Statement of Work
Flooring Replacement
United States Fish and Wildlife Service Hart Mountain National Antelope Refuge – Plush, OR
Sheldon National Wildlife Refuge – Denio, NV
1.0 General.
1.1 Contractor shall remove and replace existing carpet or vinyl flooring and replace with Luxury Vinyl Plank (LVP) flooring and install vinyl baseboard and thresholds in residences and bunkhouses on Sheldon- Hart Mountain NWR Complex.
1.2 Period of performance is Notice to Proceed + 90 Calendar Days.
2.0 Replacement Requirements.
2.1 Replacement materials shall conform to the salient characteristics and standards specified below:
2.1.1. LVP and vinyl baseboard installation shall follow general industry standards and product manufacturer’s installation requirements and specifications.
2.1.2. LVP will be “brand name or approved equal.”
• Core: Stone Polymer Core (WPC) or Rigid Core Vinyl
• Wear Layer: 20 mil (a UV-cured finish would also be allowed)
• Plank Dimensions: nominal 7 inches.
• Plank Thickness: 5 mm.
• Installation Method: Float, Direct Glue
• Environmental Certifications: GreenGuard (Certification, Gold, Private Label) Certified
2.1.3 Vinyl Baseboard will be “brand name or equivalent.”
• Baseboard height: 6 inches
• Installation Method: adhesive
3.0 Scope of Work.
3.1 The work will involve pulling, removing, and disposing existing materials; scraping and embossing;
installing LVP following manufacture’s installation, and installing rubber base and thresholds. Area in square feet is provided, however contractor should verify measurements.
The areas will be free of furniture and/or appliances.
Task 1 – Hart Mountain Manager Quarters 1 – Kitchen – Wood subfloor.
• 150 sq. ft.
3.1.1 Contractor shall:
• Pull, remove, and dispose existing vinyl flooring.
• Clean and prepare subfloor.
• Install LVP.
• Install 6-inch rubber base.
Task 2 – Sheldon NWR Manager Quarters – Living/Dining/Hallway. Wood subfloor.
• Living/Dining = 600 sq. ft.
• Kitchen = 210 sq. ft.
3.1.2 Contractor shall:
• Pull, remove, and dispose existing vinyl flooring.
• Clean and prepare subfloor.
Task 3 – Sheldon NWR Manager Quarters – Bedrooms. Wood subfloor.
• Bedroom 1 = 125 sq. ft.
• Bedroom 2 = 165 sq. ft.
• Bedroom 3 = 220 sq ft.
3.1.3 Contractor shall:
• Pull, remove, and dispose existing carpet flooring.
• Clean and prepare subfloor.
The following tasks are identified as quote option:
Task 4 – Sheldon NWR - 1000 Creek Bunkhouse Bedroom 1 concrete slab subfloor
• 180 sq. ft.
3.1.4 Contractor shall:
• Pull, remove, and dispose existing carpet flooring.
• Clean and prepare subfloor.
Task 5 – Sheldon NWR - 1000 Creek Bunkhouse Bedroom 2 concrete slab subfloor
• 170 sq. ft.
3.1.5 Contractor shall:
• Clean and prepare subfloor.
Task 6 – Sheldon NWR - 1000 Creek Bunkhouse Bedroom 3 concrete slab subfloor
3.1.6 Contractor shall:
• Pull, remove, and dispose existing carpet flooring.
• Scrape and emboss floor.
Task 7 – Sheldon NWR - 1000 Creek Bunkhouse Bedroom 4 concrete slab subfloor
3.1.7 Contractor shall:
• Scrape and emboss floor.
Task 8 – Sheldon NWR - 1000 Creek Bunkhouse Bedroom 5 concrete slab subfloor
3.1.8 Contractor shall:
• Scrape and emboss floor.
3.2 At the completion of work, the contractor shall clean all surfaces and remove all waste materials from the project site.
4.0 Submittals and Deliverables.
4.1 Contractor shall submit the following items to the COR for review and approval, in .pdf format, where applicable.
a. Material Submittals, including LVP brand and specifications, rubber base brand and specifications, and flooring embossing compound.
b. Material submittals will include style and color selections for each Task above.
5.0 Availability of Utilities and Government Furnish Equipment.
5.1 Contractor shall furnish all temporary utilities, except as noted otherwise. Power and water will be made available to the contractor as long as such use is not deemed excessive and does not negatively impact the Refuge’s operations. Refer to FAR Clause 52.236-14 for additional information.
5.2 Refuge bunkhouse may be available to use during project if space is available and with refuge manager approval.
6.0 Inspection and Acceptance.
6.1 The Contractor is responsible for all quality control to guard against defects and deficiencies in the work. Inspect and test work as needed to ensure that the quality of materials, workmanship, construction, finishes, and functional performance are in compliance with applicable specifications, drawings, building codes and standards. Inspection and acceptance will be determined by the COR or Onsite Coordinators.
7.0 Place of Performance.
7.1 Place of performance will be at the Hart Mountain National Antelope Refuge, 38782 Hart Mountain Road, Plush, OR 97637.

7.1.2 Hours of Operation: Work shall be performed between Monday thru Friday, 7:00 AM to 5:00 PM local time, excluding Federal Holidays. Work outside those dates / hours shall be coordinated with the Onsite Coordinator.
7.2 Place of performance will be at Sheldon National Wildlife Refuge, 44290 Virgin Valley Road, Denio, NV 89404.

FAR 52.234-14 Availability and Use of Utility Services (Apr 1984)
(a) The Government shall make all reasonably required amounts of utilities available to the Contractor from existing outlets and supplies, as specified in the contract. Unless otherwise provided in the contract, the amount of each utility service consumed shall be charged to or paid for by the Contractor at prevailing rates charged to the Government or, where the utility is produced by the Government, at reasonable rates determined by the Contracting Officer. The Contractor shall carefully conserve any utilities furnished without charge.
(b) The Contractor, at its expense and in a workmanlike manner satisfactory to the Contracting Officer, shall install and maintain all necessary temporary connections and distribution lines, and all meters required to measure the amount of each utility used for the purpose of determining charges.
Before final acceptance of the work by the Government, the Contractor shall remove all the temporary connections, distribution lines, meters, and associated paraphernalia.

52.237-2 Protection of Government Buildings, Equipment, and Vegetation (Apr 1984)
The Contractor shall use reasonable care to avoid damaging existing buildings, equipment, and vegetation on the Government installation. If the Contractor’s failure to use reasonable care causes damage to any of this property, the Contractor shall replace or repair the damage at no expense to the Government as the Contracting Officer directs. If the Contractor fails or refuses to make such repair or replacement, the Contractor shall be liable for the cost, which may be deducted from the contract price.

Payment requests must be submitted electronically through the U. S. Department of the Treasury's Invoice Processing Platform System (IPP).

The Contractor must use the IPP website to register access and use IPP for submitting requests for payment. The Contractor Government Business Point of Contact (as listed in SAM) will receive enrollment instructions via email from the Federal Reserve Bank of St. Louis (FRBSTL) within 3 - 5 business days of the contract award date. If the Contractor is unable to comply with the requirement to use IPP for submitting invoices for payment, the Contractor must submit a waiver request in writing to the Contracting Officer with its proposal or quotation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)
(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1), in a subcontract for commercial products or commercial services.
