Amendment No.: 2

Date of Issuance: 05/28/2019

The above numbered Request For Proposal (RFP) is amended as set forth below. The hour and date specified for receipt of Offerors remains unchanged.

The purpose of this amendment is to provide Responses to missing Questions that were received in response to this solicitation and were not included in Amendment One - Responses to Questions.

BUSINESS QUESTIONS

1. RFP Page 1 and Page 33. In Section A and Instructions to Offerors, the Government makes reference to Attachment 1

Packaging and Delivery of the Proposal. Attachment 1 of the RFP is the Proposal Intent Response Sheet. Please provide the Packaging and Delivery of the Proposal attachment. See response to Question #10 of Amendment One – Response to Questions

2. RFP Page 1. The RFP references the Attachment entitled, "Proposal Summary and Data Record, NIH 2043.” Please provide this attachment. The Offeror is to disregard the Proposal Summary and Data Record, NIH 2043. This is not required for this solicitation.

3. RFP Page 28. The RFP includes the FAR language below. Should this information be included on the Cover Page (RFP Page 33) or the Business Proposal Cover Sheet (RFP Page 39)?

“2. The first page of the proposal must show--

i. The solicitation number;

ii. The name, address, and telephone and facsimile numbers of the offeror (and electronic address if available);

iii. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation and agreement to furnish any or all items upon which prices are offered at the price set opposite each item;

iv. Names, titles, and telephone and facsimile numbers (and electronic addresses if available) of persons authorized to negotiate on the offeror's behalf with the Government in connection with this solicitation; and

v. Name, title, and signature of person authorized to sign the proposal. Proposals signed by an agent shall be accompanied by evidence of that agent's authority, unless that evidence has been previously furnished to the issuing office.” The Offeror is to use the cover page list of instructions found on page 28 for both the Technical and Business cover pages.

4. RFP Page 33. The RFP references SECTION J, Attachment entitled, TECHNICAL PROPOSAL COST SUMMARY. Please provide this attachment. See response to Question #24 of Amendment One – Response to Questions.

5. RFP Page 33. The RFP states, “The technical proposal must include direct cost and resources information, such as labor-hours and categories and applicable rates, materials, subcontracts, travel, etc., and associated costs so that the offeror's understanding of the project may be evaluated (See SECTION J, Attachment entitled, TECHNICAL PROPOSAL COST SUMMARY.).” Is this requested information outside of the 35 page Technical limit? If not, in which Factor should the Technical Proposal Cost Summary be addressed? See response to Question #24 of Amendment One – Response to Questions.

6. RFP Page 33. Are subcontractors required to provide a Technical Proposal Cost Summary? If so, may the subcontractors submit their proprietary direct cost information in a sealed package for the Technical Proposal, along with their Business Proposal? See response to Question #24 of Amendment One – Response to Questions.

7. RFP Page 35. For Past Performance, the RFP states, “A list of the last 3 contracts completed during the past Five years and currently being performed that are similar in nature to the solicitation workscope.” Please confirm contracts can be either completed in the past five years or currently being performed. The Offeror is to provide a list of the last 3 contracts completed or currently being performed furing the past 5 years.

8. RFP Page 36 and Page 43. The RFP states, “The page limitation excludes any information on the qualifications of personnel (resume/ CVs) and/ or past performance information…” Please confirm the Personnel Matrix is also excluded from the 35 page Technical limit. See response to Question #1 of Amendment One – Response to Questions.

9. RFP Page 38. In the Time (Level of Effort) by Task Area table, should the total hours be 4880 instead of 4800? Yes, the published sum should be 4880.

The SOW state, “The Contractor shall submit a detailed plan addressing both a Phase-In and Phase-Out plan period.” Since the Phase In Plan is not listed in the Deliverables table (RFP Page 6), when is the due date for the Plan? A Phase In plan is not required since there will be no transfer of responsibilities from an incumbent.

12. Will the Government allow time for additional questions once the remaining attachments have been released? There will be no additional questions to this solicitation.
