PERFORMANCE WORK STATEMENT

1. SCOPE: The Contractor shall provide all equipment material, supplies, labor, and red line drawings to manufacture two-step platforms for access to the fuel tanks bunghole and one-step platform for Chiller Plant Standby Generator. Stairs and platforms are to be made from high strength steel with galvanized coating to prevent rusting. Contractor is to install on site and ensure form, fit and function. A site visit is highly recommended to ensure proper measurements. Site visits can be scheduled September 11 and September 12, 2019.0700-

1400. Contractor can access the site by calling the NCTMAS LANT Duty Engineer at 757-

443-9180.

2. BACKGROUND: NCTAMSLANT has an immediate requirement at M51 and N26 for step platforms to provide access for two fuel tanks bunghole and one Chiller Plant Standby

Generator step platform. The two fuel tanks N26 AST-05 and M51 AST 06 do not a step platform for access. While performing correct and preventive maintenance on the fuel tanks a portable ladder is required to perform maintenance action. The Chiller Plant Generator step platform have become unreliable due to age and degradation

3. SUBMITTALS:

The Contractor will submit a final report of repair within two weeks after completing work, as applicable. The report will be submitted to the On-site Navy representative via email for distribution.

4. LOCATION: Building M-113 compound and Building N-26 rear parking lot onboard

Naval Station, Norfolk, Virginia 23511.

5. PERIOD OF PERFORMANCE: The contractor shall complete the work within 30 days after notification to proceed. No work may begin without concurrence of the On-site Navy representative. Completion shall include full operational testing of all the step platforms. The contractor will ensure that a final cleanup of job site at the completion of all work.

6. DRAWINGS: The contractor will provide a drawing of the step platforms.

7. DESCRIPTION OF WORK: The Contractor shall provide all equipment, material, parts, supplies and supervision required to perform all tasks outlined in this project within this performance work statement. The contractor will built and install the step platforms for the fuel tanks. Ensure that the platforms are stable and properly grounded to the tanks grounding system. The platform will have rails provided for fall safety of personnel’s.

The Contractor shall manufacture a step platform of metal to replace the existing wooden platform. The contractor will install the platform and test for stability and proper fit in its location at M113 Chill Plant Standby Generator access door for the platform. The contractor will comply with all safety requirement for step platform. The platform will includes a safety rail system that meets OSHA requirements. The height of the rails will be the same as the hand rails for the steps, giving the platform a uniform look all around the step platform with the platform as one unit. The fabrication of the unit and installation includes shop drawings, all labor to install and delivery to job site.

9. OTHER REQUIREMENTS:

A. The access to the work site and the equipment room is restricted. The on-site Navy representative will provide any required escorts. The contractor shall schedule work with the on-site Navy representative to ensure access.

B. The Contractor is responsible for the safety of his employees. The Contractor shall determine if any confined space is a “permit required” confined space. The Contractor shall ensure his employees are adequately trained and qualified to work in confined spaces. The Contractor shall provide a copy of his Confined Space Entry Program to the on-site Navy representative prior to starting work.

C. The Contractor shall follow all OSHA and NCTAMS LANT safety regulations pertaining to the control of hazardous energy, (lockout-tag out). The Contractor shall provide a copy of his Energy Control Program to the on-site Navy representative before work commences. The plan shall contain information sufficient to ensure government employees safety and identify lockout-tag out procedures for hazardous energy sources.

D. The Contractor shall submit, via the Contract Control Authority/ on-site Navy representative, a list with the Full Names, Social Security numbers, Date of Birth, Company Name, of all personnel who will be working on the contract job site on company letter head for access to secure areas. The Contract Control Authority/ on-site

Navy representative will forward to Physical Security Officer for processing.

For Base access, all Contractor employees shall obtain the required employee and vehicle passes from base security prior to the start of the contract. The Contractor will apply and hold “Rapid Gate” ID Badges issued by Base Security. Each Employee shall wear the

Government issued badge over the front of the outside clothing. In the event a “Rapid

Gate Pass cannot be obtained for said contract, at a minimum, a One Day Pass (ODP) form will be requested by Contractor from the on-site Navy representative. The ODP form will require specific dates of base entry required, employee’s full names, Social

Security Number, U.S. citizenship, Date of Birth, Name of Company, Command Name and Point of Contact. The ODP form will need to be sent to the on-site Navy representative and passed to the NCTAMSLANT Security Dept. 48 hours in advance of site visit to verify FBI, and Police background check. On Day of work all personnel must have two forms of photo identification.

