Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)
Acquisition Plan Action ID: 36C252-21-AP-2260 / 607-21-3-969-0162
1. Contracting Activity: Department of Veterans Affairs, VISN 12, Hines VAMC has submitted 578-21-
3-969-0171 for Surgical Bone Saws to the Great Lakes Acquisition Center (NCO12) as an Emergency.
Ronald Cherry from the Logistics will be the Hines contact point & Matthew Hohl from NCO12 will be responsible for the Contracting Action.
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
This justification and approval is for the Surgical Bone Saws which will be Sole Source to Service- Disabled Veteran Owned Small Business and awarded as a firm-fixed-priced purchase order. This Requirement is Urgent and Compelling as the site is having to cancel reschedule major surgeries as their current Bone Saws are inoperable.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
The Hines VAMC has lost/reduced functionality at the site as current Bone Saws are inoperable as the batteries have failed and are no longer being supported by the OEM. As such Surgeries are being cancelled rescheduled as the current inventory of saws is insufficient to service the demand of surgical cases.
4. Description of market research conducted and results or statement why it was not conducted:
Original Equipment Manufacturer (OEM) for this requirement is Stryker who is considered Other Than Small Business (OTSB) Per System for Award Management (Sam.gov) Reps & Certs. Beacon Associates is ready and available to meet this requirement. The site has identified this is an Emergency Requirement as their current Bone saws are complete inoperable. This is leading to risk the VA Mission at the Hines VAMC in the Surgical Department as these bone saws are critical in surgeries. This acquisition falls under Unusual and Compelling Urgency per 41 USC §3304(a)(2) as implemented by FAR 6.302-2
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
