Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

1. Contracting Activity: Department of Veterans Affairs, VISN 6, Network Contracting Office (NCO) 6, Durham VA Medical Center. Purchase request number 558-23-2-122-0069
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The Durham VAMC has a requirement to procure eighteen(18) Codonics Safe Label System SLS 550i® Point of Care Station’s (PCS). This brand is required by the Pharmacy and Anesthesia Department for medical labeling. The Codonics Safe Label System SLS 550i PCS is the standard of care in the world’s leading hospitals. An award-winning, FDA-approved Class II medical device, SLS improves the safety and accuracy of medication management and labeling compliance anywhere medications are prepared. SLS utilizes barcode technology to read information from a drug container and electronically identifies it against the pharmacy’s hospital-approved drug database to reduce medication errors and create standardization.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Codonics Safe Label System is required to provide safe labeling that complies with the Joint Commission Standards for Medical Labeling, ASTM 4774, ISO 26825, and ASA guidelines. This labeling system will provide a standard of care for the patients at the Durham VAMC by using barcode technology to read vial and ampoules at the point of care. The Codonics System is the only point of care station compatible with the Omnicell Anesthesia Workstation that is currently in use at the Durham VAMC and throughout VISN 6. When used in conjunction with the Omnicell Workstation, the system provides visual and audible identification of the drug while producing a label that includes a barcode to enable integration with the electronic health record.
4. Description of market research conducted and results or statement why it was not conducted:
Research was conducted using SBA, FPDS, NSSL, NAC, GSA, and Internet.
