c. The VA has mandated electronic invoice submission to the Veterans Affairs Financial
Services Center (VAFSC). VAFSC has partnered with Tungsten Network e-Invoicing network, for submissions of all electronic invoices to Veterans Affairs (VA). Tungsten
Network electronic invoicing is free to all VA vendors. In order to submit electronic invoices, all VA vendors must register with Tungsten Network by submitting an email to
VA.Registration@Tungsten-Network.com or calling 1-877-752-0900 option 2 for
Enrollment.
d. Contractor shall submit an electronic invoice by the tenth (10th) of the following month services were performed to the Veterans Affairs Financial Services Center (VAFSC) e-
Invoice through the website at https://portal.Tungsten-Network.com/Login.aspx. For questions regarding the submission of VA electronic invoices, Tungsten Network customer service may be contacted at 1-877-489-6135.
e. All invoices shall reference the vendor name and address, customer name, contract number, appropriate obligation/funding order number, description of services provided, quantity, unit price, and total invoice amount (any additional info). Invoices shall also include any payment discount terms.
4. GOVERNMENT INVOICE ADDRESS: (See SF1449 Block 18):
a. All invoices from the contractor shall be submitted electronically in accordance with
852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2018)
b. Facsimile, e-mail, and scanned documents are not acceptable forms of submission for payment requests.
c. VA’s Electronic Invoice Presentment and Payment System – The Veterans Affairs
Financial Services Center (VAFSC) uses a third-party contractor, Tungsten Network e-
Invoice, to transition vendors from paper to electronic invoice submission. Please go to this website: http://www.tungsten-network.com/customer-campaigns/veteransaffairs/ to begin submitting electronic invoices, free of charge.
d. As of July 1, 2020, the VA10091 form has been eliminated and the VAFSC is requiring our vendors to update and add their information in the Customer Engagement Portal
(CEP) at https://www.cep.fsc.va.gov/. This is for new vendors being added to FMS only.

5. ACQUISITION OF COMMERCIAL ITEMS:
This Request for Quote (RFQ) solicitation is issued under the authority of FAR Part 13.5
Simplified Procedures for Certain Commercial Items in which simplified acquisition procedures in FAR Part 13 will be used. FAR Part 14 and 15 will not be used. From this

6. SITE VISIT:
It is strongly suggested and expected that the offeror inspect the campus to be serviced to fully understand the nature of the work and the conditions under which the work is to be performed. In no case shall failure to inspect the campus constitute grounds for a claim after contract award.

7. RFQ QUESTIONS:
In order to maintain integrity of this solicitation and subsequent award date, all offerors are advised that any question and answer requests must be submitted in a written form via e-mail to the Contracting Officer (CO) susan.pasholk@va.gov no later than TUESDAY APRIL 26, 2022, AT NOON LOCAL TIME. Contracting will issue a consolidated response via solicitation amendment to https://sam.gov Contract Opportunities within 2 business days to all parties
8. CONTRACTING OFFICER AUTHORITY:
The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting
Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitment or issue changes that will affect price, quantity or quality of performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred thereof.
9. IDENTIFICATION, PARKING, SMOKING AND VA REGULATIONS:
a. All VA property is considered Federal property and all Federal laws are applicable.
b. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VA will not invalidate or make reimbursement for parking violations of the Contractor under any conditions.
c. All VA facilities are drug free. Possession of drugs, to include Marijuana and all
Marijuana bi-products are prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the
United States (Federal) District Court, not a local district, state, or municipal court.
d. All contractor personnel are required to adhere to all VA rules and regulations.
e. Smoking is prohibited on the entire campus, both inside and outside the VA Facility https://sam.gov/
f. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the
United States (Federal) District Court, not a local district, state, or municipal court.
g. VHA DIRECTIVE 1192: This Veterans Health Administration (VHA) directive establishes policy and provides guidance for the prevention of seasonal influenza in VHA facilities through the vaccination or masking of health care personnel (HCP).
AUTHORITY: Title 38 United States Code (U.S.C.) 7301(b), 7318(b). Contractor is responsible in ensuring that all aspects of this directive are implemented, including providing face masks and influenza vaccines for all employees working at a VA facility.
10. VHA SUPPLEMENTAL CONTRACT REQUIREMENTS FOR COMBATTING COVID-19
1. Contractor employees who work in or travel to VHA locations must comply with the following:
a. Documentation requirements:
1) If fully vaccinated, contractors shall show proof of vaccination.
i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination
Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.
2) If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and
Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test that includes an affiliated telehealth service.
3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to
VA facilities and after entry for spot inspections by Contracting Officer
Representatives (CORs) or other hospital personnel.
4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.
a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.

4. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental
Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
11. SECURITY & PRIVACY CONTROL:
The Authorization & Accreditation (A&A) requirements of VHA Handbook 6500.6 do not apply to this requirement, therefore, a Security Accreditation Package is not required. No A&A or MOU/ISA is required. Sensitive information exposure as part of this contract involves applicable security controls within the facility as part of the VA Information Security

STATEMENT OF WORK/SPECIFICATIONS
CLEAN/TREAT AND TEST THE LOW AND MEDIUM TEMPERATURE HEATING
SYSTEMS
The Contractor shall provide all labor, supervision, materials, chemicals, tools, and equipment necessary to maintain, clean/treat and test the Low and Medium Temperature Heating Systems located on the
Clement J. Zablocki VA Medical Center (VAMC) Campus, Milwaukee, WI 53295, and in accordance with the Statement of Work (SOW).
1.0 DESCRIPTION OF SERVICES
A. Upon award of contract, contractor shall flush and clean low and medium temperature heat system to include all components listed in Section 4 Conformance Standards. Contractor shall provide cleansing chemicals to accomplish flushing and cleaning. Flushing and cleaning shall be coordinated with the Contracting Officer’s Representative (COR) and shall take place at a time mutually agreed upon by Contractor and COR.
B. Contractor shall test low temperature heating system monthly and add necessary chemicals to maintain correct system balance per Section 4 Conformance Standards. Upon award, contractor shall coordinate monthly schedule with COR.
C. In each contract year, contractor shall test medium temperature heating system monthly and add necessary chemicals to maintain correct system balance. Contractor shall coordinate monthly schedule with COR.
D. Contractor shall, in writing, report lab results to COR within five (5) business days after testing. Report shall be presented in a format mutually agreed upon by contractor and COR.
E. Contractor shall respond via telephone to emergency calls within four (4) hours in the event of an emergency to the low or medium heat systems. If emergency requires contractor to report to facility for additional testing, contractor must be on-sight within 24 hours of request.
Emergencies that require the contractor to be on-sight shall be paid separate from this contract on a separate purchase order that shall be obtained prior to contractor performing emergency service.
2.0 SAFETY DATA SHEETS
Prior to commencement of services, the Contractor shall provide Safety Data Sheets to the COR in accordance with VA and OSHA regulations.
3.0 HOURS OF OPERATION
A. Normal hours of operation for the Medical Center are Monday through Friday from 7:00 am to
3:30 pm, excluding National Holidays. All work shall be completed during normal business hours.

All services shall be performed in accordance with all VA Safety and Security standards, manufacturer’s standards, OSHA, and other applicable state, local, national, and industry standards.
5.0 REPORTS
A report shall be provided to the COR within five (5) business days after monthly treatments and testing occurs. The electric reports shall include lab results. Reports shall be submitted in format mutually agreed upon by Contractor and COR. In addition to the lab results, the reports shall include date and time of testing and name of technician performing the testing.
6.0 SERVICES BEYOND THE SCOPE OF WORK
The Contractor shall immediately, but no later than twenty-four (24) consecutive hours after discovery, notify the CO and COR (in writing) of the existence or the development of any defects in, or repairs required, to the scheduled equipment, which the Contractor considers he/she is not responsible for under the terms of the contract. The Contractor shall furnish the CO and COR with a written estimate of the cost to make any necessary repairs.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor 30 calendar days prior to the contract’s performance period end. The “specified” rates under this clause will be those rates in effect under the contract each time an option is exercised under this clause.

C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor 10 calendar days prior to the contract’s performance period end; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 calendar days prior to contract’s performance period end. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed FIVE (5) years.
C.5 SUPPLEMENTAL INSURANCE REQUIREMENTS
In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employer’s liability: Contractors are required to comply with applicable
Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least
$100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
(b) General Liability: $500,000.00 per occurrences.
(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.
(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.
