Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C263-23-AP-4050

1. Contracting Activity: Department of Veterans Affairs, Network Contracting Office 23 (NCO23) for

VA Nebraska-Western Iowa Health Care System (VA NWIHCS), Omaha VA Medical Center, Facilities Management Engineering Department, Purchase Request Number: 636-23-4-5058-0202

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: VA NWIHCS has an emergent need for replacement of automatic door located at the entrance of the OR suite on the fourth floor of the Omaha VA Medical Center campus. The current automatic door is nonfunctional and beyond repair per VA NWI Maintenance Supervisor and COR, Roy Hurd. This requirement will include removal of existing nonfunctioning automatic door and replacement with new automatic door per the Statement of Work and Specifications documents. The approximate estimated value of these services is $15,828.39.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: : Due to the emergent matter of this requirement, competition for pricing for this requirement was unable to be completed. The automatic door at the OR entrance of the Omaha VAMC needs emergency replacement to maintain a safe and secure patient care environment. Because this is considered an emergency acquisition contract, the Omaha Construction Contracting office has already authorized a Contractor, BasePoint (Control Installations of IA), to perform the services of repairing and making the entrance to the OR suite operable again. This work has will be completed and accepted per VA NWI Maintenance Supervisor and COR, Roy Hurd. This work needs to be performed immediately to maintain a safe and secure patient care environment; therefore, a sole source requirement is in the best interest of the Government to make an award to a contractor who can meet the immediate need of services associated with this requirement and who has the knowledge of the systems involved. If not addressed immediately, the Government is at increased risk for patient and/or staff safety and/or security incidents. BasePoint is available immediately to perform the work without causing any additional delay, which could result in adverse patient and/or staff safety and/or security outcomes; therefore, BasePoint meets the need under this emergent and compelling requirement. Awarding a contract to BasePoint will reduce the time required to correct this emergent safety deficiency.

4. Description of market research conducted and results or statement why it was not conducted: As stated previously, immediate action is needed by a contractor with immediate availability and knowledge of automatic door removal replacement within in a healthcare delivery environment.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
