CFAS Training Kits

This is a combined synopsis/solicita�on for commercial items prepared in accordance with the format in Subpart 12.6 and FAR Part 13, SAP, as supplemented with addi�onal informa�on included in this no�ce.

This announcement cons�tutes the only solicita�on; quotes are being requested and a writen solicita�on will not be issued.

Scope of Work: This project includes the purchase of CPR First Aid Training Kits with Aneva Face Shields as specified in Atachment 2.

- Bidding suppliers must supply a detailed photo of the training items in the clear plas�c bag.

- All shipped containers/boxes must be clearly marked on the outside with the contents name, i.e.

“CPR FIRST AID Training Kit”.

- All shipped containers/boxes must not exceed 100 CPR Training kits per container/box.

Brand Name or Equal Vendor may submit quotes for alternate brand name items, provided must meet at the minimum each salient physical, func�onal, or performance characteris�cs specified by this solicita�on. Vendor MUST provide exactly what they are offering (including make, model and descrip�on). Please see Evalua�on for Award for details for submission of alterna�ve items. The government will evaluate 'equal' items on the basis of informa�on furnished by the Vendor or iden�fied in the quote and reasonably available to the government. The government is not responsible for loca�ng or obtaining any informa�on not iden�fied in the quote.

Set-Aside Requirement This solicita�on is set aside for small business in the open market. This solicita�on requires registra�on with the System for Award Management (SAM) prior to award, pursuant to applicable regula�ons and guidelines. Registra�on informa�on can be found at www.sam.gov.

Special Delivery/Installa�on Instruc�ons Delivery: 30 Days ARO. The offeror must provide within its offer the es�mated delivery date or the number of days required to make delivery a�er it receives a purchase order from the buyer.

Shipping Condi�on Shipping must be free on board (FOB) des�na�on CON US (Con�nental U.S.), which means that the seller must deliver the goods on its conveyance at the des�na�on specified by the buyer, and the seller is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified des�na�on.

Equipment Condi�on New Equipment ONLY; No remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty.

Evalua�on for Award - (Lowest Price Technically Acceptable (LPTA))

The Government will award a firm fixed price (FFP) contract resul�ng from this solicita�on to the responsible Offeror whose offer conforming to the solicita�on as Lowest Price Technically Acceptable.

The following factors shall be used to evaluate offers: price, technical acceptance, and delivery of the item offered to meet the Government requirement. Technical Evalua�on will be performed on all alterna�ve end products and delivery.

Offeror must present a line-by-line comparison of alterna�ve products using manufacturer name, model/part number, and salient features for all alterna�ve end products. In order to be deemed technically acceptable, offerors must meet all the requirements provided in the solicita�on to include, but not limited to, the end-product offered and delivery.

Offeror Atachments (Shall be returned as part of your quote to be considered acceptable).

In addi�on to providing pricing, Offeror shall include certain non-pricing informa�on as document(s) atached to their quote, so they are received no later than the closing date and �me of this RFQ.

Atachment(s) can total no more than 20 MB, whether mul�ple files, or one file, and may be zipped to decrease their size. An Offeror's failure to comply with these terms may result in its quote being determined incomplete and shall not be evaluated.

Items to Return:

1-Vendor must return the RFQ with Atachment 1 (page 3) of this RFQ signed with pricing matrix completed. Vendor must complete the columns for BRAND NAME MAKE AND MODEL OFFERED, UNIT PRICE, and EXTENDED PRICE. Signature implies that the vendor understands the buy terms for the solicita�on. Delivery cost must be included in pricing.

2- Vendor shall return a quote on company’s official document that includes SAM.gov’s UEI. Pricing shall be included for the unit pricing and extended pricing to be evaluated.

3-Vendor supply a detailed photo of the training items in the clear plas�c bag specified under paragraph 6 Product Guidelines.

4- Alterna�ve Brand offer: Vendor shall return any alterna�ve brand name manufacturer’s specifica�on sheets with a line-by-line comparison to the requested brand name’s manufacturer’s specifica�on sheet in order to be evaluated for award. Failure to provide a comparison will make the offer unaccepted and the offer will not be evaluated for award.

Acceptance of an award cer�fies that the contractor will comply with FAR 52.222-50 Comba�ng Trafficking in Persons by no�fying all employees and subcontractors at all levels.

Atachment 2: Scope of Product Requirements

Mandatory

1. Bidding suppliers must supply a detailed photo of the training items in the clear plas�c bag.

2. Subs�tu�ons of individual products are not allowed. Items are mandated. Items listed are brand name or equal.

3. Products must be safe for use in all 50 states.

4. All shipped containers/boxes must be clearly marked on the outside with the contents name, i.e.

"CPR FIRST AID Training Kit".

5. All shipped containers/boxes must not exceed 100 CPR Training Kits per container/box. Product Guidelines

6. The CPR First AID Training Kit consists of the following:

A. Clear Plas�c Bag

1. Clear, non-vacuum sealed and re-useable.

2. Bag must have a large s�cker or labeled "CPR FIRST AID TRAINING ONLY".

3. Requested training items must be placed and shipped in the clear bag.

B. Emergency Bandage, White, 1 each *BRAND NAME OR EQUAL*

1. Non-adherent pad, vacuumed sealed.

2. Built in Pressure Applicator Bar / C Clip, with teeth.

3. Built in Closure Bar with teeth.

4. Sterile FDA approved modular bandage with an 8-year expira�on date.

5. Built in Sleeve for self-applica�on.

6. Dimensions: Bandage Width - 4 inches wide x 7 inches long. Total length un-stretched is 63 inches.

1. Sterile An�sep�c alcohol wipes contain 70% Isopropyl Alcohol for rapid-ac�on skin cleansing.

2. 1.18" x 1.1" size wipes are ideal for pre-injec�on skin prepping and cleaning minor cuts and scrapes.

3. Non-woven pad fabric is so� and absorbent.

4. Pad can be unfolded for greater coverage.

5. Each wipe is individually sealed for convenience and safety.

G. Aneva Disposable CPR MANIKIN Training Face Shields, 3 each *BRAND NAME OR EQUAL*

1. Designed to OEM specifica�ons, these shields are made of vinyl so they'll drape over the manikin's face and lay flat, unlike the plas�c shields which o�en curl up during use.

2. These easy-to-use vinyl face shields, with flat one-way breathing filters, protect the student from contaminants during CPR training.
