STATEMENT OF WORK
OFFICE TRAILERS
428TH FIGHTER SQUADRON
MOUNTAIN HOME AFB (MHAFB), IDAHO
1.0. OBJECTIVE: The objective is to provide office trailers needed during the renovation of buildings 1364 and 1365.
2.0 AUTHORITY: Foreign Military Sales Case SN-D-NDA Line 796
3.0 GENERAL SCOPE: The Contractor to prep area, deliver and setup, under lease, office trailers of good condition and sizes needed to allow the 428th Fighter Squadron to continue flying operations during the renovation of the Operations and Maintenance buildings. At end of lease, contractor to tear down and remove the trailers and any debris from the trailers.
3.1. Contractor to remove Juniper bush and concrete landscape edging, provide additional rock similar to what is in the area and level the area. Refer to Attachment 4.
3.2. Contractor shall deliver trailers to the 428 FS, 1409 Alpine St, Bldg. 1364, MHAFB, ID 83648, and perform set-up and installation no later than 15 December 2021. They are to be set up at the locations in attachment 3. Contractor shall be responsible for all maintenance during the period of performance (PoP).
3.3. Trailers: The contractor shall provide two (2) trailers as outlined in the floor plans (See Attachments 1 and 2).
3.4. Stairs: The contractor shall provide and install stairs with handrails at all external doors.
3.5. Electrical: Available power is 120v/208v, 3 phase, 60Hz and 120v/240v single phase (possibly; trailer 2 only); provide trailers with main electrical panels that are compatible with available power. Each trailer shall not exceed 200A. The contractor shall provide a separate 20A circuit for the electrical outlets in each room. Connection and disconnection to the Mountain Home AFB power grid shall be performed by contractor. Trailer 1, briefing rooms will require a minimum of 2 outlets, and the mission planning room will require 4 outlets. No more than one outlet will be placed on any given wall. Trailer 2 will require a minimum of 6 outlets per room. No more than two outlets per wall.
3.6. Access Controls: The contractor shall purchase and install Simplex PL1000 Keyed Cypher locks prior to delivery. Keys will be given to the 428FS unit security manager.
3.7. Trailer Placement: The OPS trailer shall be assembled and/or set-up on a rock surface at the location specified in Attachment 3. The Maintenance trailer shall be assembled and/or set-up on an asphalt surface at the location specified in Attachment 3. Trailers shall be mounted in such a way to provide adequate crawl space underneath to facilitate electrical cable runs and installation.
3.8. Communications Requirements: Contractor will provide all backbone infrastructure to include ## of terminated drops at specified locations within the trailer. VOIP phones and Juniper Switches will be installed by 366 CS with internal network access installed by the 428 Fighter Squadron contracted PKL/IT personnel.
3.9. Plumbing: The trailers shall not require any plumbing connections or bathrooms.
4.0 Quality Assurance Evaluator (QAE): will act as an Air Force point of contact for all delivery issues. The Contracting Officer (CO) shall be the only person authorized to enter into, administer, and terminate contracts and make related determinations and findings. The term includes certain authorized representatives of the CO acting within the limits of their authority as delegated by the CO as defined in the Federal Acquisition Regulation (FAR) 2.201.
5.0 GENERAL INFORMATION
5.1. Hazardous Material Identification: Material Safety Data Sheets (MSDS) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract), and for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. MSDSs shall be submitted by the contractor at the pre-performance conference. Any changes to paint or detergent types after submission of MSDS will require the contractor to submit new MSDSs prior to use of the product.
5.1.1. HAZMART: The government (HAZMART personnel) will inventory all chemicals that the contractor brings on to Mountain Home AFB. The inventory will be performed prior to commencement of work. Criteria for identifying hazardous waste is contained in Subpart C of 40 CFR, Part 261.
5.1.2. Spill Response: The contractor will be briefed on Mountain Home AFB spill response procedures at the pre-performance conference. The contractor shall report and promptly cleanup all spills in a manner consistent with current environmental regulations. In the event that it is necessary to utilize government material, equipment or personnel to clean up a contractor-caused spill, the contractor shall be required to reimburse the government for all associated costs.
5.1.3. Hazardous Material/ Waste Management: The contractor will be briefed on Mountain Home AFB Hazardous Material / Waste Management Plan at the pre-performance conference.
