I. JUSTIFICATION

The West Roxbury Campus of VA Boston Healthcare System is renovating the Medical Intensive Care Unit (MICU). The newly renovated space will require new furnishings and accessories to meet staff and patient needs and provide proper care to our veterans. Existing furniture is worn, damaged and inefficient for facility needs. The new furniture listed in this procurement will meet all infection control requirements and create environments that are patient-centered, promote healing, and improve patient satisfaction.

II. SCOPE OF WORK

The selected Contractor shall furnish, deliver, and install furniture to the VA Boston Healthcare System West Roxbury Campus located at 1400 VFW Parkway, West Roxbury, MA 02132.

The timeframe for delivery receipt shall be no later than 24 weeks following award of contract, with coordination of specific dates mutually agreed upon by the VA Medical Center and the selected Contractor. The selected Contractor may be required to store all or portions of the awarded items to accommodate potential construction delays up to 30 days. The Contractor’s price proposal shall account for this. The selected Contractor shall deliver and install during regular business hours, Monday through Friday, 8:00 AM to 4:00 PM. No product for this solicitation shall be drop shipped directly to the project site. All deliveries must be coordinated by the selected Contractor to their warehouse and delivered to the West Roxbury Campus at the time of installation. The selected Contractor will be required to coordinate the scheduling for delivery and installation with the VA Interior Designer. A map showing the loading dock location and the route inside the building to the installation space will be provided to the selected Contractor.

The selected Contractor shall provide timely response and assistance on all warranty and service claims made by the VA Medical Center. Proposed manufacturer shall have local representatives within 100 miles to provide on-site warranty service. The selected Contractor and/or Manufacturer Representative shall perform a site visit for item inspection and review within 5 business days of correspondence from the VA Medical Center requesting warranty or service claim assistance. An action plan shall be submitted by the selected Contractor for all warranty and service claims within two weeks following the initial site visit. The selected Contractor shall ensure that all owner’s manuals, warranty information, assembly instructions, maintenance and care instructions, operating instructions and any other information is provided to the Contracting Officer Representative (COR) upon completion of the installation.

III. BID REQUIREMENTS

All bids are to be formatted appropriately to ensure a consistent basis for comparison. A preformatted bid price proposal is included as Appendix A.

Cost of item (as defined below) shall be wholly inclusive of all required components. DO NOT break out costs for any parts, pieces, or COMs in either the cost or extended cost columns.

• Comprehensive freight of all items to be indicated as a total amount.

• Comprehensive installation of all items to be indicated as a total amount.

• Potential contractors shall be solicited on brand name or equal. All bid submissions shall comply with the minimum technical requirements for each item described herein. All “OR EQUAL” product bids shall include 2D and 3D drawings of the line item and cut sheets or other documentation supporting the item meets or exceeds all the indicated required characteristics.

• At least three references required; healthcare related preferred.

• Vendor shall assume one installation, over a maximum of one day, regular time.

IV. FREIGHT & INSTALLATION

The selected Contractor shall provide all necessary materials, equipment, labor, supervision, and management to: (1) coordinate pick-up; (2) load and unload; (3) place/install per provided floor plan in designated areas; (4) assemble items as required;

(5) secure and store, as designated; and (6) properly dispose of all associated packing/crating materials.

All products shall be installed by manufacturer certified installers. Installation shall be coordinated by the selected Contractor with the designated VA Interior Designer. Loading docks and elevators will be available for installation. VA staff and patients will have priority access over the elevators and the selected Contractor’s time shall accommodate for this. Building hours are 8:00 AM – 4:00 PM. Installation shall be performed during regular hours, Monday through Friday, to be selected and confirmed by the VA Interior Designer. Multiple days of the week must be available for installation.

The selected Contractor shall supply sufficient installation labor to complete the installation in a maximum of 5 business days. The selected Contractor shall inspect all items to ensure that the furniture is free of surface dirt and defects and that the installation is complete, and furniture is ready for use. The selected Contractor shall furnish a written punch list, if necessary, to the VA Interior Designer within 5 business days of installation, along with an action plan for completion.

The selected Contractor shall contact the designated VA COR at least 2 weeks prior to the date that services are to begin. Pick-up, delivery, and installation services are expected to be performed per future information. The selected Contractor shall coordinate installation times with the designated VA COR to confirm that the building is ready for furniture. In the event there is a delay in installation, the selected Contractor must notify the designated VA COR immediately.

