STATEMENT OF NEED

ZEISS ARTEVO 800 OPHTHALMIC MICROSCOPE

1. INTRODUCTION

1.1. The Corporal Michael J. Crescenz VA Medical Center (CVAMC) has a requirement for ZEISS Artevo 800 ophthalmic microscope for the Ophthalmology Department located at 3900 Woodland Avenue, Philadelphia PA. 19104. The ZEISS Artevo 800 ophthalmic microscope that is delivered to the Medical Center shall be the most current version available out in the marketplace.

2. BACKGROUND

2.1. A ZEISS Artevo 800 ophthalmic microscope is needed to allow proper visualization during ophthalmic surgical procedures in the operating room. The current microscope is old and is limited by dated technology. While using the current microscope patient safety is significantly compromised with severe lack of depth perception, poor visualization of critical detail along with differing views between supervisory and resident surgeon. The VA needs to procure a ZEISS Artevo 800 ophthalmic microscope that will enable multiple specialties within ophthalmology to continue performing procedures in-line with the standard of care.

2.2. The procurement of a ZEISS Artevo 800 ophthalmic microscope would provide ease of use for both the attendings and residents who operate at the CVAMC as the Operating Room (OR) has all ZEISS microscopes. The current model microscope that is being replaced has reached its end-of-life utilization. Without ZEISS Artevo 800 ophthalmic microscope the VA would have to utilize Community Care to perform retinal surgeries at a considerable cost to the VA for patient care and possible hospitalization.

2.3. The microscope will be shared among the entire ophthalmology team: Cataract, Retina, and Glaucoma. Both attendings and residents are familiar with the ZEISS family of microscopes and the OR stocks ZEISS disposables for the current set of microscopes.

3. SCOPE OF WORK:

3.1. VA anticipates award of a firm-fixed unit price from this solicitation.

3.2. Contractor shall deliver all items after receipt of award issued by the Government.

3.3. The Medical Center is looking for delivery and set up of the ZEISS Artevo 800 ophthalmic microscope with intraoperative OCT for the Ophthalmology Surgical Service in the Operating room located at 3900 Woodland Avenue, Philadelphia PA. 19104. Delivery activities shall consist of the following:

3.3.1. Set-up and/or any installation which includes all preparation required to ready the ZEISS Artevo 800 ophthalmic microscope for use.

3.3.2. Training of both end-users (Surgeons/Nurses) and Biomedical Engineering.

3.3.3. Post installation inspection to meet VA and vendor requirements.

4. INSPECTION AND ACCEPTANCE:

4.1. The warehouse located at 3900 Woodland Avenue, Philadelphia PA 19140 shall receive all equipment prior to Biomedical Engineering inspecting the equipment.

4.2. Disputes shall be resolved by the Contracting Officer.

5. DELIVERY/STORAGE REQUIREMENTS:

5.1. Deliver materials to job in manufacturers original sealed containers with brand name marked thereon or maintain coverings in place and in good repair if applicable.

5.2. Deliver specified items only when site is ready.

5.3. Store equipment in dry condition inside facility.

5.4. Any Government requested delayed delivery up to 90 days after initial delivery date, shall be at no additional cost to the Government.

5.5. PO# must be on outside of shipping box for easy identification.

5.6. Delivery Days are Monday through Friday during normal business hours which are defined as 7:30am to 3:00pm. National Holidays are excluded. Any delivery service performed for emergency orders may be delivered other than normal business work hours and must be approved.

5.7. The Contractor is not required to provide delivery services on the following National Holidays, nor shall the Contractor be paid for these Holidays and any other declared national holiday by the President.

5.8. The Contractor shall deliver all equipment in original shipping cartons to the Corporal Michael J. Crescenz VAMC located at 3900 Woodland Avenue, Philadelphia PA 19104 The Contractor shall coordinate all deliveries, staging areas, and parking arrangements with the VA Police and Warehouse personnel. It is the Contractors responsibility to remove all related shipping debris and clean-up any construction associated with delivery of specified items.

Contractor shall remove all packing from the Corporal Michael J. Crescenz VAMC premises.

Contractor is responsible for any damage to the building that occurs due to Contractor error or neglect.

6. ADDITIONAL DELIVERABLES:

6.1. Operation and Maintenance Manuals

6.2. Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packed in binder(s).

7. OPERATOR TRAINING:

7.1. Contractor shall provide technical training for Surgeons/Residents/Nurses and Biomedical Engineering. The cost for this training and all training related expenses such as travel, lodging, and other expenses are included in the contract price. This training for Biomedical Engineering shall cover the preventative maintenance, troubleshooting, and general maintenance of the equipment. Training shall allow for classroom instruction if applicable as well as individual hands- on programming, troubleshooting and diagnostic exercises.

7.2. This training program shall include at a minimum following: Technical Training, Operational Training, and Hardware/Software trouble shooting. Training shall occur onsite during normal business hours, 8:00am – 4:30pm, Monday through Friday.

7.3. Scheduling of all training shall be coordinated with the Medical Center after delivery is complete.

8. ASSEMBLY:

8.1. Contractor shall provide all labor, materials, tools, equipment etc. to prepare for the assembly of the equipment at the Corporal Michael J. Crescenz VA Medical Center.

8.2. All employees of the general contractor and subcontractors shall comply with CVAMC policies including but not limited to vendor credentialing, parking, check-in procedures, name badges, smoking policies, and confidentiality.

8.3. Contractor is responsible for removing all packing materials and related waste and dispose of off-site.

9. POINT OF CONTACT:

10. WARRANTY:

10.1. The Contractor shall provide at least a one-year manufacturer’s warranty on all parts. Any recalls to the equipment, the vendor agrees to replace within three business workdays without any additional cost to the government. The warranty shall include shipping costs associated with any warranty repair.

11. VHA Supplemental Contract Requirements for Combatting COVID-19 dated 26 January 2022

1. Contractor employees who work in or travel to VHA locations must comply with the following:

a. Documentation requirements:

1) If fully vaccinated, contractors shall show proof of vaccination.

i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.

2) If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test that includes an affiliated telehealth service.

3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.

4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.

a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.

4. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
