Subject: Solicitation 36C25721Q0003, Laser Service for Dallas, Texas VAMC - Request for Quote

Prospective Offers:

The Dallas Texas VA Medical Center invites your firm to submit an offer regarding the subject project.

Please note that the Dallas Texas VA Medical Center anticipates this contract to have a Period of Performance: Base + 4 Option Years: Anticipated start 10/01/2020.

The Government intends to make an award of a firm-fixed contract to a responsible contractor whose offer, conforming to the solicitation; will be most advantageous to the Government utilizing the evaluation process specified in FAR part 13.5

Please submit an offer in accordance with the instructions on page 4 & 45 of the enclosed solicitation. The offeror shall complete sections 12, 17A, 30(a) (b) and (c) on the attached SF 1449, Solicitation/Contract/Order for Commercial Items and return it to this office along with the other requested documents specified within the solicitation. Additionally, please ensure to complete the Contract Administration Data B.1 on page 3 and the Price/Cost Schedule in its entirety beginning on page 30 of the solicitation. Lastly, please ensure you read and understand the Evaluation Factors information beginning on page 45.

Your reply to this request for quote must set forth full, accurate, and complete information as required by this solicitation.

Your complete offer shall be submitted via email on or before September 11, 2020 at 0900 a.m. CST.

The last day for Requests for Information/Questions shall be 0900 a.m. CST on September 7, 2020. After that date, no further questions will be answered.
