SCOPE OF WORK

AIHC DRX Radiology Equipment 2022

Anadarko Indian Health Center Anadarko, Oklahoma

A. Overview of Project

The intent and objective of this Scope of Work is to trade-in current Carestream DRX1 Evolution Digital Diagnostic Radiographic equipment with state of the art Digital Radiography equipment which will perform digital diagnostic x-ray, long-bone studies, spine studies, Artificial Intelligence, bariatric table, high capacity generator with minimal exposure to the patient. We would like to purchase the equipment with in the next month. The addition of Digital Diagnostic Radiographic equipment will make available the most update digital diagnostic radiology services to the Anadarko Indian Health Center patient population. To ensure a smooth transition into Digital Diagnostic Radiographic equipment the project will need to be turnkey.

B. Services to be Performed by the Vendor

1. The Vendor will sell to the Anadarko Indian Health Center a state of the art Digital Diagnostic Radiographic equipment

2. The Vendor purchased Digital Diagnostic Radiographic equipment will consist of the following items:

i. Digital Diagnostic Radiographic System

ii. Turn-key delivery to include complete room update

3. Vendor will provide the following setup and installation services:

i. Deliver Digital Diagnostic Radiographic equipment

ii. Complete setup of Digital Diagnostic Radiographic equipment

iii. Complete room update

· Setting up Digital Diagnostic Radiographic equipment in designated room including leveling and anchoring

· Connect utility service at the Anadarko Indian Health Center’s source

· Connect phone and data to the Anadarko Indian Health Center’s network

· Startup and testing of Digital Diagnostic Radiographic equipment

· Physicist Shielding Inspection Pre-Installation, Physicist Acceptance Post-Installation, Radiation Protection Survey Post-Installation and for compliance by Radiation Consultants, 620 Oak Summit Road, Edmond, OK 73025 (405) 340-1195

4. Minimum System Requirements

i. The Digital Diagnostic Radiographic equipment

· 1-17x17 wireless detector interchangeable in table or wall stand

· 1-14x17 wireless detector interchangeable in table or wall stand

· Windows 10 or Linux Based Platform

· Automatic tube tracking for table and wall stands

· Bariatric table height from 21” to 37”, weight limit at least 850 lbs, longitudinal tabletop travel +/- 18.9”, Flat tabletop

· Wall stand, vertical travel range up to 57.1”, low central beam height 10.6”, tilting +90/-20, grids removable from the sides, 40” focused grid, 72” focused grid, 40-72” multi-focus grid; Automatic Exposure Control utilizing three ion chamber sensors

· Ceiling mounted overhead tube, 10.4” touch user interface, eye level touchscreen

· X-ray tube: 80kW generator; Tube voltage range: 40 to 150 kV; Tube current range: 10 to 1000 mA; Loading time range: 20 to 2000 ms: AEC max backup: 512 mAs and/or 2000 ms; Anode heat storage capacity: 580,000J

· Cybersecurity features: whitelisting, 90 day patching, HD data encryption, role-based access

· Virtual Collimation, Auto Thorax Collimation, Smart Virtual Ortho

· Edit Patient Registration fields, switch features on and off, choose different workflows and define favorites tools

· Fully automated, positioning tube and wall or table stand at the touch of a button

· Low dose imaging

· Individual clinical protocols

· Tailored dose setting for defined patient size

· Dedicated clinical protocols set according to department defined age brackets

· Artificial Intelligence for at least Chest X-Rays

· Auto Stitching for Scoliosis and Long Leg studies

· Scoliosis board or equivalent

· License for Scoliosis Stitching

· License for Artificial Intelligence

· Query/Retrieve

· 3D Camera for Live Positioning

· System Wireless Access Point for internal WIFI communication

· Operation Console

· Quality Assurance System

· Repeat/Reject Analysis

· Auto Protocol Assist

· Auto Field of View

· Uninterrupted Power Supply

· Main Disconnect Panel (as required)

· Clip on grid

· Grid holder

· Extra batteries and a battery charger

· Weight bearing Cover for Detector

· Additional line items for yearly maintenance and service for option years 1-4.

ii. Turn-Key Installation

· Trade in of Carestream DRX 1

· Removal, rigging and disposal of Carestream DRX 1

· Replacement of flooring

· Repaint walls with anti-microbial paint

· Reconfigure ceiling tiles, lighting and fixtures to meet specifications of new over-head tube

· Replace Uni-Strut as required to meet specification of new equipment

· Remove and replace existing countertops in control room with a 4” back splash, which includes a solid surface shelf for the diagnostic imaging system’s CPU.

