Performance Work Statement Dated: 20 July 2018

1. GENERAL:

This is a non-personnel services contract to provide asphalt sealing, repair, and remarking/restriping at the NIH/RML Campus, 903 S. 4th Street, Hamilton, MT 59840. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.

1.1 Description of Services/Introduction:

The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and all other items and non-personal services necessary to perform the sealing, repair, and remarking/restriping of the NIH/RML campus as defined in this Performance Work Statement. The contractor shall perform to the standards detailed in this contract.

1.2 Background:

The NIH/RML is comprised of approximately 237,929 square feet of asphalt roadway and employee/visitor parking. This area comprises approximately 298 demarked parking spaces, approximately 14 demarked handicapped parking spaces, and approximately 14 demarked crosswalks.

1.3 Objectives:

a. Inspect and repair all joints or seems in the existing asphalt prior to resealing.

b. Remove and replace all existing demarcation reflective tape (white & yellow).

c. Reseal all existing asphalt with good quality commercial grade asphalt sealer.

Restripe/repaint all parking space lines, and handicapped markings to original locations, with good quality traffic paint as appropriate, crosswalks will be demarked using highly reflective white peel and stick pavement tape.

d. Provide a three-level cost estimate providing for quality of materials good, better, best.

1.4 Service Objective:

1.4 Scope: Contractor shall include a firm fixed-priced price proposal for the services described in this Statement of Work.

1.5 Period of Performance:

The period of performance shall be for 30 days for the completion of the work after starting. The contractor shall coordinate with the COR any portions of the work to be completed on Weekends.

1.6 General Information

1.6.1 Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract. The QCP is to be delivered, with the contractor’s proposal. A revised copy of a comprehensive written QCP shall be submitted to the COR within 5 working days when changes are made thereafter. After acceptance of the quality control plan the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his QC system.

1.6.2 Recognized Holidays: The Contractor is not required to perform work on the following recognized holidays;

New Year’s Day Labor Day

Martin Luther King Jr.’s Birthday Columbus Day

President’s Day Veteran’s Day

Memorial Day Thanksgiving Day

Independence Day Christmas Day

1.6.3 Hours of Operation: The contractor is responsible for conducting business, between the hours of 7:00 A.M. and 4:30 P.M. Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. If working on the weekends is the desired the contractor may request weekend work hours and proceed when approved by the Contracting Officer.

1.6.5 Place of Performance: The work to be performed under this contract will be performed at National Institutes of Health, Rocky Mountain Laboratories, 903 South 4th Street Hamilton Montana 59840.

1.6.6 Type of Contract: The government will award a firm fixed price.

1.6.7 Security Requirements: Contractor personnel performing work under this contract must have a photo I.D., also all employees must be able to pass a standard Government security background check.

1.6.8 Pre-Performance Conference: The Contractor agrees to attend any pre-performance conference convened by the contracting officer in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.6.10 Contracting Officer Representative (COR): The (COR) will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies;

coordinate site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

1.6.11 Key Personnel: The follow personnel are considered key personnel by the government:

Project Manager. The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the contracting officer.

The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.

1.6.12 Identification of Contractor Employees: All contract personnel shall wear their contractor badges in plain view while performing under this contract.
