B.1 PRICE SCHEDULE – CISCO VoIP TELEPHONE SOLUTION
Furnish and install a Cisco VoIP Phone System at the Crow/Northern Cheyenne Indian Hospital, Crow Agency, Montana; Lodge Grass Health Center, Lodge Grass, Montana; and Pryor Clinic, Pryor, Montana; and upgrade the Cisco equipment at the Billings Area Office, Billings, Montana to support the network infrastructure.
The procurement is conducted pursuant to the authority of Federal Acquisition Regulation (FAR) Subpart 13.5, Test Program for Certain Commercial Items (41 U.S.C. 3305, 3306, and Chapter 37); and FAR Part 12, Acquisition of Commercial Items (41 U.S.C. 1906 and 1907)).

B.3
PURPOSE OF CONTRACT
The mission of the Billings Area Indian Health Service is to provide the best possible health care services to the Native American population within the Billings Area. To meet the mission, the Crow/Northern Cheyenne Hospital in Crow Agency, Montana requires contractor support to upgrade the current telephone system to a VoIP telephone solution.
The VoIP equipment must be Cisco due to the existing IOS platform, existing knowledge base, and because the existing Indian Health Service helpdesk support is provided by Cisco certified technicians.
Items quoted must be NEW. No grey market, refurbished, used, or third party products will be accepted. The contractor shall source only new Cisco products directly from Cisco or through the U.S. distribution channels that are only authorized by Cisco. Products obtained outside these channels may require additional costs associated with product relicensing and inspection fees before Cisco will enter into warranty/service support for the product in question.
In the event there are questions pertaining to the validity of the Cisco products, the Government reserves the right to verify the origin of the Cisco products with Cisco Systems, Incorporated. In the event the products have been acquired from unauthorized channels, the Government further reserves the right to return the products for a full refund.
B.4
SCOPE OF WORK
Current Telephone System:
Crow/Northern Cheyenne Hospital
The current telephone system is a Nortel Option 61 with dual CPU’s with a Meridian voicemail system. Crow Hospital has 201 existing phone sets, 25 sets at Pryor Clinic and 25 sets at Lodge Grass Clinic. There are two T1 circuits to Project Telephone for incoming DID calls and all outgoing calls. Also connected to the phone system is a public address (PA) system. Current phone system is not VoIP and is not interfaced into the IHS LAN/WAN networks.
The contractor shall furnish all labor, equipment and software (as listed below by location), materials, and technical expertise necessary to perform and complete the work described in this scope of work, specifications, plans or other instructions provided, herein.
The contractor must provide and install the equipment, warranties, licenses and training from Cisco as listed below in the “Equipment Requirement” section.
B.5
LICENSING
Telephone Station Set quantities and Unified Workspace Licensing listed below may vary, depending on the type of device and/or user quantities. Any additional licenses being added after award of contract will be done via contract modification. The Indian Health Service reserves the right to modify the telephone set and license quantities at the time of purchase.
The contractor will be responsible for purchasing the new equipment, software, and licenses as specified in the Scope of Work. Equipment, software and required licenses provided shall be compatible with this VoIP implementation project and the provision of a fully operational system. The contractor shall ensure that the equipment, software and license ownership shall transfer to the Indian Health Service at the completion of this project. The contractor will also supply all warranty and maintenance agreements for supplied equipment, software and licenses in the name of the Indian Health Service.

