STATEMENT OF WORK

Temporary Berthing for Reserve June ADT USCG Port Security Unit 309 Port Clinton, OH 43452

1. Scope of Work:

a. The Coast Guard is seeking Temporary Berthing for Coast Guard personnel.

b. The period of performance is:

I. 04 JUN 2021 – 18 JUN 2021

c. Total proposed number of nights is 770 during the period of performance.

d. Total number of rooms 80 is during the period of performance.

The following room types and dates are required.

Below Listed Dates are Date of Check In / Date of Check Out:

June I. (45) Single Occupancy Room, 04 JUN 2021 – 18 JUN 2021 II. (35) Single Occupancy Room, 14 JUN 2021 – 18 JUN 2021

e. All rooms shall be non-smoking rooms.

f. The place of performance is within (15) miles from N. Lawrence Drive, Port Clinton, OH 43452.

g. Cancellations shall be accepted 48 hours prior to check in date.

h. Authorized points of contact to Make, Change or Cancel reservations are:

2. Requirements:

a. Contractor shall provide temporary berthing of Single Occupancy rooms.

b. The lodging facility shall be located within 15 miles of North Lawrence Drive, Port Clinton, Ohio 43452.

c. Multiple Hotels can be used to meet requirements, but all must meet the Standards of paragraph 3 below.

3. Standards:

a. The contractor shall ensure that all Berthing facilities provided shall meet the following standards:

I. Meet the Hotel and Motel Fire Safety Act of 1990 (PLI0l-391) and be listed on the Hotel-Motel Master list.

II. Facility shall minimally meet the requirements of Three Diamonds for the American Automobile Association (AAA), or a Classification of 4 from Travel Weekly.

III. Facility room entrances shall be accessible only by internal hallway(s). The facility internal hallways(s) shall have locked exterior doors, accessible by key-holding facility guests only. Internal hallway(s) shall be accessible by the facility lobby. The facility lobby shall be manned by a front desk attendant 24 hours a day and 7 days a week.

4. Allowed Contract Charges:

a. The Government will only pay for the items/services required on the purchase order itself.

b. The Government is exempt from Federal, State of Ohio, and local taxes. Tax Exempt Certification will be provided to Awardee upon request.

c. The Government will not pay for any additional charges. The following are examples of what the Government will not pay for:

I. Telephone II. Premium TV/Movie Service III. Internet Service IV. Room Service V. Restaurant Charges VI. Honor Bar VII. Laundry Vlll. Gratuities IX. Smoking fee X. Parking

5. Personally Charged Services:

a. Contractor shall make sure alternate arrangements have been made for any charges other than that of the room charges in the Contract Line Item Numbers.

b. Individual Coast Guard members will be solely responsible for any other valid charges including:

I. Telephone II. Premium TV/Movie Service III. Internet Service IV. Room Service V. Restaurant Charges VI. Honor Bar

vii. Laundry

viii. Smoking IX. Gratuities

6. Admin:

a. Contractor shall provide a copy of the final invoice and folio to the following:

I. Unit Berthing Coordinators:

7. Room Assignments List:

a. The Authorized Personnel making reservations and/or cancellations will provide the room assignment list to the contractor five (5) Days prior to Check-In. The list will include the names of those assigned to each room, and the dates each member will be Checking In and Checking Out. The contractor shall provide confirmation of rooms back to the authorized personnel as soon as rooms are reserved.
