This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested in response to this notice and a written solicitation will NOT be issued. This solicitation is being issued as a Request for Quote (RFQ). Submit only written quotes for Hologic Trident HD System. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05. This solicitation is not a 100% Small Business set-aside. The associated NAICS code is 339112.

PERIOD OF PERFORMANCE: Deliver 30 days ARO

Vendor Requirements:

a. Installation of Trident HD, Atec Sapphire and Eviva Adapter Kit will be completed by a Hologic certified field engineer.

b. On-site training will be completed by a certified Hologic field engineer

Evaluation: FAR 52.212-2 Evaluation -- Commercial Items (completed as follows):

(a) The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

No “Or Equals” as this machine is the second part of a complete Trident System.

This solicitation will result in a firm fixed price purchase order pursuant to the terms and conditions below. Terms and conditions other than those stated will not be accepted. The above pricing is all inclusive.

1. General Requirements

1.1 Description of Requirements

Claremore Indian Hospital (CIH) Radiology Department seeks to procure a Hologic TridentHD Radiography System a Atec Sapphire 100 Breast Biopsy System for Stereotactic Breast Biopsy and Eviva Adapter Kit to satisfy the remainder of needs to provide Stereotactic biopsies at CIH.

1.3 Specifications

a. Hologic Trident HD Radiography System: A digital cabinet X-Ray system that provides sharp, high quality images of core biopsy and surgical specimens, for rapid verification and confidence at the point of care. Eliminates transporting the specimen to another room for imaging. Eliminates waiting for margin confirmation. The only specimen radiography system to take advantage of amorphous selenium direct capture imaging to generate crisp, clear, high resolution biopsy images that inspire confidence. Same detector technology and compatible with the current Dimensions® Mammography System in use in the Mammography Department.

b. Atec Sapphire 100 Breast Biopsy System for Stereotactic: ATEC vacuum-assisted breast biopsy system is an all-in-one platform intended for use under ultrasound, MRI, stereotactic and 3D™-image guidance. Fast tissue acquisition occurs every 4.5 seconds, typically lasting less than 30 seconds total. Easily delivers local anesthetic directly to the biopsy site during every cycle without interruption. Combination of saline lavage and consistent aspiration helps ensure a core with every cycle. Safe, closed system reduces contamination risk. Fully disposable device and tubing minimize staff and patient exposure to biohazard. Simple one-minute setup and clean up. No software required for user to program or operate the console. Feature saline lavage helps ensure consistent retrieval of high quality tissue samples. Y-Valve enables physicians to automatically or manually deliver local anesthetic directly to the biopsy site without interrupting the procedure.

a. The Indian Health Service requires the contractor to provide all tools, parts and labor for this contract.

b. Installation of Trident HD, Atec Sapphire and Eviva Adapter Kit will be completed by a Hologic certified field engineer.

c. On-site training will be completed by a certified Hologic field engineer
