Scope of Work Sequential Intercept Model Statewide and Community Evaluation Report 11/1/2025- 9/29/2026 The Indiana Division of Mental Health and Addiction (DMHA) is seeking a qualified vendor to provide evaluative work on over 60 communities implementing a sequential intercept model (SIM). In 2019, DMHA granted State Opioid Response (SOR) funds to the Indiana Office of Court Services to implement SIM models across the State of Indiana. Through other community partners and stakeholders, local communities worked to map their SIM and implement at least one intercept with funding from the SOR grant. These grants have been renewed annually for communities who choose to reapply.

1) Executive Summary of Indiana SIM Models Grants during FY (statewide)

2) Community-Intercept specific “One-Pagers” for each participating community (60+); including

a. Easy to understand language for layperson;

b. Infographics and visually appealing depictions of community intercepts

c. Summary on Community Intercepts including mission, activities, personnel, etc.…

d. KPI reporting

i. KPIs will be negotiated by DMHA and IOCS with communities

3) Statewide recommendation for subsequent years of SIM grants and sustainability measures for communities to consider If budget allows, the vendor may include limited technical assistance to counties in improving their intercept model and enhancing reporting capabilities.

The contract for this work will start no earlier than November 1, 2025, and finish no later than September 29, 2026. Contracts will be deliverable based on a negotiable deliverable schedule but will at minimum include a final report product no later than September 29, 2026.