5.1.4. Hazardous Material Handling: The base Bio-Environmental Engineer (BEE) and HAZMART sections shall approve any chemicals, solvents, lubricants, and other products requiring MSDSs prior to handling, using, and storing them on MHAFB. The contractor shall identify these materials and products on Air Force Form 3952 Chemical/Hazardous Material Authorization Request, Process Identification Form, and Shop Disposal Form. The contractor shall provide one copy of the MSDSs for each item to the BEE and HAZMART sections for review prior to any chemicals being brought onto Mountain Home AFB. The contractor shall maintain one copy of the MSDSs for each hazardous material line item used within the work area. The contractor shall not use products that are or contain Toxic 17 chemicals, Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals. Any hazardous material containing one of these banned substances will not be allowed on base.
5.2. Environmental Protection: The contractor shall conform with all federal, state and local laws pertaining to environmental protection. The contractor shall develop a plan to operate within these guidelines in the performance of this PWS. Any violation of law, DoD Guidance, or the contractor’s own Environmental Protection Plan will be identified to the CO immediately.
5.3. Contract Management: The contractor shall provide management oversight for all work performed in this PWS. The contractor shall identify a contract manager (CM) to serve as the interface between the government and contractor. The contractor shall appoint an alternate CM if the primary is unavailable for duty (vacation, illness, etc.). The CM or alternate shall have full authority to act for the contractor on all matters relating to this contract. The CM or alternate shall be available during normal duty hours within one hour to meet on the installation with government personnel during periods when work is being performed. At all other times, CM or alternate shall be available to meet telephonically with Government personnel within one business day of notification during normal business hours. As changes occur, new names and phone numbers must be submitted within three calendar days to the CO. Normal duty hours at MHAFB are Monday – Friday, 0730-1630.
5.4. Access: The contractor shall allow government personnel access to work areas, provide support and not interfere with Quality Assurance Personnel (QAP), state, federal and other designated personnel in performance of their official duties.
5.5. Contractor Personnel: Attire shall be in good repair and shall not be considered offensive, disruptive, or unsafe. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats bearing the company name or logo. The coloring or design of the items selected should be such that the items identify personnel easily and quickly for reasons of safety and personal protection. The government is authorized to restrict the employment under the contractor of any contractor employee or prospective contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
5.6. Conflicts of Interest: The contractor shall not employ any person who is an employee of the U.S. Government if employing that person would create a conflict of interest. Additionally, the contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to DoD Directive 5500.7, Standards of Conduct, subsections 2-2064 and 2-303. The contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in paragraph 3 of AFI 64-106, Air Force Industrial Labor Relations Activities. Contract personnel shall readily identify themselves as contractors in correspondence, telephone conversations and in meetings they attend IAW AFI 33-119, paragraph 5. The contractor shall not employ persons for work on this contract if such employee is identified by either the contractor or the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation or its population.
5.7. Hours of Operation: Normal MHAFB hours of operation are from 0730 – 1630. During these times the contractor shall have access to various agencies that may be needed.
6.0. Security Requirements
6.1. Pass and Identification Items: The contractor shall provide a completed 366th SFS Form 30 for each employee requiring installation access to the 428FS USA within five (5) business days following contract award. After approval, the 428FS USA will notify the contractor that the employees may get their installation access ID at the Visitor’s Center.
6.2. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires i.e. terminated for cause, retirement, etc.
6.3. Traffic Laws: The contractor shall comply with all MHAFB traffic regulations while on
MHAFB.
6.4. Weapons, Firearms, and Ammunition: Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on MHAFB.
6.5. Controlled Unclassified Information (CUI): The contractor shall comply with DoD 5400.7- R, Chapter 4, DoD Freedom of Information Action (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting and safeguarding CUI material.
6.6. Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
6.7. Physical Security: The contractor shall be responsible for safeguarding all government property. At the end of each work period, all government facilities, equipment, and materials shall be secured.
6.8. The CO may direct the contractor to remove, and the contractor shall remove, an employee from any assignment under this contract for reasons of security or misconduct. All contractor personnel are subject to security forces jurisdiction and the authority of the Installation Commander while on MHAFB.
7.0. Performance of Service during Crisis Declared by the National Command Authority:
This service is determined to be non-essential for performance during crisis according to DODI
3020.37. However, in the event of a crisis or significant emergency is declared, the CO will contact contractor personnel to notify them of the situation and any possible changes to their contract responsibilities.
