BASEWIDE TREE TRIMMING & REMOVAL
SERVICES

APPENDIX 1: Environmental Compliance Requirements
Part 1 General Information
1. GENERAL: This is a non-personal services contract to provide Basewide Tree Trimming & Removal Services for Vandenberg AFB, CA.
1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Basewide Tree Trimming & Removal Services as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract.
1.2 Background: Tree trimming and/or removal is required to support various missions of Vandenberg Air Force Base (VAFB) infrastructure. The contractor shall perform tree trimming and/or removal of trees located in electrical overhead distribution lines and those that pose a safety concern to facilities and occupants. Various dead, decaying and diseased trees pose a threat to life, health, and safety along commonly travelled routes. Tree overgrowth poses a significant threat to electrical distribution lines and infrastructures, in many cases, the cause of unscheduled power outages affecting base mission and causing numerous delays with space, missile launch operations, and other critical activities that require reliable power and causing damage to facilities and base’s infrastructures.
1.3 Objectives: To provide tree trimming and/or removal services on VAFB. The contractor shall provide tree services requested by the Government on various types of trees located on and around VAFB. The contractor shall comply with all Federal, State, Local and VAFB rules and regulations. Performance of services shall follow commercial industry standards as described by professional organizations such as the National Arborist Association, American Society of Landscape Architects, and American National Standards Institute (ANSI). Some tree services will encompass tree lines on or near high voltage electrical distribution on North, South and Main Cantonment of VAFB.
1.4 Scope: The contractor shall provide all management, tools, supplies, equipment, and labor necessary to ensure that Basewide Tree Trimming & Removal Services are performed at Vandenberg Air Force Base (VAFB) in a manner that will maintain a healthy, thriving, attractive and professional appearance of tree landscapes.
1.5 Period of Performance: The period of performance shall be for twelve (12) months after contract award.
1.6 General Information
1.6.1 Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract. The contractor’s Quality Control Plan (QCP) is to be delivered within 30 days after contract award. Two (2) copies of a comprehensive written QCP shall be submitted to the Contracting Officer (CO) and Contractoring Officer’s Representative (COR) within five (5) business days when changes are made thereafter. After acceptance of the quality control plan the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his QC system. The QCP must contain, at a minimum, the items listed below:
1.6.1.1 A description of the contractor’s inspection system which should cover, at a minimum, all requirements listed in the Services Summary. Description shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, and the title and organizational placement of the inspectors.
1.6.1.2 A description of the methods to be used for identifying and preventing defects in the quality of service performed.
1.6.1.3 A description of records utilized to document inspections and corrective actions taken.
1.6.1.4 The contractor’s inspection records shall be kept throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract. Upon request, the contractor shall immediately make these records available to the Government for review/inspection.
1.6.2 Quality Assurance: The government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).
1.6.3 Recognized Federal Holidays: The contractor is not required to perform services on recognized federal holidays unless otherwise provided specific written authorization by the CO to work special hours.

1.6.4 Hours of Operation: The contractor is responsible for conducting business, between the hours of 06:30 AM PST to 4:00 PM PST, Monday through Friday, except recognized federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.
1.6.5 Place of Performance: The work to be performed under this contract will be performed at Vandenberg AFB, CA.
1.6.6 Type of Contract: Reserved.
1.6.7 Security Requirements: At all times while on Vandenberg AFB premises, contractor employees shall comply with Vandenberg AFB security requirements imposed by the Installation Commander. The contractor shall obtain all necessary passes, decals, badges, or other items required for access to perform contract services at Vandenberg AFB. The contractor shall be responsible for accountability of all such media issued to contractor personnel and these media shall be surrendered to the 30th Security Forces Squadron (30 SFS) Pass and Registration Section upon completion or termination of the contract or upon termination of an individual’s employment with the contractor. The contractor shall comply with all VAFB policies regarding entry to the installation, rules of the road, contingency operations compliance, and photography prohibitions. Contractor employees are authorized entry to the installation only for the purpose of work under this contract, and are not authorized in other facilities or areas without prior Government authorization. Due to safety/security requirements, contractor personnel shall be required to fluently speak, understand, read, and write English.
1.6.7.1 Main Base Access: Current security access restrictions at the Santa Maria Gate (Main Base Entry Gate) require all contractor vehicles (which are the size of a standard bread delivery truck or larger) to enter only through the Lompoc Gate (located at intersection of Santa Lucia Road and Pine Canyon Road). There is a vehicle check pull-off area at that gate and the contractor will be required to submit to a vehicle search prior to gaining entry to the base.
