All Interested Parties :

This Request for Quote (RFQ) is to provide substance abuse treatment and mental health treatment services for offenders held underauthority of the United States in Cedar Rapids, Iowa area.

Services are to be provided in accordance with the Federal Bureau of Prisons Community Transitional Drug Abuse Treatment and Mental Health Treatment Services Statement of Work (April 2011 - No Master ' s Required), Revision 01, April 2013 , PSB" .

The following must be completed and submitted with your Proposal:

Standard Form (SF) 1449 - Solicitation/Contract/Order for Commercial Items - · Please compl ete blocks 12, 17a, 17b , 23, 24, 30a, 30b, and block 30c of the SF-1449 . Also , please complete the attached continuation sheets. Please include your telephone number in Block 17a . Original signatures are required.

Representations and Certifications (FAR 52.212 - 3) - Prospective quoters are required to submit Annual Representations and Certifications via the Online Representations and Certifications Applicat ion (ORCA) website at https : //www . sam . gov/portal/public/SAM/ . By submission of a quote under this solicitation , the quoters acknowledges completion of the annual representations and certifications via ORCA and verifies , by the submission of the quote , that the representations and certifications currently posted in SAM were entered or updated within the last 12 months and are current, accurate, complete and applicable to this solicitation. All quoters must be capable of and comply with FAR 52 . 212-1(j), Instructions to Quoters - Commercial Items, Data Universal Numbering System (DUNS) Number (JULY 2013) and FAR 52 . 212-4 (t), Contract Terms and Conditions - Commercial Items, System for Award Management (SAM), (SEP 2013).

The Government shall award to the lowest priced, technically acceptable, responsive and responsible quoter . Award will be decided based on the contents of your solicitation response, therefore be sure that all requirements have been satisfied .
