B.3 STATEMENT OF WORK
STATEMENT OF WORK (SOW)
1.0 PURPOSE: The purpose is to procure a brand name or equal 6050 JCB 505-20 TC Compact Telehandler for Amarillo VA Medical Center – Engineering Service.
2.0 GOVERNMENT FURNISHED RESOURCES:
Packing list shall include lot numbers, serial numbers and expiration dates of all delivered products and equipment.
3.0 SECURITY REQUIREMENTS:
The Contractor personnel must follow all VA policies, standard operating procedures, applicable laws and regulations while on VA property. Violations of VA regulations and policies may result in citation and disciplinary measures for persons violating the law.
- Currently all Attachments owned by facility are JCB brand, must be compatible with the following attachments: Fork extensions, Buckets.
4.0 Salient Characteristics:
Must be brand name or equivalent to 6050 JCB 505-20 TC Compact Telehandler with the following salient characteristics:
- Block Heater For local weather
- Front and rear drive axles:
Epicyclic hub reduction drive/steer axles with torque proportioning differentials.
- Electric selectable 4-wheel steer - 2-wheel, 4-wheel, crab, knob controlled in cab
- Service brakes: Hydraulically operated self-adjusting, oil-immersed in-board multi-disc type mounted on front axle.
- Parking brake: SAHR handbrake, self-adjusting, oil-immersed in-board multi disc type mounted on front axle operated by in-cab switch.
- Two section, 20ft lift height with hydraulic parallel lift, 5,500 lbs. max capacity, 3,000 lbs to full height.
- Two-stage gear pump, fitted with suction strainer and filter.
- One boom end auxiliary hydraulic service, with option for a second auxiliary service.
- Warranty: 2YR or 2000 hours year full coverage warranty (see the machine warranty certificate for details).
- Hydraulic pin lock, sae standard, 1 aux,t4 hi-viz only
- Basic cab with solid lower door with front and rear wiper
- Skid-steer carriage- hydraulic pin locking
- Front worklights only
- Strobe beacon
- Air conditioning
- 48 inch (1200mm) Long Industrial Pallet Forks & Frame for skid steer carriage

5. WARRANTY:
The contractor shall provide the standard warranty that will cover all parts, labor, supplies, travel costs, and user support at no additional cost to the Government.
6. INSPECTION AND ACCEPTANCE:
6.1 Only the Contracting Officer (CO) or Contracting Officer’s Representative (COR) has the authority to inspect, accept or reject all deliverables. All rejected deliverables will be corrected at the contractor’s expense with no additional cost to the government.
6.2 The contractor shall provide a complete set of unabridged operator manuals, service manuals, electronic schematics, troubleshooting guides, and parts lists for each piece of equipment which is part of the system procured by the Medical Center.
6.3 The contractor shall provide a complete set of manufacturer’s cleaning guidelines/instruction foreach system component that is used.
7. PLACE OF PERFORMANCE/ DELIVERY:
Timely submission of deliverables is essential to successfully completing this requirement. Contractor shall provide an on-site delivery team to perform unpacking, uncrating, and removal of packing material, setup and inside delivery to location specified by Contracting Officer Representative (COR) or delegate.
The Contractor shall ship the procurement/item to:
Amarillo VA Medical Center – Engineering Service Warehouse 6010 Amarillo Blvd. West Bldg. 33 Amarillo, TX 79106
8. HOURS OF OPERATIONS:
Delivery shall be scheduled from 7:30 am - 4:00 p.m., Monday through Friday, excluding Federal Holidays.
There are ten (11) Federal holidays set by law (USC Title 5 Section 6103) that VA follows:
Under current definitions, five are set by date:
New Year's Day January 1 Juneteenth Independence Day June 19th Independence Day July 4 Veterans Day November 11 Christmas Day December 25
If any of the above falls on a Saturday, then Friday must be observed as a holiday.
Similarly, if one falls on a Sunday, then Monday must be observed as a holiday.
The other six are set by a day of the week and month:
Martin Luther King's Birthday Third Monday in January Washington's Birthday Third Monday in February Memorial Day Last Monday in May Labor Day First Monday in September Columbus Day Second Monday in October Thanksgiving Fourth Thursday in November
If the President of the United States declares any other day a federal holiday it shall be included in the above federal holiday list.
9. Mandatory Check in/out and Removable of Media Scanning:
9.1 For any services performed on-site the Contractor must, upon arrival at the AMAVAMC, report to the Facility POC to check in before proceeding to the any department and before performing any services.
9.2 Prior to leaving the medical center, the Contractor must check out with the POC.
This check in and check out is mandatory.
9.3 Upon check in the Contractor must ensure that any removable media is scanned by the Biomedical Engineering Section prior to connecting to any AMAVAMC network, device or system. The Contractor must provide any removable media to Biomedical
Engineering staff to perform malware/virus scan checks. The Government will not perform any virus or malware removal on the Contractor’s removable media.
9.4 The Contractor shall be responsible for assessing the Medical Center for all requirement cabling and other equipment needed for installation.
9.5 The contractor will re-connect the new unit to the existing utilities and perform the start-up check to ensure proper function.
9.6 The Contractor will consult and coordinate with the in-house engineering group to gain access to restricted areas.
9.7 All installation plans will be reviewed and signed off on by the project POC and engineering representative.
9.8 The Contractor will contact POC for delivery/install