Contractor should plan and allow for delays as a result of in-place security procedures.
1.6.7.2 Base Access Letter: To request base access, the contractor shall send a letter to the Contracting Officer. The letter must be on company stationery and shall include: Contractor’s company address and telephone number, the contract number, names of the Contractor Program (On-site) Manager and Contractor Security Manager, and their alternates, requiring access to Vandenberg AFB, their social security numbers, and duty hours. After approval, this letter will be maintained on file with Security Forces at the Visitor’s Control Center, Building 17596.
Contractor shall provide this letter at the pre-performance conference and submit updated letters as changes occur. Entry requirements are subject to change, without notice, due to installation and world situations.
1.6.7.3 Contractor Personnel: Upon employment, and after the contractor’s base access letter is on file with the Security Gorces, all contractor employees shall apply for vehicle passes and individual identification badges at the Vandenberg AFB 30 SFS, Visitor's Control Center, Building 17596. Passes may be picked up Monday through Friday prior to 3:30 PM PST. No appointment is necessary. The contractor shall surrender all passes and identification badges to the 30th SFS on the first workday following completion or termination of the contract or contractor’s employee’s employment.
1.6.7.4 Security Manager: Upon contract award, the contractor shall designate, in writing to the Contracting Officer, a Security Manager who shall be the contractor’s point of contact for all security matters. Within five (5) working days of contract award, the Contractor’s Security Manager shall contact the 30th CES Unit Security Manager (in the CES compound located at 1172 Iceland Ave) for assistance in accomplishing contractor personnel security requirements.
1.6.7.5 Contractor personnel are required to attend security and OPSEC orientation under HSPD Title 12. Contractor shall schedule initial and annual refresher training (for all assigned contractor personnel) through the 30th CES Unit Security Manager.
1.6.7.6 All contractor personnel shall be thoroughly briefed by the 30th CES Unit Security Manager on established security requirements and procedures as stipulated in Air Force Policy Directive (AFPD) 31-1 Physical Security Program.
1.6.7.7 The contractor shall brief all personnel that they shall immediately leave any area upon notification by the Contracting Officer, COR, constituted authority, or the Security Forces, when directed, as occasional operational requirements, real world activities, or emergencies dictate.
1.6.7.8 The 30th CES Unit Security Manager is the source for the necessary forms and will provide instructions for receiving, preparing, and processing these forms. After forms have been properly completed, the 30th CES Unit Security Manager will make an appointment for contractor employee(s) to go to the Security Forces for submittal of paperwork and fingerprinting. Appointments will be scheduled within two weeks of request.
1.6.7.8 A minimum of 180 days is required to process contractor employee NACs. Additional time will be required if the NAC is unfavorable and may result in denial of entry to restricted/controlled areas or denial of base LAN access. The 30th CES Unit Security Manager will notify the contractor of any negative NAC results. After completion of a favorable NAC, each eligible employee shall report to the 30th CES Unit Security Manager for processing of AF Form 2586, Unescorted Entry Authorization Certificate. The 30th CES Unit Security Manager will schedule appointments for issuance of badges.
1.6.7.9 The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the 30th Security Forces Squadron (30 SFS) as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. Due to safety/security, all contractor personnel shall be able to fluently speak, read, and write English.
1.6.7.10 Contractor Vehicles: Each contractor vehicle shall have the name of the contractor exhibited on each side of the vehicle. The contractor shall provide company identification tags and vehicle identification at no additional cost to the Government.
1.6.7.11 Physical Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use.
1.6.7.12 Key Control: The Contractor shall establish and implement methods of making sure all keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. NOTE: All references to keys include key cards. No keys issued to the Contractor by the Government shall be duplicated. The Contractor shall develop procedures covering key control that shall be included in the Quality Control Plan. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas. The Contractor shall immediately report any occurrences of lost or duplicate keys/key cards to the Contracting Officer.
1.6.7.12.1. In the event keys, other than master keys, are lost or duplicated, the Contractor shall, upon direction of the Contracting Officer, re-key or replace the affected lock or locks; however, the Government, at its option, may replace the affected lock or locks or perform re-keying.
When the replacement of locks or re-keying is performed by the Government, the total cost of re-keying or the replacement of the lock or locks shall be deducted from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the Contractor.
1.6.7.12.2. The Contractor shall prohibit the use of Government issued keys/key cards by any persons other than the Contractor’s employees. The Contractor shall prohibit the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Contracting Officer.
