About this file
This is a Certification Statements form issued by the State of Ohio for supplier preference evaluation in state and local contract opportunities. The form requires suppliers to complete appropriate certifications to receive scoring preferences and includes three primary preference categories: Domestic Preference (Buy American), Ohio Preference (Buy Ohio), and Veterans Preference (Buy Veteran).
The form establishes that suppliers must certify their eligibility for one or more preference categories based on specific criteria. For Domestic Preference under Revised Code 125.11, suppliers must identify where products are mined, raised, grown, produced, or manufactured, with options including the United States, Canada, Mexico, or other countries, with provisions for products manufactured outside the United States that contain at least 50% U.S.-sourced components. For Ohio Preference under Revised Code 125.09, suppliers must demonstrate that products are produced in Ohio or that the supplier has significant economic presence within the state, verified through payment of required taxes, registration with the Ohio Secretary of State, and employment of ten or more employees in Ohio, or alternatively qualify as a border state supplier with operations in Kentucky, Michigan, New York, Pennsylvania, or Indiana. For Veterans Preference under Revised Code 122.925, suppliers must be certified as a Veteran Friendly Business Enterprise. The form explicitly warns that suppliers who intentionally submit false or misleading information will be immediately disqualified and may be subject to administrative or legal action including debarment, with the State reserving the right to clarify information during evaluation.
