C.3 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR
2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

C.5 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT
REQUESTS (NOV 2018)
(a) Definitions. As used in this clause—
(1) Contract financing payment has the meaning given in FAR 32.001;
(2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. This office can be contractually designated as the receiving entity. This office may be different from the office issuing the payment;
(3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests;
(4) Invoice payment has the meaning given in FAR 32.001; and
(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.
(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:
(1) VA’s Electronic Invoice Presentment and Payment System at the current website address provided in the contract.
(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI).
(d) Invoice requirements. Invoices shall comply with FAR 32.905.
(e) Exceptions. If, based on one of the circumstances in this paragraph (e), the Contracting Officer directs that payment requests be made by mail, the Contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office.
Submission of payment requests by mail may be required for—
(1) Awards made to foreign vendors for work performed outside the United States;
(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;
(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;
(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or
(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

C.6 VAAR 852.242-71 ADMINISTRATIVE CONTRACTING OFFICER (OCT
2020) The Contracting Officer reserves the right to designate an Administrative Contracting Officer (ACO) for the purpose of performing certain tasks/duties in the administration of the contract.
Such designation will be in writing through an ACO Letter of Delegation and will identify the responsibilities and limitations of the ACO. A copy of the ACO Letter of Delegation will be furnished to the Contractor.

(d) Comptroller General Examination of Record. SECTION D - CONTRACT DOCUMENTS, EXHIBITS, OR ATTACHMENTS
D.1 STATEMENT OF WORK
I. TITLE:
Automatic Transfer Switches (ATS) Maintenance.
II. BACKGROUND INFORMATION:
As per the NFPA99, NFPA 110 and the Join Commission, the VA is required to perform inspections, testing, maintenance, and calibration of Automatic Transfer Switches (ATS) components every 12 months. The services shall comply with all VA, local, state, and Federal policies and regulations.
III. OBJECTIVES:
Contractor shall provide Maintenance of 25 Automatic Transfer Switches (ATS) to cover the annual preventive maintenance and testing on 25 Automatic Transfer Switches (ATS) to include a final test report for the VA Loma Linda Health Care System (VALLHCS), Facilities Management Service (FMS).
Contractor shall provide all supervision, labor, transportation, materials, tools, supplies, and equipment to accomplish the following:
Visual and Mechanical Inspection
1. Inspect physical and mechanical condition.
2. Inspect anchorage, alignment, grounding, and required clearances.
3. Prior to cleaning the unit, perform as-found tests.
4. Clean the unit.
5. Use appropriate lubrication on moving current-carrying parts and on moving and sliding surfaces.
6. Verify that manual transfer warnings are attached and visible.
7. Verify tightness of all control connections.
8. Inspect bolted electrical connections for high resistance using one or more of the following methods:
a. Use of a low-resistance ohmmeter in accordance with Section
7.22.3.B.1.
b. Verify tightness of accessible bolted electrical connections by calibrated torque wrench method in accordance with manufacturer’s published data or ANSI/NETA Table100.12.
c. Perform a thermographic survey in accordance with ANSI/NETA Section 9.
9. Perform manual transfer operation.
10. Verify positive mechanical interlocking between normal and alternate sources.
Infrared Temperature Survey
1. Provide an infrared detecting system for detecting and measuring infrared radiation. Provide the services of a certified electrical equipment field test engineer to perform tests and make necessary measurement.
2. The infrared detecting system shall be capable of measuring actual temperature or relative temperatures of an object by comparison with a reference object. The detecting system shall be able to measure operating temperature from 0 degrees centigrade to 1,000 degrees centigrade and shall be capable of measuring thermal gradients as small as 1 degree centigrade.
3. Detecting system shall have the capability of providing photographs of areas tested.
4. The infrared survey shall be conducted at times of maximum loading, and with equipment covers removed in all cases (from 07:00 a.m. to 3:00 p.m.
Monday through Friday). Coordinate with the COR.
5. Provide a report indicating the following:
a. Problem areas (location of the hot spots).
b. Indicate the temperature rise between the "hot spots" and the normal or reference area.
c. Indicate the cause of heat rise.
d. Indicate phase unbalance if present.
e. Index of areas scanned.
f. Report any problem areas to the Electrician Supervisor immediately so corrective work may be initiated in a minimum of time.
Electrical Tests
1. Perform resistance measurements through bolted connections with a low resistance ohmmeter in accordance with Section Manufacturer’s recommendation and ANSI/NETA 7.22.3. A .8.1.
2. Perform insulation resistance tests on all control wiring with respect to ground. The applied potential shall be 500 volts dc for 300-volt rated cable and 1000 volts dc for 600-volt rated cable. Test duration shall be one minute.
3. Perform a contact/pole-resistance test.
4. Verify settings and operation of control devices.
5. Calibrate and set all relays and timers in accordance with Section 7.9.
6. Perform automatic transfer tests:
a. Simulate loss of normal power.
b. Return to normal power.
c. Simulate loss of emergency power.
d. Simulate all forms of single-phase conditions.
7. Verify correct operation and timing of the following functions:
a. Normal source voltage-sensing and frequency sensing relays.
b. Engine start sequence.
c. Time delay upon transfer.
d. Alternate source voltage-sensing and frequency sensing relays.
e. Automatic transfer operation.
f. Interlocks and limit switch function.
g. Time delay and retransfer upon normal power restoration.
h. Engine cool down and shutdown feature
Reports shall contain a section covering the procedure used to make necessary tests and a description of the test equipment used.
Reports shall have a section containing a summary of all test results and the contractor shall make recommendations based on a component engineering analysis of the test data. Recommendations will include areas where immediate action is required and recommended areas where additional maintenance needs to be performed.
Reports shall be submitted in an original and four (total of 5) copies to the Chief Engineer, VALLHCS, within 30 days of work completion.
Automatic Transfer Switches (ATS)
IV. PERIOD OF PERFORMANCE:
Base year plus four (4) option years. Work shall be performed on a weekday or weeknights (i.e., Monday through Friday during the day, Saturday, and/or Sunday during the day) (starting at 6am to 5pm for days or 6pm to 5am for nights due to the requirement of load demand). Work shall be coordinated with COR.

