About this file

This document is a Certification Statements form for state suppliers seeking preference in government procurement, specifically for Ohio state contracts. The form is designed to help suppliers qualify for three types of preferences: Domestic (Buy American), Ohio (Buy Ohio), and Veterans preferences. Suppliers must carefully complete the appropriate sections to potentially receive scoring advantages, with strict warnings about submitting false or misleading information that could result in disqualification or legal action.

The certification form requires suppliers to provide detailed information about their business origins, manufacturing locations, employee base, and state tax compliance. Suppliers can qualify for preferences by demonstrating that their products are manufactured in the United States, Ohio, or border states, or by having significant economic presence within Ohio. The form includes multiple verification steps for each preference category, such as confirming product origins, number of employees in Ohio or border states, registration with the Ohio Secretary of State, and veteran-friendly business status. The document emphasizes the state's commitment to supporting domestic and local businesses through these procurement preference mechanisms.
