About this file

This is a qualifications requirements form (AFLC Form 761 supplement) that outlines source approval requirements for manufacturers of critical parts. The document establishes two sets of qualification criteria - one for requirements established before October 19, 1984, and another for those after that date.

For both time periods, the document emphasizes that prequalification is essential for maintaining high quality and operational safety. The key requirement is that only manufacturers who have successfully completed all testing required by the original design company can be considered as approved sources. In some cases, manufacturers who have completed testing on similar complex and critical items may also be considered. The document references the OC-ALC Source Approval Information Booklet for detailed qualification requirements and indicates this applies to a cylinder assembly being procured by the Defense Logistics Agency Aviation.