VII. SPECIFIC REQUIREMENTS:
• Schedule with Contracting Officer Representative (COR) or Designee five (5) business days in advance before work starts.
• One week prior to start of job, all personnel shall have a valid VALLHCS PIV badge.
• Prior to working, contractor shall coordinate ALL LOCK OUT /TAG OUT of energy sources. Upon completion of work, contractor shall notify VALLHCS Electrical supervisor that these sources will be re-energized and verified.
• Prior to work each day, the contractor’s designated lead or supervisor are/is to check in with VALLHCS COR or in their absence, check in with Graphics Control in the plant. (Room, Bldg. #)
• Staging areas and temporary parking will be arranged as needed.

XI. SAFETY AND PPE EQUIPMENT:
Shall be provided for each Contractor employee by the Contractor.
XII. OVER AND ABOVE REPAIR:
Upon discovery of a malfunctioning part the Contractor shall immediately notify the COR. The repair job shall not be initiated until approval is obtained from the Contracting Officer and appropriate Facilities Staff/COR. Charges for repair, labor and parts shall originate only when required and upon prior approval being granted by the Contracting Officer. At the VA’s option the Contractor may be requested to provide a price quotation for the repair job. The VA has sole discretion to select the Contractor who will provide these repair services and they will be ordered as a separate action (not as a part of this contract).

The contractor will guarantee workmanship and all parts furnished by the contractor for a period of one (1) year from the date of installation. Warrantee for labor will be 1 year from service provided.
VHA Supplemental Contract Requirements for Combatting COVID-19
1. Contractor employees who work in or travel to VHA locations must comply with the following:
a. Documentation requirements:
1) If fully vaccinated, contractors shall show proof of vaccination.
i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.
2) If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test that includes an affiliated telehealth service.
3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.
4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.
a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.

E.1 ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—
COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES
INSTRUCTIONS TO OFFERORS:
Offerors are notified that VA Form 10091, FMS Vendor File Request Form and W-9, Request for Taxpayer Identification Number and Certification shall be faxed directly to the VA Financial Service Center in Austin, Texas and be submitted with their quote. Failure to include this information can eliminate the offeror from award consideration or considerably delay the award.
Each quotation submitted in hard copy format shall include one copy of the quote. If an alternative quote is submitted, the Contractor shall submit one copy of the alternative quotation.
Quotes submitted by email shall be sent to Rochelle.Bennett@va.gov and will be accepted by the Government at the time the email is delivered to the Government email server. Quotes must also be in PDF, and compatible with Microsoft Office Software to be accepted. The Contractor submitting a quote is responsible for ensuring that the contract specialist has received the quote before the deadline for receipt of quotes, and the government will not be responsible for delays or failures or either the contractor’s email system or the Government’s email system. Facsimile offers will not be accepted.
System for Award Management: IAW FAR 52.212-4 (t) Contractors submitting quotes are encouraged to go to the SAM database at https://sam.gov/content/entity-registration and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. After initial registration, the contractor is required to update registration data as changes occur and must re-register annually to ensure all data remains current. Noncompliance with this requirement will preclude the exercising of any option periods that may be included herein and will be cause for termination of the contract at such time noncompliance is discovered. Refer to cited Clause for more details.
GENERAL INSTRUCTIONS FOR QUOTATION SUBMISSION
Format of Quotation: In order to be considered for selection, the Contractor must submit a complete response to this solicitation using the sequence and format instructions provided.
Quotes shall be clear and concise, logically assembled (with all pages appropriately numbered) and indexed and cross-indexed to applicable parts of the Statement of Work (SOW) or Request for Quotation, as appropriate. To reduce quote size, the Contractor submitting a quote shall confine submission to essential matters sufficient to define the quotation and provide an adequate basis for evaluation. The Quotation shall not contain classified data. NO ZIPPED FILES are permitted.
The Contractor submitting a quote may be asked to clarify certain aspects of their quotation.
Communication conducted to resolve minor or clerical errors will not be constituted as discussions. Although the government intends to award the contract without discussions with respective Contractors, the government reserves the right to conduct discussions if deemed in its best interest.
Submittal Instructions: The Contractor’s quotation must be labeled with the Contractor’s name and business address of the firm, the solicitation number 36C26223Q0634, and point of contact to include name, telephone, and email address. The Government is not responsible for identifying inconsistencies between the two and may rely on either version at its discretion.

The Contractor is advised that all submissions and related materials become the property of the U.S. Government and will not be returned. Quotations, if accepted by the Government, will form binding parts of the contract that results from this solicitation. Therefore, care must be taken to properly address the requirements set forth in this RFQ.

Questions: All questions must be submitted by email to Rochelle.Bennett@va.gov no later than April 13, 2023 at 12:00pm (MST). The Government reserves the right to not answer questions not meeting this timeline if doing so would result in an amendment requiring and extension of the solicitation and the extension would not be in the best interest of the Government. All responses to questions will be provided on the spot or posted on Contract Opportunities no later than April 17, 2023.
