STATEMENT OF WORK

Physical Access Control System (PACS)

VISN 17 Center of Excellence, Building 93

REQUIREMENTS

The purpose of this SOW is to complete a Turn-Key project for creation of a security system in the VISN 17 Center of Excellence, building 93, located on the Waco, Texas VAMC. This security system will enable select staff-members to monitor the building for safety compliance including monitoring foot traffic of staff and the public. This project will encompass the removal of existing PIV card reader hardware and wiring and replacement of these components with new materials. Additional requirements will be to add video cameras, software license, and an entry door push-button. All labor, materials and equipment needed to address these requirements will be included in this contract, resulting in a fully operational security system.

All work performed will be done at the Central Texas Veterans Health Care Facility in Waco, at 4800 Memorial Drive, Waco, Texas 76711. The results of this project will result in a single badge access database for the PIV badges in building 93, Center of Excellence, creating access control, personnel accountability and provides a safe environment veterans and staff.

After all work is completed, the contractor will have a final walkthrough with the VA COR or Alternative COR to ensure all work is completed and ready to receive payment.

All work shall be done in accordance with the terms, conditions and specifications that meet NFPA 70 standards along with OSHA and EPA rules and regulations.

Working Hours

Work will be completed on weekdays, excluding federal holidays, during the hospital’s regular weekday business hours. Normal working hours are 8:00 am – 4:30 pm, Monday through Friday. Federal holidays observed during the execution of work may be exchanged for contractor observed days, only on a one-for-one basis, requested by the contractor and subject to approval by the contracting officer (CO).

Scope of Work

• The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, materials, supplies, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service per the requirements and specifications herein.

• The contractor shall install new equipment and CAT 6 wiring:

o No refurbished, returned, or used equipment is permitted.

o Do not use any existing equipment or wiring.

o All finishings must match existing door hardware and building details (Bronze).

o The contractor is responsible for door hardware testing and functionality.

• The contractor shall be familiar with all the terms, conditions, and requirement herein contained. All work must comply with local, state, federal regulations.

• An extended warranty with an optional Service Contract must be provided by the contractor.

Period of Performance

The contractor shall complete the work required under this SOW in 120 calendar days or less from the Notice to Proceed (NTP), unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor’s proposal, the contractor’s proposed completion date shall prevail. Contractor shall not work on Federal Holidays unless VA COR deems it necessary. Schedule power outages will be on weekends and cable splices will be on weekend.

General Requirements

• The contractor must be a licensed and certified installer/integrator for CCURE 9000

• Structural members shall not be cut or altered, except where noted on drawings and/or specifications, without the authorization of the CO.

• Work remaining in place that is damaged or defaced during this contract shall be restored to the condition existing at the time of award of contract.

• The contractor shall provide all necessary barriers, safety equipment, and signage for both pedestrian and vehicle traffic impacted while work is in progress.

• All personnel engaged in the contract activities specified herein must be licensed by the state, local authority, and/or the city local authority in those trades, crafts, or professions that require licensing by such jurisdictions. The license must be of a grade or other level consistent with the requirement of the work being performed and/or as established by said jurisdictions.

• The contractor shall protect all walls, ceilings, floors, furniture, etc., from damage due to water, staining, or falling debris created during the coring/sawing process. The contractor will repair any damage to the original condition at the contractor’s expense. All areas will be cleaned after core drilling.

• The contractor shall seal all penetrations produced or impacted by this project meeting the fire rating and/or weather barrier to NFPA 99 requirements and/or local fire code, whichever is greater.

• No unauthorized work shall be initiated without the prior approval of the CO or the COR.

The CO will approve any request, or proposal for changes in project scope and/or additional requirements, in writing prior to the contractor proceeding with any related work.

Any changes accomplished without prior written approval, will be done at no additional expense to the Government.

• All measurements and locations are approximate and shall be verified by the contractor.