1.6.7.13 Lock Combinations: The Contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The Contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations. These procedures shall be included in the Contractor’s Quality Control Plan.
1.6.8 Special Qualifications: The contractor is responsible for ensuring all employees possess and maintain applicable/current certification(s), license(s), and permit(s) to perform required services on Vandenberg AFB, CA.
1.6.9 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.6.10 Contracting Officer Representative (COR): The (COR) will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies;
coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.
1.6.11 Key Personnel: The contractor shall provide a Project Manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the Contracting Officer. The Project Manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The Project Manager or alternate shall be available during normal business hours. During normal hours, the Project Manager or alternate shall be available within 30 minutes to meet on the installation with Government personnel to discuss problem areas.
1.6.11.1 Contractor personnel shall present a neat appearance and be easily recognizable. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company name and employee name.
1.6.11.2 Contractor personnel shall comply, and ensure that subcontractor personnel comply, with current federal, state, local, Department of Defense (DoD), and Air Force regulations, plans, and policies including all applicable parts of the Air Force Occupational Safety and Health Standard (AFOSH) and United States Department of Labor Occupational Safety and Health Administration (OSHA).
1.6.11.3 The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with (IAW) DoD 5500.7-R, "Joint Ethics Regulation (JER)", "Standards of Conduct," Dated August1993.
1.6.12 Identification of Contractor Employees: All contractor personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed. Contractor personnel will be required to obtain and wear badges in the performance of this service.
1.6.13 Loss or Damage: The Government will not be liable for any loss or damage to the contractor’s property or for expenses incidental to such loss or damage unless caused by the Government.
1.6.14 Fines Assessed Against the Government: The Government will retain amounts otherwise due the contractor an amount equal to any fines assessed against the Government as a result of the contractor’s failure to perform in accordance with the terms of this contract or in the event that a regulatory agency issues a Notice of Violation (NOV).
PART 2
Specific Tasks
2. Specific Tasks:
2.1. Tree Trimming & Removal Services. Includes the massive trimming and/or removal of trees as required to include stump grinding where needed and herbiciding of Eucalyptus tree stumps to prevent re-growth. The contractor shall perform all tree service to ensure a healthy, natural or manicured appearance. All tree services shall be performed in accordance with commercial and ANSI standards and the Air Force (AF) Form 813—Tree Removal at High Voltage Power Lines page 1 of 5-- and Environmental Compliance (APPENDIX 1) including referenced Combined Federal Regulations (Utility Pruning and Emergency Service Restoration) ANSI A300 (Part 1) – Tree Pruning ANSI A300 (Part 3) – Tree Support Systems (Cabling, Bracing, and Guying) ANSI Z60.1 – American Standards for Nursery Stock ANSI Z133.1 – Safety Requirements for Tree Care Operations.
2.1.1 The contractor is cautioned to take special care when entering any type of equipment or vehicle on regularly maintained grounds areas. The contractor shall restore any damage the contractor makes to the grounds while performing tree maintenance at no additional cost to the Government. This is to include but is not limited to re-contouring and restoring with native vegetation any access routes, roads, or disturbed areas created by the contractor. If required, the contractor shall remove and replace any fence that interferes with the work requested. Site restoration should be coordinated with 30 CES/CEANC. At a minimum, soil and native vegetation should be pulled back onto the disturbed area at the original contours to include, but not limited to, any access roads created by the contractor.
2.1.2 All trees to be trimmed and/or removed between 15 February and 15 August will be assessed for nesting birds and between 15 June through 15 September for the Flight Season of the Monarch Butterfly through the Asset Management Flight, Natural Resources Section. Once approved, the Contracting Officer Representative (COR) will issue a work order for contractor to complete this area within a two week period. If the area is not completed within the required time limit there will be a work stoppage until Natural Resources can reassess the area for nesting birds/Monarch Butterflies. Work orders prior to or after the dates noted above will be issued to the contractor as funds are available.
2.1.3 Power Line Tree Trimming/Removal. When power line tree trimming/removal is performed, industry standards will prevail. Trimming or pruning of trees that touch or hang over energized utility poles or power lines shall be trimmed to a minimum clearance from primary lines (over 600 volts) of 8 feet. Minimum clearance from secondary lines (under 600 volts, i.e.
electric service drops, telephone, and cable TV) shall be 4 feet. A minimum clearance of 20 feet per side for 70 kV power distribution lines shall be used for tree removal. Any tree that violates this envelope shall be cut to ground level. Trees that have been identified for removal shall be cut completely to ground level. Trees to be trimmed/removed may cover the distance of one to several power line poles. Each power pole is between 150 to 200 feet apart. The quantity of trees within the power lines varies and may have as few as three (3) to five (5) trees or be very dense and have as many as over one hundred (100) trees. The diameter of a tree may vary from one (1) foot to greater than five (5) feet.
