About this file
This document is a Certification Statements form for state procurement, specifically related to Ohio state purchasing regulations. The form requires suppliers to complete various preference certification sections, including Domestic (Buy American), Ohio Preference (Buy Ohio), and Veterans Preference sections. Suppliers must provide details about their product origins, manufacturing locations, economic presence in Ohio or border states, and veteran-owned business status to potentially qualify for scoring preferences in a state procurement process.
The certification form outlines specific criteria for receiving preference points, such as demonstrating that products are manufactured in the United States, Ohio, or border states, paying required state taxes, having business registration, and maintaining a specified number of employees in Ohio or border states. The document warns that intentionally submitting false or misleading information could result in immediate disqualification and potential administrative or legal action, including potential debarment. Suppliers must carefully document their product origins, component sourcing, and business presence to be considered for preference status.
