Scope of Work CT Scanner and MedRad Stellant CT Injector

Radiology Department, Phoenix Indian Medical Center 01.28.2021

1. INTRODUCTION:

The Phoenix Indian Medical Center is seeking a service agreement for CT Scanner (GE Brightspeed Elite 16 Slice) and MedRad Stellant CT Injector for Radiology Department, CT Unit/1st Floor, Phoenix Indian Medical Center. The purpose of the CT scanner and MedRad Stellant CT Injector is to show tissues, blood vessels, and bones in various parts of the body. The medical device scans cross sectional images of the body. Service agreement shall include a full service semi-annual preventive maintenance (PMs), repairs, replace parts if necessary, replace glassware if required, labor and travel. Emergency services shall include 24/7 repairs, after 4:30PM to 8:00AM (M –F), weekends, federal holidays and technical supports only if needed.

2. BACKGROUND:

Phoenix Indian Medical Center, Biomedical Department is in need of a service agreement with a Base plus Two Option Years. CT scanner and MedRad Stellant CT Injector are located in CT Unit outside by the Emergency Room (ER) loading dock and Biomedical is seeking a full service agreement for semi-annual preventive maintenance (PMs), repairs, replace parts, replace glassware if required, labor and travel. A full repair service agreement shall provide better medical equipment maintenance, gather accurate information, and increase operational efficiency.

24/7 emergency repair services shall include only if services are needed for after-hours from 4:30PM to 8:00AM Monday - Friday, weekends, federal holidays and provide technical support.

NOTE: Emergency calls are only when services are required after 4:30PM to 8:00AM (M-F), 24/7 coverage, weekends, federal holidays and technical support.

**Base plus Two Option Years, the existing CT Scanner shall retire after Option Year Two. A new CT Scanner is in the process to be purchase and a new building.

3. SCOPE OF WORK:

Description of the Medical device:

• GE Brightspeed Elite 16 Slice and MedRad Stellant CT Injector ID No. 3014226355454

• Serial Numbers: SHICT41

• Coverage: Full Service, include glassware replacement

• Product: 24/7 Coverage

• Radiology Department

All repairs and replacement parts shall meet the Manufacturer’s specification requirements.

4. DESCRIPTION OF SERVICE:

The contractor shall provide supervision, labor, parts, materials, tools, supplies, and test equipment necessary to provide required PMs, repairs, and annual inspection. The service shall include certifications, calibrations/calibration verifications, replacement parts (if required), and provide service reports on the items in SOW Section 3 above.

4.1 The contractor shall obtain and maintain all required manufacturer, Federal, State, local licenses and certifications associated with the performance of the services required.

4.2. Performance shall be according to the SOW, supporting documents and shall also conform with:

4.2.1 Original Equipment Manufacturers (OEM) specification requirements

4.2.2 National Fire Protection Association (NFPA) 99 and 101

4.2.3 Standards for Health Care Facilities

4.2.4 Environment of Care (EC.02.04.01, EC.02.04.03)

4.2.5 The Joint Commission standards

4.2.6 Federal, Tribal, State and local regulatory guidance shall be applied in regards to bio hazardous waste, disposal, and applications.

4.3 Work shall be performed in the hospital environment.

5. PREVENTIVE MAINTENANCE SERVICE (PMs) AND FIELD SERVICE REPORTS:

Service Technician shall provide PMs and field service reports upon completion of inspections and repairs.

Requesting contractor be consistent with scheduling on a yearly basis, to not deter from schedule during each option year.

5.1. Contractor shall utilize the Original Equipment Manufacturers (OEM) established procedures and checklists or Contractor – supplied equivalent; satisfactory to the designated Point of Contact (POC).

5.2. Return the equipment to operating condition defined in OEM specifications.

5.3. Test and replace faulty and worn parts.

5.4. Inspect/replace electrical wiring and cables for wear and fraying.

5.5. Complete OEM field service updates and engineering change notices.

5.6. Perform annual test to meet the manufacturer’s specified requirements and standards.

5.7. Contractor shall perform calibrations, cleaning, oiling, adjusting, and repair of the equipment except those services necessitated by accident, fire, or abuse.

5.8. Inspect all mechanical components, including, but not limited to, mounting hardware, pumps, seals, belts, interlocks, sensors, controls, bearings, hoses, heaters, radiators, generators, and motors for operational integrity.

5.9. Replace any OEM labels, decals, or warning tags that are not legible.

5.10. Corrective Maintenance: The Contractor shall notify the designated POC of the existence or development of any defects or repairs to the equipment covered under this contract which the Contractor considers is not PM (such as operator misuse).

5.11. Contractor shall warrant all services performed and shall remain free from defects 30 days from the date of acceptance.

5.12. The contractor shall include repairs, however, all repairs require pre-approval from the Contracting Officer.

5.13. Clean equipment: Return equipment in a clean state after work is completed.

6. DOCUMENTATION/REPORTING REQUIREMENTS:

Contractor shall comply with IHS visitation policies. The Contractor’s personnel shall report to the PIMC Biomedical Department at the beginning and end of their visitation, to sign-in and receive facility “Guest” ID badge from Security.

7. SCHEDULE:

The contractor shall provide a detailed schedule to the PIMC POC for approval prior to the commencement of any work. Exceptions to the schedule shall be arranged and approved in advance with the designated POC. Schedule and Notification: The Contractor shall notify the designated POC prior to starting services and upon completion of services.

8. EQUIPMENT CERTIFICATION OF INSPECTION: The contractor shall provide new device, warrant coverage for approximately a year in case if the device is not function properly that need to be return to the vendor for replacement. Upon award contractor will have the environmental controls and permits to perform installation. ICRA permits from infection control will be provided, contractor will adhere to all safety measures when performing installation for CT scanner and MedRad Stellant CT Injector.

1. Repair Service and Installation: The repair shall be free of charge while under warranty.

2. Parts: Parts shall be replaced at no cost to the Government.

9. CONFORMANCE STANDARDS:

Contractor shall comply with the Equipment Manufacturer’s specifications, National Fire Protection Association (NFPA) 101, 99, and the Standards for Health Care Facilities and Environment of Care (EC.02.04.01, EC.02.04.03) and Joint Commission Standards. All materials used in performance of work shall be Underwriter's Laboratory (U.L.) approved for their purpose and function.

10. ADDITIONAL PROJECT REQUIREMENTS:

All shipments of materials, equipment and/or supplies to the project site shall be addressed to the Contractor and not the Indian Health Service. Preservation, packaging and packing shall be in accordance with industry standard packaging appropriate for the item(s) involved. The Indian Health Service is not responsible in any manner for deliveries intended for the project that are not generated by the Indian Health Services.

Equipment and materials provided by the Contractor shall be new and, where appropriate, shall arrive on-site sealed in original manufacturer’s containers. The Government reserves the right to refuse any items deemed to not meet this requirement.

11. DESIGNATED PROJECT OFFICER:

The Project Officer's responsibilities will include inspections of new device to ensure the electronic device is operating in good standing and provide accurate results. The Project Officer does not have the authority to change or alter the order amount, terms and conditions. Requesting contractor to cooperate with Project Officer on the scheduling and repair of device.