B.7
PROJECT MANAGEMENT
The contractor shall identify project team members and their individual rolls. The contractor shall supply a complete description of the key activities and responsibilities required for the upgrade. A master project schedule and plan must be included, along with a work responsibility matrix, identifying the tasks the contractor will perform and the tasks the government is expected to perform to successfully implement this project.
B.8
DELIVERABLE
The contractor shall provide the Crow/Northern Cheyenne Service Unit with administrative and training manuals, CDs, et cetera.
B.9
FUNCTIONAL REQUIREMENTS
The VoIP implementation solution shall be fully functional and compatible with the current network LAN/WAN equipment and infrastructure.
B.10
ADDITIONAL REQUIREMENTS
The Contractor shall:
Perform on-site requirements gathering to capture details about the existing environment as well as the desired features and functionality for the proposed solution.
Provide customer with a pre-implementation checklist detailing any recommendations for, or required changes to, the existing network/virtual environment.
Create a design document deliverable detailing the solution to be deployed and the method for deploying it.
Develop a system test plan to validate the functionality of the installed solution.
Review the design document and system test plan with the customer.
Deploy two ea. Cisco BE6M servers and install Cisco Unified Communications Manager, Unity Connection, InformaCast, and SRST Manager.
Migrate user data from legacy CUCM 7.x environment, cutover existing sites to the new cluster, and provide Day One support.
Deploy E-SRST at Crow, Pryor, and Lodge Grass sites and provision voice gateways for PSTN connectivity.
Deploy up to 305 phones to the desktop.
Cutover Crow, Pryor, and Lodge Grass sites and provide Day One support.
Provide knowledge transfer to the customer for the as-built solution.
One day 'train-the-trainer' session to cover high level features and functionality and enable the customer to support end users.
B.11
TRAINING AND ASSISTANCE
The contractor will provide a guidance/training and assistance plan for review and approval prior to the award of contract.
The contractor will provide remote assistance for minimum of 30 days commencing with the first day the system starts receiving live calls.
B.12
INSTALLATION TIMELINE
Equipment Staging: The contractor will perform staging of hardware and software. The contractor will work with Indian Health Service to ensure that all equipment has been received and inventoried. During this staging process the contractor will inventory, assemble, connect, power cycle, configure, and test the new equipment and software based on solution determined by site survey.
B.13
PERSONAL QUALIFICATIONS
A Gold Certified, Master Unified Communications Specialized Cisco Partnership or equivalent. The contractor must be license/certified in Cisco VoIP equipment, software, and installation.
The onsite lead installation technician must be CCIE certified or equivalent.
B.14
COMPLIANCE WITH SECTION 508
Section 508 Program Need
Requirements for accessibility based on Section 508 of the Rehabilitation Act of 1973 (29 U.S.C. 794d) are determined to be relevant for the following program need: "IP Telephone."
Section 508 Deliverable Requirements
Technical standards from 36 CFR part 1194 Subpart B have been determined to apply to this acquisition. Offerors must describe how their proposed Electronic and Information Technology (EIT) deliverables meet at least those technical provisions identified as applicable in the Product/Service Accessibility Template (PAT) at http://www.hhs.gov/web/section-508/contracting/technology-products/product-accessiblity-template/index.html.
Functional performance criteria from 36 CFR part 1194 Subpart C have been determined to apply to this acquisition. Offerors must describe how their proposed Electronic and Information Technology (EIT) deliverables meet at least those functional performance criteria identified as applicable in the Product/Service Accessibility Template (PAT).
Information, documentation, and support requirements from 36 CFR part 1194 Subpart D have been determined to apply to this acquisition. Solicitation respondents must describe how the information, documentation, and support proposed for Electronic and Information Technology (EIT) deliverables meet at least that information, documentation, and support requirements identified as applicable in the Product/Service Accessibility Template (PAT).
Section 508 Evaluation Factors
Responses to this solicitation will only be considered for award after it has been determined that the proposal adequately addresses the requirements for Section 508. Only proposals which contain adequate information to document their responsiveness to the Section 508 requirements (e.g. a completed PAT or equivalent and supporting documentation) will be eligible for any additional merit consideration.
Section 508 Acceptance Criteria
IP Telephones delivered as a result of this solicitation will be accepted based in part on satisfaction of identified Section 508 requirements for accessibility. IP Telephones delivered must include a completed PAT.
B.15
WORKING CONDITIONS
Work is performed in the telephone and server rooms, which are adequately lighted, and ventilated. Due to the nature of the equipment, the telephone and server room temperatures are below room temperature.
B.16
QUALITY ASSURANCE
Periodic inspections will be completed by the Indian Health Service technical representative.
B.17
WORK SCHEDULE
The facilities regular business hours are Monday through Thursday, 8:00 a.m. to 5:00 p.m., and Friday from 8:00 a.m. to 4:30 p.m., excluding federal holidays.
System downtime must be scheduled during non-business hours whenever feasible. The contractor must ensure that the work is scheduled in such a manner that will have minimal effect on patient care and business functions.
The contractor will provide the Indian Health Service with an installation time line and milestone plan, starting with installation and submit to the Crow/Northern Cheyenne Service Unit Site Manager.
The contractor’s personnel will not be permitted to leave the installation to perform work required by service calls that do not pertain to this contract.
B.18
TIMELINE
Installation will begin no later than three (3) weeks after contract award.
Performance of the contract shall be completed no longer than six (6) months after award.
B.19
WARRANTY AND MAINTENANCE
Cisco – SMARTnet Maintenance (Hardware and Software) components
All Cisco equipment as part of the proposal will include one year of SMARTnet Maintenance on all hardware and software components.
The Cisco SMARTnet components will include but not be limited to the following descriptions.
(1) Cisco Operating System (Including Cisco IOS®) Software Updates
Cisco will respond quickly to changes in networking environments and provides immediate Cisco system software updates (such as IOS), thus incorporating technology refreshment into the standard maintenance package. SMARTnet will provide automatic updates, on request, of all major releases, with sophisticated upgrade planning processes, as well as maintenance fixes.
(2) Cisco Customer On-line (CCO) Access
Through www.cisco.com software updates must be available 24 hours a day, seven days a week. The Indian Health Service should have the capability to search, access, and download software and firmware upgrades at Cisco’s customer web location.
(3) Advanced Replacement on Hardware
In the event of hardware failure, the SMARTnet Advance Replacement feature must provide a hardware replacement on failed parts the next business day.
(4) Technical Assistance Center (TAC) The Indian Health Service must have access to the single point of contact for all Cisco software and hardware maintenance and support services through the Cisco Technical Assistance Center (TAC). The Cisco TAC must be available 24 hours a day, seven days a week via telephone (1-800-553-2447), electronic mail, or www.cisco.com case submission tool. The Indian Health Service must have access to the customer support engineers with Cisco product line and communications networking technology experience.
B.20
CONTRACTING OFFICER AUTHORITY
Authority to negotiate changes in terms, conditions, or amounts cited in this contract is reserved to the Contracting Officer.
B.21
CONTRACTING OFFICER’S REPRESENTATIVE (COR)
The Contracting Officer shall designate a COR, in writing, at time of award. The COR shall be responsible for:
a. Monitoring the Contractor’s technical progress;
b. Interpreting the statement of work;
c. Technical evaluation as required;
d. Technical inspections and acceptance required by the contract; and,
e. Assisting the Contractor in the resolution of technical problems encountered during performance of the contract.