The Government will not be held responsible for any error or variation in any measurements or other data listed, nor will the contractor, because of an error or variation in any measurement listed, be relieved of his responsibility to carry out the contract in accordance with the true intent and meaning of the specifications without additional expense to the Government.

Other Considerations

• This contract requires a site visit prior to bid to be fully acquainted with existing site conditions and scope of work. It is Contractor’s responsibility to field verify prior to bid all existing condition, transformer, equipment, cable distance, cable route, measurements and areas provided in scope of work. It is contractor responsibility to locate underground utilities.

• Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the Contracting Officer’s attention.

• Contractor must provide own lock for Lockout/Tagout procedures to work on the transformer.

• The contractor will be required to submit time schedules for how long a power outage will last.

• The contractor must comply with the requirements for Type C/Class II (Lowest Risk) Infection Control Risk Assessment (ICRA).

• It is the contractor’s responsibility to provide adequate documentation verifying that all transformer equipment is USA made and proposed that it will meet or exceed all federal government requirements.

• The contractor’s supervisor shall always be available to address needs the VA COR/Alternative COR may have during the period of performance which is 120 days from start to finish.

• The contractor shall thoroughly clean inside and outside of project site daily with contractor’s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc.

• The contractor shall be responsible for the removal of project demo materials/debris /equipment from this facility daily due to nature of the working environment (i.e. mental ward areas).

SPECIFICATIONS & LOCATIONS

The location and types of security are threefold:

First, PIV readers (200-bit card encryption) need to be placed at the following locations and they are to be attached to the CTVHCS CCURE 9000 system for use. The PACS system will contain 25 keycard/PIV readers: both exterior doors and interior doors, to include coverage for mechanical rooms, telecom rooms, hazardous material rooms, second-level access points, and the MRI offices. Complete installation and maintenance checks of controllers, displays, alarm set points, battery backup, including adjustments as needed, by qualified field service engineers. Smart 3000 battery back-ups for the main building front and back entry doors (3 total), provided by the contractor, each lasting at least 3 hours to allow for access control during electrical outages. Door access (exterior/interior) will be controlled through VA Police Service and the CoE’s “Thick Client” Monitoring Account for viewing of all cameras and access controls. As stated under Scope of Work, only new equipment and CAT 6 wiring is to be used. All finishings must match existing door hardware and building details (Bronze). The contractor is responsible for door hardware testing and functionality.

**Remove Old Readers**

Second, installation of 8 security cameras with varied viewing (locations below) to be aligned with a NVR Recorder held within the CoE and tied into the VA Network through an IP address. The reception desk will have a desktop monitor that allows them to view their surroundings (limited camera access) and a selected POC(s) will have monitoring access to the security system so they can always review building security, which will allow for remote monitoring. Smart 3000 battery back-ups through UPS System support will be provided by the contractor, each lasting at least 1 hour to allow for electrical outages without room lockouts or insecurities. Recordings should be stored for 90 days at 15 frames per second on stream one and 7 frames per seconds on stream 2. The live video should be viewable on three workstations supplied by the client with specifications provided by the vendor.

Third, a push-button access system will be installed under the reception desk for building accessibility. This button will be used to open the front entry doors (2 sets of double-doors).

Notification of Work

• Prior to the start of the work, contractor must notify VA COR 72 hours in advance to schedule a time to start working.

• Contractor must notify the COR 72 hours in advance to schedule a power outage.

• The contractor shall contact government COR no later than 9 a.m. if scheduled work for that day will not be performed.

Site Conditions

• The work covered by these specifications is an occupied facility.

• It shall be the contractor’s responsibility to field verify all existing conditions and dimensions prior to bid.

• Constant communication with the COR to coordinate work to be performed is required. The contractor shall leave no area unsecured or unsafe at the end of the day.

• The contractor shall always keep the premises as clean as possible and free from an accumulation of waste materials and rubbish.

• All materials removed shall be disposed of offsite by the contractor.

Documentation of Services Performed

• The contractor shall provide at a minimum the following data:

o All manufacturer warranty information for each piece of equipment installed o The contractor shall provide a database (excel spreadsheet) card reader serial number, card reader type, location, floor, and room number (if applicable).