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
This is a lowest price technical acceptability in which competing offerors' technical capability and price will be evaluated to determine who represents the greatest benefit to the Government at a price that can be determined fair and reasonable. By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement or inability to perform or provide all services requested shall result in an offer being determined technically unacceptable. The evaluation process shall proceed as follows:
Following receipt of quotes, the Government will perform a comparative analysis of the services quoted. The Government will compare quotes to one another to select the service that best benefits the Government in fulfilling the requirement. The evaluation of Government requirements outlined in this Request for Quote will determine suitability.
TECHNICAL QUESTIONS:
Technical questions shall be accepted by the Government in writing via email only no later than 11:00 A.M. Central Daylight Time (CDT), 08/09/2022. Please annotate subject line of e-mail with “Technical Questions – 36C25722Q0872 – Compact Telehandler (Forklifts)” and send to Jose Campa, Contract Specialist, at jose.campa@va.gov. Please include in your questions the section of the solicitation/page number that your question is referencing.
OFFER SUBMITTAL INSTRUCTIONS:
1) Offeror shall submit offers directly via email to email to Jose Campa, Contract Specialist, at jose.campa@va.gov. Subject line of e-mail shall be “Quote - 36C25722Q0872 – Compact Telehandler (Forklifts)” by Due Date/Time specified in Block 8 of the Standard Form (SF) 1449 of the solicitation.
2) Offeror must respond to the representation in paragraph (b)(2) and (v)(2) of Provision 52.212-3.
3) Offer must sign and return/email the Standard Form (SF) 1449/Solicitation. By signing the SF 1449, the Contractor attests acknowledgement of receipt of all solicitation amendments if applicable and the terms and conditions. See Step 1 above for complete details.
4) Offer must submit all required evaluation documentation to allow evaluation in accordance with the Basis for Award. Please see below for minimum documentation/information required.
Pricing: Award will be made based on price and other non-price factors, to the responsible Offeror whose offer, conforming to the solicitation, and which is most advantageous to the Government. Therefore, the Government reserves the right to select a response that provides benefit to the Government that exceeds the minimum but is not required to do so. Responses may exceed the requirements, but the government is not requesting or accepting alternate quotes/offers; each response must respond to the solicitation requirement statement.
Technical Capability: The Contractor shall attest as being able to deliver two (2) each Compact Telehandler (Forklifts) to the Amarillo VA Health Care System in accordance with the SOW. All warranty information will be evaluated and weighed with technical capability.
Documentation that confirms the company is registered and is the required socio-economic category in the System for Award Management (SAM). Service Disable Veteran Owned Small Business (SDVOSB) offerors must be verified by the Department of Veterans Affairs Center for Verification at the time of submission of proposal. Failure to be verified will result in the offeror’s proposal to be deemed non-responsive. Offerors must be registered in VETBIZ.va.gov to receive consideration for this contract.
Period for acceptance of offers: The offeror agrees to hold the prices in its offer firm for 90 calendar days from the date specified for receipt of offers.
Failure to submit a complete offer may result in a non-responsive determination.

E.3 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL
SERVICES (NOV 2021)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
• Price
• Technical