· Remove and replace existing diagnostic imaging system’s main disconnect panel as necessary

· Remove existing diagnostic imaging system’s power feeder(s) (wire, conduit and fittings) from the existing diagnostic imaging system’s main disconnect panel to the source electrical panel in the exam room.

· Remove abandoned in-floor cable ducting and surfaced-mounted or exposed electrical conduits, cable ducting, junction boxes and panels.

· Remove existing electrical step transformer that currently provides electrical power to the current x-ray system.

· Furnish and install concrete patching material to fill the existing floor depressions and the opening where the existing in-floor electrical cable ducting has been removed.

· Furnish and install a self-leveling cementitious underlayment in the exam room to the extent necessary to fill the existing floor depressions and the openings where the existing floor anchors have been removed and level the existing floor in the area below the diagnostic imaging system so it complies with the pre-installation manual’s specification.

· Furnish and install CPU cart under existing control countertop.

· Solid surface countertops will be Corian, or equivalent, fabricated and installed in accordance with the manufacturer’s specifications

· Furnish and install caulking and fire-rated sealant compounds. The fire-rated materials will be consistent with the fire rating of the penetrated building component.

· Patch, priming included as applicable, the areas of the existing walls in the exam room damaged by the production of the work.

· Furnish and install vinyl composition floor tile in the Exam room, using materials similar to the existing floor tiles.

· Furnish and install 4” rubber base in the exam room.

· Patch the existing ceiling within the site in the areas damaged by the work using materials similar to the existing ceiling materials.

· Paint the existing and new walls, door and window frames in the exam room with 2 coats of latex paint.

· Paint the existing and new electrical cable ducting and conduit in the exam room with 2 coats of latex paint similar to the paint color on the adjacent walls.

· Paint finish will be similar to the existing paint finish in nearby areas.

· Perform final air balancing in the exam room using an independent certified air test and balancing company, including furnishing a written final air balancing report to customer.

· Furnish and install a new 3-wire, 3-phase electrical feeder (conduit, wire and fittings) rated for 75 amps at 480 volts. The feeder will extend from installed in the existing electrical distribution panel located in the Electrical room.

· Furnish and install a 150 KVA step-up transformer in the Electrical room.

· The existing electrical cable ducting, junction boxes, conduit and fittings throughout the site will be used if adequate for the new equipment, they will be replaced as necessary.

· Install one manufacturer specified main disconnect panel and associated system emergency off button.

· Transportation, delivery, rigging and installation of new diagnostic imaging equipment

· Final site cleaning

5. Contractor will provide the following support

i. On-line help

ii. Technical support

iii. Customized Equipment to support clinical needs

iv. Applications Training – Initial 32 hours, and follow-up 16 hours

v. Customer Support 24/7

vi. Site Assistance, Pre-planning and Set-up, Project Management

vii. Transportation, On-Time Delivery

viii. Systems meet OEM Specifications or greater

ix. Equipment Maintenance

x. Trade in and de-installation of current Carestream DRX1 equipment

6. Government Furnished Items

· The Government will ensure the existing floor structures and building system will support the loads imposed by the work, the occupancy and use of the site and the new diagnostic imaging system’s components.

· Removal and/or abatement of asbestos or other hazardous or toxic materials prior to the commencement of work.

· Constructions related reviews and permits as related to the work.

· Heating, ventilation and air-conditioning systems are adequate to support the operation of the new diagnostic imaging system and the occupancy or use indicated in the drawings.

· Suppling electrical power to the electrical feeder in the existing electrical room that complies with the vendors power specifications for the diagnostic imaging equipment.

C. Pre-purchase Inspection:

Pre-purchase Inspection – pre-purchase inspection date is the date established in the contract documents when the Digital Diagnostic Radiographic equipment is to be setup, fully functional, tested (i.e. images transmit to PACS), properly anchored and is ready to turn over to the Government. All deliverables shall be complete and submitted to the Government at that time.

D. Period of Performance:

Installation-September 15, 2022; The Pre-purchase Inspection is September 1, 2022.

EVALUATION CRITERIA

The quotes received in response to this RFQ will be evaluated by a simple PASS/FAIL, either the proposal meets all of the requirements in the Statement of Work or it does not.