E. All Contractors and their employees must report to Base Security Department/ Pass

Office to obtain permission to park each day prior to beginning any operations at the site.

At this time the Contractor and employees will be required to identify and register any private or company vehicles that will be on site in association with the contract. Ensure the driver has current vehicle registration, insurance, updated license plates and vehicle inspection.

Proper operator licensing for vehicle operators is mandatory. Immediately report instances of lost or stolen badges or passes to the on-site Navy representative.

NCTAMS LANT SPECIFIC WIDE AREA WORKFLOW (WAWF) INSTRUCTIONS (JAN 2017)

The information contained in this instruction is supplemental to DFARS 252.232-7006.

The information contained in the table in DFARS 252.232-7006 is for WAWF purposes only. Information included in DFARS 252.232-7006 and this WAWF instruction apply only to WAWF Invoicing and WAWF Receiving Reports. Contradictory information elsewhere in this contract, e.g. Ship to DoDAAC, shall be followed per the terms and conditions of the contract.

When entering the invoice into WAWF, the Contractor shall fill in the DoDAAC fields or DoDAAC extensions exactly as shown in the table in DFARS 252.232-7006. Fields that should not be filled in when entering the invoice into WAWF will be indicated with the direction, “Leave Blank.”

In some situations the WAWF system will pre-populate the “Pay DoDAAC,” “Admin By DoDAAC” and “Issue By DoDAAC.” The Contractor shall verify that those DoDAACs automatically entered by the WAWF system match the information in the table in DFARS 252.232-7006. If these DoDAACs do not match, then the Contractor shall correct the field(s).

If Receiving Reports are required, ensure that the “Inspection” and “Acceptance” defaults of “destination” for both fields are not changed in the WAWF online interface. The CLINs on the WAWF invoice shall be entered exactly as set forth in the contract document including CLIN number (e.g. 0001), Quantity (may be adjusted for actual quantity or dollar value delivered and invoiced), and Unit Price (e.g. $1.00). The dollar amounts on each CLIN or SubCLIN on the WAWF invoice shall reflect final performance values, but in no instance can the dollar amount for each CLIN or SubCLIN exceed what is specified in the contract document. The Contractor shall bill to the lowest level, e.g., the SubCLIN level. The Quantity and Unit of Measure fields must be filled out exactly as indicated in the CLINs and SubCLINs to reduce the possibility of the invoice being delayed or rejected during processing.

Before closing out of an invoice session in WAWF, but after submitting the document or documents, the Contractor will be given the option to send additional email notifications by clicking on the “Send More Email Notifications” link that appears on the page. The Contractor shall click on this link and add the Technical Point of Contact’s (TPOC) or Contracting Officer’s Representative’s (COR) email address in the first email address block and add any other additional email addresses desired in the following blocks.

This additional notification to the Government is important to ensure the acceptor/receiver is aware that the invoice documents have been submitted into the WAWF system.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (DEC 2018)

(a) Definitions. As used in this clause—

“Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.

“Document type” means the type of payment request or receiving report available for creation in Wide Area

WorkFlow (WAWF).

“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.

“Payment request” and “receiving report” are defined in the clause at 252.232-7003, Electronic Submission of

Payment Requests and Receiving Reports.

(b) Electronic invoicing. The WAWF system provides the method to electronically process vendor payment requests and receiving reports, as authorized by Defense Federal Acquisition Regulation Supplement (DFARS) 252.232-

7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall—

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.sam.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training

Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/.

(e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data

Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor shall use the following information when submitting payment requests and receiving reports in WAWF for this contract or task or delivery order:

(1) Document type. The Contractor shall submit payment requests using the following document type(s):

(i) For cost-type line items, including labor-hour or time-and-materials, submit a cost voucher.

(ii) For fixed price line items—INVOICE 2IN1

(A) That require shipment of a deliverable, submit the invoice and receiving report specified by the Contracting

AUTHORIZED CHANGES ONLY BY THE CONTRACTING OFFICER

(a) Except as specified in paragraph (b) below, no order, statement, or conduct of Government personnel who visit the Contractor's facilities or in any other manner communicate with Contractor personnel during the performance of this contract shall constitute a change under the "Changes" clause of this contract.

(b) The Contractor shall not comply with any order, direction or request of Government personnel unless it is issued in writing and signed by the Contracting Officer, or is pursuant to specific authority otherwise included as a part of this contract.