2.1.3.1 If power disruption is required, the contractor shall coordinate with the COR whom will then coordinate with 30th Civil Engineer (30 CES) Electric shop (CEOFE), so that power can be shut off to ensure the safety of government and contractor personnel and real estate. 30 CES/CEOFE personnel will notify facility occupants of upcoming scheduled outages for power line tree trimming/removal.
2.1.4 Stump Removal. On occasion, the contractor may be tasked to remove stumps. Stump removal shall be accomplished using industry standards. The holes shall be filled in and compacted with topsoil and seeded/sodded to match the adjacent area to ensure there are no safety hazards.
2.1.5 Tree Treatment. Unless otherwise instructed, the Contractor shall apply herbicide to all Eucalyptus trees immediately after the cut while the stump cut is fresh, to prevent any re-growth, the contractor shall then reapply herbicide within 6 months to all previously treated stumps.
2.1.5.1 All pesticides applied on VAFB shall be on the current Armed Forces Pest Management Board (AFPMB) Standard Pesticides List Available to DoD Components and Agencies listing also known as the ‘DoD Approved Pesticide List’:

2.1.5.1.1 If the contractor desires to use a pesticide that is not on the DoD Approved Pesticide List, the contractor shall provide the Installation Pest Management Coordinator (IPMC) all pertinent information related to the non-standard pesticide. The government will provide an electronic .pdf file after contract award. This document shall include a comprehensive justification and rationale of why the products on the DoD Approved Pesticide List cannot be substituted for the proposed pesticide; the benefit gained by using the proposed product verses what is already available (i.e. less active ingredient). The IPMC will submit to AFSPC Pest Consultant for consideration and possible approval of the proposed pesticide.
2.1.5.1.2 The contractor shall record all pesticide products that are consumed/applied that day.
The report shall include the following information: Date; Location; Type of Operation; Target Pest; Pesticide Used; EPA number; % Concentrate; Amount of Concentrate; Amount of Finished Product; Units of Measure in SF (square feet); Applicator’s Initials. It is imperative that all blocks of the form be filled in accurately for contractor/government reporting to higher headquarters.
2.1.5.1.3 The contractor will be granted access to IPMIS web and shall be fully responsible for accurate pestide reporting. Contractor shall submit required reports to the government no later than the 5th business day of the month for the prior month’s applications.
2.1.5.2 Certification and License. All contractor personnel who apply pesticides on VAFB must possess a valid California pest control license in the appropriate category for type of work being performed.
2.1.5.3 Pesticide Mixing and Storage. “All pesticides shall be mixed and stored off base”. All unused pesticides, empty pesticide containers, and residue shall be disposed of off base. In the event the contractor spills or releases any hazardous substance, the contractor shall immediately notify the government and take appropriate action.
2.1.5.3.1 The Contractor shall coordinate with Base HAZMART ensuring all products have proper bar coding and labels as pertains to installation requirements.
2.1.6 Emergency Tree Trimming/Removal. The contractor shall respond to emergency calls within one (1) hour after notification during normal duty hours and within two (2) hours during non-duty hours. The contractor shall remove tree, tree limbs off power lines, structures, roadways and walkways to remove any safety hazards for personnel, vehicles, facilities and equipment (tanks, fences etc…). The contractor shall remove all debris from site.
2.2 Waste Materials/Weigh-In and Disposal of Tree Materials. A primary objective of waste/tree material disposal is the diversion (logs no longer than 18 inches long) of useable wood products. With proper coordination and pre-approval, methods of diversion can include on-site chipping, leaving cut wood pieces on site for personal or commercial use firewood collectors (these pieces cannot be left by any buildings or facilities located on VAFB), coordinating with log haulers to harvest trees in such a way that the main stems will be transportable on log trucks. Regulatory requirements are mandated as directed by the California Integrated Waste Management Board (CIWMB) in Title 14, California Code of Regulations (CCR) Division 7, Chapter 3.1, Articles 1-3 inclusive.
2.2.1 All trees removed from VAFB must be weighed and information provided to the government to track diversion weight.
2.2.2 The contractor shall provide a monthly hazardous material usage report to the Base Entomologist located at 1172 Iceland Ave., building 11439. This report shall be submitted within the first five (5) business days of each month.
