Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Estimated amount:

There is a global supply chain imaging contrast dye shortage. This is an emergency order for brand name equipment that allows the medical center to better control its use of contrast dye. This unit provides a way for the medical center to use less contrast per injection by allowing multiple uses from one vial thereby minimizing waste. The circumstances are urgent and compelling as the hospital looks for all possible solutions in response to its own shortage of contrast. Time is not permitted to locate additional sources.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

This unit provides a way for the medical center to use less contrast per injection by allowing multiple uses from one vial thereby minimizing waste.

4. Description of market research conducted and results or statement why it was not conducted:

Once it was discovered the OEM has only one authorized distributor for the required equipment, efforts to locate additional sources were ceased.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