(c) The Contracting Officer is the only person authorized to approve changes in any of the requirements of this contract and notwithstanding provisions contained elsewhere in this contract, the said authority remains solely with the Contracting Officer. In the event the Contractor effects any change at the direction of any person other than the

Contracting Officer, the change will be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in charges incurred as a result thereof.

BASE ACCESS/REPAIR FACILITY

BASE/REPAIR FACILITY ACCESS

Navy installations have begun transitioning to Defense Biometric Identification System (DBIDS) to enhance base security. Rapid Gate/SURE ID is no longer allowed to accept applications for NCAC credentials and installations are no longer issuing NCAC credentials. The current NCAC card, if contractor has one in their possession, is only valid until July 15th of this year, after that it will not be allowed. All contractors need to report to the Pass and ID office and register for the new DBIDS credential that will replace the NCAC card for no cost. Initially contractors will submit for a 180 day temporary DBIDS pass until their new DBIDS credential can be made.

1. NCAC cards received by Pass and ID will not be issued, instead:

a. NCACS cardholders will have 90 days starting from April 17, 2017 to obtain a temporary DBIDS paper pass.

After July 15, 2017, NCACS cards will no longer be accepted for installation access. After the applicant obtains their DBIDS paper pass, they will have an additional 90 days to obtain an actual DBIDS card at no cost to them.

b. To obtain a DBIDS credential and obtain base access; an individual needs to acquire DoD sponsorship, have a thorough identity check, be properly vetted, and have a legitimate reason for base access. Identity checks require a passport or a Real ID Act-compliant state driver's license. Vetting occurs when the individual's background and ID are compared against authoritative criminal justice data bases, and the individual has a letter or official document from the sponsoring organization which articulates the reason for access. When the above criteria are satisfactorily met, DBIDs credentials are issued.

3. One of the added benefits to DBIDS is that the NCACS credentialing process took up to 31 days to complete and the credentialing process for DBIDS only takes 30 to 60 minutes.

DOD Sponsorship shall provide the following;

1) Verify all your NCAC personnel via the list sent out by the Deputy Director of Public Safety. If you have personnel/contractors no longer needing access purge them from your list before sending your update to the Region.

2) The Name of the Company, the company address as well as the Name, Phone Number, and email address of your

POC at the company.

3) The requested days of the week and the hours that contractors will need access to the installation? (i.e. 24/7, Monday through Saturday 0600-2000, or some other combination)

4) The installation sponsor for contractors must be an Active Duty CAC card holder or a Federal Employee with a regular CAC credential with an email address ending in navy.mil and in addition must supply an installation phone number and full name of the Sponsor's Full Name, Job Title.

CONTRACTOR DIRECTION

If you currently have a Navy Commercial Access Control System (NCACS) card (used with Rapidgate), the following is required to get a DBIDS credential:

Present your NCACS Card and a completed copy of the SECNAV FORM 5512/1 to the base Visitor

Control Center representative.

The VCC will pull up your information in the computer, ensuring all information is current and correct.

Once your information is validated, a temporary DBIDS credential is provided.

Your temporary credential will have an expiration date, prior to which you will need to obtain your permanent DBIDS credential (~ 90 days).

For each additional U.S. Navy installation to which you need access, the first time you visit you only need to bring your DBIDS credential and statement of purpose for base access when arriving at the Visitor

Control Center.

The representative will enter base access authorization and then you may proceed to work.

If you do NOT have an NCACS Card, the following is required to obtain a DBIDS credential:

Present a letter or official document from the government sponsoring organization that provides the purpose for your access.

Present valid identification, such as a passport or Real ID Act-compliant state driver's license.

Present a completed copy of the SECNAV 5512/1 form to obtain your background check.

Upon completion of the background check, the Visitor Control Center representative will complete the

DBIDS enrollment process, which includes your photo, finger prints, base restrictions, and several other assessments; after all this is done, you will be provided with your new DBIDS credential.

You may now proceed to work.

If you were issued an NCACS credential PRIOR to 17 April 2017, your NCACS credential will remain in effect for base access through 14 August 2017, when NCACS credentials will no longer be valid.

If you are issued an NCACS credential between 17 April and 30 June 2017 you will also be required to obtain a DBIDS credential in order to obtain base access.

Any new contractor, vendor or supplier requesting base access AFTER 30 June will be required to obtain a

DBIDS credential. NCACS credentials will no longer be issued.

Only DBIDS credentials will be issued to ALL contractors, vendors and suppliers seeking base access AFTER 14

August 2017
