ADDENDUM TO 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (JUN 2008)
In addition to the instructions listed in the master clause, the Offeror must submit the following documents:
Technical Proposal/Personnel: The Offeror’s technical proposal must address a comprehensive understanding of the Statement of Work and demonstrate that the Offeror is able to successfully perform the Government’s request.
The technical proposal must include a full list of key personnel who will be used to complete this request. This personnel section should be distinguished from the rest of the technical proposal in order to ensure that each evaluation factor is appropriately addressed.
The vendor shall provide evidence to support the proposed staffing levels for this project. The vendor shall include resumes showing that key personnel have degrees in the field of Information Technology, Computer Programming, Computer Science, Engineering, or a related degree. Resumes shall illustrate experience in business and systems architecture, feasibility studies, emerging technologies, compliance with accepted international standards. In addition, key personnel shall demonstrate knowledge of library bibliographic and subject authority data.
The technical proposal shall be no more than 50 8” x 11” pages (25 front/back sheets). Margins shall be 1” on all sides. Font size should be at least 12 point. All other formatting is at the contractor’s discretion. The page limit INCLUDES past performance documents and resumes. The Government WILL NOT review technical proposal pages in excess of the 50th page.
Price: The Offeror shall provide a document that describes the components that went into calculating the cost for the linked data consulting requirement. The rationale that goes into the pricing should be reasonable and realistic. The vendor should provide justifications and descriptions to the fullest extent possible.
Any discounts (i.e. quantity) off of the Offeror’s pricing shall be listed as a separate item. The price/business proposal shall be submitted as a separate attachment from the technical proposal and GPAT attachment.
Past Performance Documents: The Offeror shall provide past performance references for up to three previous contracts. Each past performance business engagement should be as similar as possible to the size and scope of this requirement. Each past performance reference should be no longer than one page. Margins shall be 1” on all sides. All other formatting is at the contractor’s discretion.
Past performance references shall include at a minimum, a description of the service provided, the contract value, a contract number (if a federal reference is used), total contract value, contract type and the name of the recipient company/organization. A point of contact for each reference should also be provided along with that individual’s phone number and email address.
Section 508 Compliance: The Offeror must complete a copy of the GPAT attached to the combined/synopsis solicitation in order to receive consideration for this award. Attachment 2, Section 508 Compliance contains more information pertaining to this requirement. The GPAT shall be submitted as a separate attachment from the technical proposal and price proposal.

All proposals shall be submitted electronically to Justin Fraser at Justin.Fraser@nih.gov by the close date listed on the SF 1449. Late proposals will not be accepted.
