PERFORMANCE WORK STATEMENT

MAINTENANCE FOR

BD (GSL) SUPPORT SERVICES: DEVICES AND SYSTEMS

APPLICATIONS

1. DESCRIPTION OF WORK

a. SCOPE. With the exception of those items listed in paragraph 5 as Government furnished supplies and services, the Contractor shall provide all transportation, labor, parts, materials, individual tools and test equipment required to service and maintain the following equipment:

b. LOCATION. The BD Support Devices and Systems Applications are located at the 19th Medical Group, Main and Refill Pharmacy, Bldg 1090, Little Rock Air Force Base, AR. The Contractor's service technician shall report to the 19 MDG Biomedical Maintenance Office, prior to and upon completion of their work to sign in and out.

2. PERSONNEL

a. The Contractor's service technician assigned to this contract, as well as any temporary personnel, shall have successfully completed training on this equipment, or similar equipment by an authorized BD Support Services training program.

b. The Contractor shall provide documentation substantiating their service technician is qualified to service and maintain this equipment.

3. QUALITY CONTROL

a. The Contractor shall develop and maintain a quality program to ensure services are satisfactorily performed in accordance with the terms and conditions specified herein. The contractor shall develop and implement procedures to identify and prevent defective services from recurring.

4. HOURS OF OPERATION

a. The Contractor shall schedule all work to be done on this unit with the Biomedical Equipment Repair Section or Pharmacy; Monday through Friday.

5. GOVERNMENT PROVIDED SERVICES

The Government will provide:

a. An individual from the using activity to be on-hand if scheduled service inspections or service calls aren't completed before the end of our duty day, 4:30 pm.

b. An individual from the using activity to sign service reports verifying the completion and satisfaction of the service(s) performed.

6. CONTRACTOR FURNISHED EQUIPMENT, PARTS, MATERIALS AND

SERVICES.

a. Except for the supplies and services stated in paragraph 5 to be Government furnished the Contractor shall furnish/provide the following:

(1) TECHNICAL DATA. The Contractor's service technician shall maintain a complete file of technical documents on all components of this equipment, to include schematics, wiring diagrams, service manuals, trouble-shooting guides and service update memos.

(2) EQUIPMENT UPDATES. The Contractor shall furnish and install all software updated and equipment modifications normally provided by the manufacturer.

(3) REPLACEMENT PARTS. The Contractor shall provide all parts at no additional cost to the Government.

(4) PARTS SHIPMENT COSTS. No additional costs shall be charged to the Government for the shipment costs associated with any removed parts and/or new or rebuilt parts under the terms of this contract.

7. DOCUMENTATION

a. The Contractor's service technician shall prepare a service report for each service call (scheduled and unscheduled), and obtain a signature from the using activity to certify the service was complete and accepted. The service report shall include, at a minimum, the following LEGIBLE information:

(1) Service call number.

(2) Start date/time and completion date/time.

(3) Description of the problem and corrective actions.

( 4) List of parts replaced, if any.

(5) Name of the service technician.

(6) Signature from the using activity.

(7) Total dollar value of the service, INCLUDED parts and labor.

(NOTE: This information is REQUIRED for our Biomedical Maintenance section to post to the Equipment Historical Data Records for accreditation inspection purposes.)

b. The Contractor's service technician shall give the above service report to the Biomedical Maintenance or Pharmacy section on completion of the service calls. If service is completed after our normal duty hours, or the parts and labor dollar values are not immediately available, the service report shall be mailed within five (5) working days to:

9. RELIABILITY

This unit must maintain a monthly percentage uptime of 95%, based on the total number of duty hours in each month.

a. Uptime is defined as the time a system is performing according to the manufacturer's specification.

b. Downtime is defined as the time when an equipment unit is deemed not available due to breakdown, ongoing corrective maintenance, or failure to perform, according to the manufacturer's specifications. The period of downtime will be from the time of notification to the Contractor, until the system is accepted by the Government's representative as being in operating condition. This will include waiting for delivery and installation of replacement parts. Total downtime is the cumulative number of hours per month the system is non-functional, either whole or in part.
