Statement of Work Central Alabama Veterans Health Care System Geriatrics, Extended Care & Rehabilitation Service (GEC&R)
Scope of Work
LOCATION: Audiology Clinic – Robert S. Poydasheff VA Clinic
DESCRIPITION: Deliver, install, and perform training on Audiology Equipment. The contractor will supply all equipment and tools needed to complete installation. Onsite delivery and installation to the Robert S. Poydasheff VA Clinic located in Columbus, Ga.
1. The contractor shall deliver, install, and calibrate as selected for designated clinics. Installation and training shall be performed in accordance with manufacturer’s guidelines and safety codes for health care facilities.

6. (3 each) Hi-Pro- is a universal hardware interface designed to program all kinds of hearing instruments. The high-speed mode enables fitting modules to communicate with the hearing instrument faster than the Hi-Pro USB version.

3. The Contractor and the designated representative from Audiology department will jointly review the specific site of installation within the clinic prior to initiation of work.
4. Changes to the design after formal acceptance will be agreed upon jointly by the Contractor and the designated representative from Audiology Clinic; with the representative providing the Contractor written documentation of the changes.
5. Testing Process:
· The Contractor will perform safety checks/testing in accordance with manufacturer’s guidelines at the conclusion of installation.
· The Government Owner (Audiology department representative and/or Biomedical Engineering representative) reserves the right to be present during any or all of testing.
· 100% of installed equipment must be tested. All tests must pass acceptance criteria.
6. Standards Compliance & Test Requirements
Refer to manufacturer’s guidelines.
7. Documentation Requirements:
· Test reports may be submitted in hardcopy or electronic format. Hand-written test reports are not acceptable;
· Test reports shall be submitted within seven business days of completion of testing.
· Electronic reports are to be submitted via e-mail. If proprietary software is used, CD shall contain any necessary software required to view test results. If the results are delivered in a standard format like Excel, Access, CSV files, etc., then software to read these files are not provided. Electronic reports must be accompanied by a Certificate signed by an authorized representative of the Contractor warranting the truth and accuracy of the electronic report. Certificate must reference traceable circuit numbers that match the electronic record.
8. Information Technology Requirements
· The Certification & Accreditation requirements do not apply, and a Security Accreditation Package is not required.
· The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract.
9. Acceptance
Once all work has been completed, test documentation has been submitted, and the Government (Audiology and Biomedical Engineering departments) is satisfied that all work is in accordance with contract documents, the Government shall notify Contractor at time of installation of equipment acceptance.
10. Acceptance Requirements
Contractor must warrant in writing that 100% of the installation meets the requirements specified under Standards Compliance & Test Requirements above.
Acceptance shall be subject to completion of all work, successful post-installation testing which yields 100 percent PASS rating, and receipt of full documentation as described under Documentation.
11. Warranty
Manufacturer’s warranty includes a period of one year for parts and labor. Contractor shall adhere and warrant installation against all product and labor defects for a period of one year.
12. The contractor shall provide a timeline to the Government on the installation and training. Contractor shall deliver and install no earlier than December 23, 2021. 13. Schedule of Performance: Contractor will provide dates and times when items are to be accomplished to facility, Logistics/Warehouse.