Manuals will include a Compact Disk (CD) with electronic versions of all documents, as well as any viewing, management, or configuration license on CD.

o Contractor shall include programming guides, configuration manuals, and user guides.

• Submit the proper documentation and reports to receive payment on this service. See information below:

o Payment to be made monthly, in arrears, following satisfactory performance by Contractor’s personnel and upon receipt of a properly prepared invoice.

o Contractor shall be required to submit invoice through the Tungsten System for payment at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.

o The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.

o The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.

VA Responsibilities

• The VA facility will provide access to all equipment being worked on.

• The VA COR will coordinate with the facility managers on power outages.

• The VA COR will let the contractor know of any hazards or other harmful conditions.

• The VA COR will provide emergency response teams members if needed.

• VA personnel will ensure unqualified personnel do not enter restricted areas in the building.

Codes and Standards

The project shall be in accordance with the latest Uniform Building Code (UBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), VA Specifications and Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.

The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following:

(a) Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.

(b) Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on-site “competent person” have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor’s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work.

(c) Compliance with EPA and State environmental regulations.

Safety Plan Requirements Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations.

The safety plan shall include, but not limited to the following:

A. Safety and Health Policy (i.e. company policy)

B. Safety and Health Objectives (i.e. company goals, procedures, etc.)

C. Job Site Inspections (i.e. safety inspection/potential hazards/ mishap prevention, etc.)

D. Contractor Safety Administrator (i.e. competent person/safety officer)

E. Personal Protective Equipment (i.e. head, hearing, and eye protection, footwear, clothing, gloves, etc.)

F. Safety Rules (i.e. rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.)

G. Job Safety Training (i.e. OSHA, safety meetings, etc.)

• 30 Hour OSHA Construction Safety Course – competent person

• 10 Hour OSHA Construction Safety Course – all others

H. Safety Discipline (i.e. correction of violations, etc.)

I. Emergency Procedures (i.e. responsibilities, procedures, etc.)

J. Lockout/Tagout procedures but be known by the technician

Safety Inspections

On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following:

A. Means of Egress o Fire/Smoke Doors unlocked, free of obstructions.

o Means of Egress (includes Exits) free of obstructions and clearly marked.

B. Housekeeping and Traffic Control o Trash and debris are removed promptly and transported outside the construction area.

o Floor mats and dust tack mats are located at the entrance to construction area.

o Tools and equipment are properly stored.

o Signs installed to restrict patient access (doors locked as needed).

o Dust control measures are in place.

o Barriers are in place and properly sealed.

C. Clothing o PPE is available and properly worn.

o Workers clothing is relatively dust free while in the construction areas and especially during travel to patient areas.

Warranty Period

• Contractor must provide one (1) year warranty on parts and labor.

• Warranty period does not start until VA COR signs off on the project.

VHA Supplemental Contract Requirements for Combatting COVID-19

1. Contractor employees who work in or travel to VHA locations must comply with the following:

a. Documentation requirements:

1) If fully vaccinated, contractors shall show proof of vaccination.

i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.

2) If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test that includes an affiliated telehealth service.

3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.

4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.

a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.

4. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

C.3 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

Addendum to FAR 52.222-42 STATEMENT OF EQUIVALENT RATES FOR

FEDERAL HIRES

The information provided in this clause is for informational purposes only and is not a wage determination. This is not an all-inclusive list, but only a representative list of federal service employees that could be employed under this contract. This clause also states the wages and fringe benefits payable to each if they were employed subject to the provisions of 5 U.S.C. 5341 or 5332 (i.e., GS/WG federal employee). It is the contractor’s responsibility to determine the human capital required to execute the terms of the contract. Likewise, contractors are responsible for confirming with DOL that they are using the appropriate wage determination for the employees they use in the performance of this contract and it is the contractor’s responsibility to ensure employees are paid in accordance with the appropriate classification as determined by DOL.
