 2 years from start-up of equipment by an factory authorized installer.