b. The Contractor agrees to include the following information on each invoice:
(1) Contractor’s name and address;
(2) Contact name, title, and telephone number;
(3) Contract Number;
(4) Invoice number and date;
(5) Description, quantity, unit of measure, unit price, and extended price of supplies delivered or services performed;
(6) Remit to Address;
(7) Taxpayer Identification Number (Employer Identification Number or Social Security Number); and
(8) Data Universal Numbering System (DUNS) number (to ensure payment is made to the correct bank account) and vendor mailing address, as provided in the System for Award Management (SAM), formerly known as the Central Contractor Registration (CCR) database.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

SECTION E - SOLICITATION PROVISIONS
E.1
52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2015)
(a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees.
(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show—
(1) The solicitation number;
(2) The time specified in the solicitation for receipt of offers;
(3) The name, address, and telephone number of the offeror;
(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;
(5) Terms of any express warranty;
(6) Price and any discount terms;
(7) “Remit to” address, if different than mailing address;
(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);
(9) Acknowledgment of Solicitation Amendments;
(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and
(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender’s request and expense, unless they are destroyed during preaward testing.
(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions or commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.
(f) Late submissions, modifications, revisions, and withdrawals of offers.
(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and—
(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or
(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government’s control prior to the time set for receipt of offers; or
(C) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.
(g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.
(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer.
(i) Availability of requirements documents cited in the solicitation.

(4) Nongovernment (voluntary) standards must be obtained from the organization responsible for their preparation, publication, or maintenance.
(j) Data Universal Numbering System (DUNS) Number. (Applies to all offers exceeding $3,500, and offers of $3,500 or less if the solicitation requires the Contractor to be registered in the System for Award Management (SAM) database.) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation “DUNS” or “DUNS+4” followed by the DUNS or DUNS+4 number that identifies the offeror’s name and address. The DUNS+4 is the DUNS number plus a 4-character suffix that may be assigned at the discretion of the offeror to establish additional SAM records for identifying alternative Electronic Funds Transfer (EFT) accounts (see FAR Subpart 32.11) for the same concern. If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. An offeror within the United States may contact Dun and Bradstreet by calling 1-866-705-5711 or via the internet at http://fedgov.dnb.com/webform. An offeror located outside the United States must contact the local Dun and Bradstreet office for a DUNS number. The offeror should indicate that it is an offeror for a Government contract when contacting the local Dun and Bradstreet office.
(k) System for Award Management. Unless exempted by an addendum to this solicitation, by submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the SAM database accessed through https://www.acquisition.gov.
(l) Debriefing. If a post-award debriefing is given to requesting offerors, the Government shall disclose the following information, if applicable:
(1) The agency’s evaluation of the significant weak or deficient factors in the debriefed offeror’s offer.
(2) The overall evaluated cost or price and technical rating of the successful and the debriefed offeror and past performance information on the debriefed offeror.
(3) The overall ranking of all offerors, when any ranking was developed by the agency during source selection.
(4) A summary of the rationale for award;
(5) For acquisitions of commercial items, the make and model of the item to be delivered by the successful offeror.
(6) Reasonable responses to relevant questions posed by the debriefed offeror as to whether source-selection procedures set forth in the solicitation, applicable regulations, and other applicable authorities were followed by the agency.
