Item Description Quantity Unit of Issue Unit Price Total Price
1. Laboratory Testing 12 Month B.2 Period Of Performance
The performance of this purchase order shall be from Date of Award thru December 31, 2019 B.3 Statement of Work The Blackfeet Service Unit laboratory requires certain types of patient testing that we are unable to perform in house, but are required by state law or are part of a public health surveillance program. For example, each state requires PKU testing on all newborns. Blackfeet Service Unit’s contracted reference lab does not perform this test nor does it report positive emidemilogy results or congenital defects to the State of Montana. Other examples of specialty specimen testing include but are not limited to Acid-Fast Bacillis, Neisseria gonorrhea, ova and parasites, viral cultures, etc.
1. PKU (Phenylketonuria) testing for newborns is required by the State of Montana and results are required to be reported to the State of Montana.
2. On a daily basis, patient samples of various types are sent for testing. The Blackfeet Service Unit sends tests to the vendor for testing and results are reported back to our facility via phone, fax and mail. As part of the required reporting for communicable diseases, if vendor gets a positive result, they phone the results back to our laboratory, we in turn notify the Community Health Nurses. The contracting laboratory will be required to submit the results to the State of Montana as part of the the public health surveillance program.
3. Without the ability to send the various types of specimens to a certified lab testing facility, numerous types of specialty testing would not be available to our patients, and the quality of patient care would be hindered. Also, Blackfeet Service Unit would not be compliant with the state requirment for PKU testing of newborns.
4. Results turnaround time is maximum of 14 days for PKU and 7 days for epidemiology results.
B.4 COMPLEXITY
Patient specimens will be sent to lab testing facility on an as needed basis. The vendor will return patient test results to our lab via mail, fax or phone. Positive tests that are on the communicable disease listing will require phone reports to our facility. The vendor will supply our facility with pre-printed mailing labels, test request forms, biohazard zip-lock bags for specimens and where indidcated, specimen containers and transport media. Courier service to pick up specimens on a daily basis will be provided, as indicated by the lab. Vendor will also report epidemiology results and congenital defects to the State of Montana database.
B.5 PURPOSE OF CONTRACTS
The purpose of this contract is to secure necessary timely specialty specimen testing that cannot be performed at the Blackfeet Community Hospital Laboratory. Vendor will also report epidemiology results and congenital defects to the State of Montana database.