• Mattress cover shall be fluid, crack, peel, abrasion, and bed bug resistant, as well as anti-microbial and anti-fungal.

• Mattresses shall be able to be cleaned with a minimum 10% bleach solution.

VI. OTHER CONSIDERATIONS FOR PROPOSAL

The selected Contractor shall be responsible for any damage to building walls, doors, or windows during delivery. The selected Contractor shall furnish and install all supplementary or miscellaneous items, appurtenances, and devices incidental to or necessary for sound, secure, and complete installation. The selected Contractor shall provide complete coordination of all product delivery and installation and provide competent supervision personnel.

VII. CONTRACTOR REQUIREMENTS

The selected Contractor shall perform all work under this contract in strict accordance with all safety codes to eliminate the possibility of damage or injury to personnel, equipment, and building structures.

• Scope of work to include, but not limited to, provision of furnishings, delivery, and installation.

• Furniture layout shall meet the requirements and design intent of ABA, NFPA, OSHA, ANSI, BIFMA, CAL 117, VA, for all accessibility, life safety, occupational safety, VA, and federal requirements for a business, healthcare, or residential occupancy, as applicable.

• Track all orders to ensure timely delivery and communicate issues to VA Interior Designer.

• Furniture delivery not to interfere or disturb other tenants in the building or the operation of the facility. The selected Contractor will coordinate the work of the project with the VA Interior Designer.

• Upon delivery of goods, the selected Contractor shall unload at the loading dock with permission of building management. Please contact the VA Interior Designer for scheduling.

• Provide final cleaning of products prior to project completion.

• The selected Contractor shall leave one key in lock on each unit and submit each duplicate key to the VA Interior Designer upon completion of installation.

• Provide furniture cut sheets, warranty, and delivery schedule. The selected Contractor shall provide O&M manuals including warranty, care, and manufacturer contact information to VA Interior Designer at completion of project. VA Interior Designer to provide digital format for O&M manual.

• Freight all furniture in an environmentally responsible manner, i.e. blanket wrapped or equal, when available. Remove and recycle or discard all packaging materials in an environmentally responsible manner.

• Repair or replace damaged products at the discretion of the VA Interior Designer.

• Final layout to be determined by the VA Interior Designer.

• Responsible for containing any rubbish/waste generated by the work force or material delivery at the end of each day.

• All waste to be removed and disposed of by the selected Contractor daily.

VIII. PAYMENT SCHEDULE

The selected Contractor shall bill on completion and acceptance of the work.

IX. RECORDS MANAGEMENT STATEMENT

1. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228.

2. Contractor shall treat all deliverables under the contract as the property of the U.S.

Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest.

3. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records.

4. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act.

5. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract.

6. The Government Agency owns the rights to all data/records produced as part of this contract.

7. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.

8. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.].

9. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation.

Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules.

10. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.

X. NON-PERSONAL SERVICES STATEMENT

Contractor employees performing services under this order will be controlled, directed, and supervised at all times by management personnel of the contractor. Contractor management will ensure that employees properly comply with the performance work standards outlined in the SOW. Contractor employees will perform their duties independent of, and with occasional supervision of VA COR. The tasks, duties, and responsibilities set forth in the SOW may not be interpreted or implemented in any manner that results in any contractor creating or modifying Federal policy, obligating the appropriate funds for the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating contracting regulation.

REPORTING REQUIREMENTS: The Contractor shall be required to report to the Police Office to check in. This check-in is mandatory. When the service is completed, the FSE shall document services rendered on a legible ESR(s). The FSE shall be required to check out with Engineering and submit the ESR(s) to the COR or designated representative. ALL ESRs shall be submitted to the equipment user for an "acceptance signature" and to the COR or designated representative for an "authorization signature". If COR or designated representative is unavailable, a signed, authorized copy of the ESR will be left with the user.

IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: The Contractor's FSE's shall wear visible identification at all times while on the premises of the VAMC. The Contractor shall park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VAMC will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited on all VAMC property. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.

SECURITY REQUIREMENTS: All contractor personnel shall obtain a short-term identification badge issued by the COR or designated representative. Such badge shall be worn by the individual and prominently displayed at all times while on VA Property. No employee of the contractor shall enter the project site without a valid identification badge issued by the VA. In order to obtain a short-term identification badge, contractor personnel shall present to the COR a valid (non-expired) photo identification issued by a US federal, state, or local government agency.

Escort will be provided as required in sensitive work areas.