2.3 Chipping/Composting Operations. The contractor shall process (chip/grind) all organic/green waste materials generated under this contract at the job site prior to transport to any local landfill. Larger diameter material can be cut to 14 inch to 16 inch lengths and stacked on site for collection and use by firewood collectors as noted in the work order, some areas wood cannot be left. Slash (forest harvest residue) that is not near facilities that would be harmed by insects or rodents can be left for wildlife habitats or burning later by fire crews, as is practiced by the US Forest Service.
2.3.1 The contractor shall not proceed with any regulated activities until authorization/permit is received from 30 CES/CEA. The contractor shall be responsible for complying with all requirements of any permits or registrations issued under the Chipping/Composting Rule. This rule places limitations on processing and storage of green waste and wood materials generated by chipping/composting type operations. The contractor shall support/comply with regulatory inspections executed under the State/County authority granted by this rule.
2.4 Invoicing. The Contractor shall invoice electronically to the Defense Finance Accounting Service (DFAS) utilizing the iRAPT application on the Wide Area Work Flow (WAWF) e- Business Suite website (www.wawf.eb.mil) within five (5) working days of the month. The contractor shall notify the COR upon submittal for processing. The contractor shall provide supplemental documentation with each monthly invoice to be uploaded in iRAPT. Supplemental files shall include work orders issued and other documentation that was generated for each task (inspection records). Supplemental files are used to validate the pricing for the monthly invoicing.
2.5 Contractor Management Reporting (CMR)
2.5.1 The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the US Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address http://www.ecmra.mil
2.5.2 Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the ECMRA help desk.
PART 3
Contractor Furnished Items And Services
3. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES:
3.1 General: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract that are not listed under Section 4 of this PWS.
3.3. Materials. The Contractor shall furnish all materials, supplies, and equipment necessary to meet the requirements under this PWS.
3.4. Equipment. The Contractor shall furnish all equipment necessary to meet the requirements under this PWS.
3.4.1 All contractor equipment brought into VAFB for the purpose of fulfilling the requirements of this contract must be taken out on a daily basis.
3.4.2 Chippers and Grinders. Chippers/grinders powered by diesel or gasoline fired engines 50 bhp and above must be registered in the California Air Resources Board (CARB) Portable Equipment Registration Program (PERP). Registration in this program is the responsibility of the equipment owner; unregistered equipment shall not be allowed on VAFB. Compliance with the provisions of the registration is the responsibility of the equipment owner. Each equipment unit will have two registrations, one for the combustion emissions from the engine and one for the particulate matter emissions from the grinding/chipping activity. The registrations consist of a paper registration which identifies the equipment and the recordkeeping/reporting conditions required by CARB. In addition, a sticker with the registration number and expiration date must be affixed to the equipment. If PERP registrations cannot be obtained, the contractor must have a permit to operate (PTO) from the Santa Barbara County Air Pollution Control District valid to operate at various locations in Santa Barbara County.
3.4.3 Chainsaws. At job completion, the contractor will be required to provide 30 CES/CEAQ a summary of the fuel usage and hours of operation along with chainsaw identification information (make, model, horsepower, etc.) for each unit operated.
3.4.4 Cleaning of Equipment. The contractor shall clean all equipment prior to entering Vandenberg AFB to prevent the spread of invasive plant seeds. In addition, when working with Pine Trees, all cutting tools such as saws, chippers and grinders shall be cleaned with a disinfectant prior to moving to a new stand of trees to prevent spread of Pine Pitch Cancer.
PART 4
Government Furnished Property, Equipment, And Services
4. GOVERNMENT FURNISHED ITEMS AND SERVICES:
4.1. Services.
4.1.1 Medical Response Services. The Government will provide initial medical treatment for life-threatening emergencies. It shall be the responsibility of the contractor or contractor employee to seek continued medical treatment if needed. The contractor or employee shall reimburse the Government for all Government-provided medical services. The emergency phone number is 911 or if using a cellular phone the number is 606-3911.

Definitions
5.1 DEFINITIONS: The following is a list of terms used within PWS that require further definition.
5.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.
5.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government.
Note: The only individual who can legally bind the government.
5.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S.
Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
5.1.4. DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.
5.1.5. DELIVERABLE. Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.
5.1.6. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.
5.1.7. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.
5.1.8. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.
5.1.9. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.
5.1.10. QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.
5.1.11. SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.
5.1.12. WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.
5.1.13. WORK WEEK. Monday through Friday, unless specified otherwise.
