About this file
This is a Past Performance Questionnaire template used to evaluate contractor performance on federal contracts. The form consists of three main sections: (1) Contract Information to be completed by the contractor, including firm details, CAGE code, UEI number, contract numbers, dates, and pricing; (2) Rating definitions with four levels - Exceeds, Satisfactory, Unsatisfactory, and Not Applicable; and (3) Performance evaluation criteria to be completed by the client reference covering six major areas: Quality, Schedule, Cost Control, Management, Regulatory Compliance, and Overall Customer Satisfaction.
The questionnaire requires detailed ratings on contractor performance metrics including technical quality, problem resolution, schedule compliance, cost control, invoice accuracy, management effectiveness, workforce capabilities, regulatory compliance, and overall satisfaction. The form uses a standardized rating scale where "E" indicates exceeds requirements with no issues, "S" indicates satisfactory performance with minor issues resolved, "U" indicates unsatisfactory performance with serious unresolved problems, and "N" indicates not applicable. The client reference must provide contact information, describe their role, sign the evaluation, and can include additional explanatory remarks.

RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT YOUR EVALUATION OF THE
CONTRACTOR’S PERFORMANCE OF THE SCOPE IDENTIFIED ON PAGE 1
RATING DEFINITION NOTE
(E) Exceeds Performance meets contractual requirements and exceeds aspects to the Government/Owner’s benefit. The contractual performance of the element was accomplished with no problems, and needed no corrective action throughout the term of the contract.
An “Exceeds” rating is appropriate when the Contractor performed all aspects of the scope of work without issue or need for corrective action.
There should be no weaknesses identified.
(S) Satisfactory Performance meets contractual requirements. The contractual performance of the element may have encountered minor problems for which corrective actions taken by the Contractor appear or were satisfactory.
A “Satisfactory” rating is appropriate when there were only minor problems, or major problems that the Contractor recovered from without impact to the contract. There should be no significant weaknesses identified.
(U) Unsatisfactory Performance does not meet contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element contains serious problem(s) for which the Contractor’s corrective actions appear or were ineffective.
An “Unsatisfactory” rating is appropriate when significant events occurred that the Contractor had trouble overcoming, and which impacted the Government/Owner.
Significant weaknesses were identified.
(N) Not Applicable No information or did not apply to your contract.
