STATEMENT OF WORK

1. General: This is a service contract to provide Maintenance and Repair Services for Histology

Equipment. The Contractor shall comply with Federal, State, and Local Laws, plus any Federal

Regulations as applicable to the performance of this contract. The Contractor shall not accept any instructions issued by any person employed by the U. S. Government, other than the

Contracting Officer (KO) or designated representative all acting within the limits of their authority.

1.1 Description of Services: Contractor shall provide Maintenance and Repair Services for

Histology Equipment as defined in this Statement of Work. The contractor shall perform to the standards in this contract.

1.2 Scope: The contractor shall provide preventive and corrective maintenance in accordance with manufacturers’ recommendation. All maintenance and service shall be performed by competent, Lieca certified, professionally trained and technically qualified service technicians. The contractor shall provide a field service report for all work actions to biomedical engineering division upon completion of work. Such documentation shall be in accordance with current regulations and industry standard practice.

1.2.1 Performance: The contractor shall respond to corrective maintenance requests by telephone within twenty-four (24) hours, on site within forty-eight (48) hours, and have the equipment in good operating condition within seventy-two (72) hours of initial request for service. If, after seventy-two (72) hours, the equipment is not in good operating condition as determined by the operator(s), the head of materials management department shall be notified by the contractor immediately so that appropriate action may be taken. The contractor shall furnish maintenance service support to include as a minimum corrective and preventive maintenance for the equipment to ensure that it is kept in good operating condition. The contractor shall provide all software and part upgrades including price listing (except consumables), labor, service literature (with schematics, diagrams, and identification of parts) and travel necessary to keep the equipment in good operating condition and meet all published specifications listed by the original equipment manufacturer (local service providers are preferred).

1.2.2 Preventative Maintenance: The contractor shall comply with the original equipment manufacturer’s preventive maintenance procedures, which shall include as a minimum lubrication, cleaning, functional checks and adjustments, leakage current measurements, and other maintenance to keep the equipment in good operating condition.

A minimum of two preventive maintenance inspections shall be performed during the contract year. The contractor shall schedule all preventive maintenance inspections with the biomedical engineering division to ensure that the equipment will be made available for service by the equipment custodian.

All field service reports shall be documented using reference to the equipment model and serial no. & contract purchase order number. All field service reports shall be dropped off by the field service representative to biomedical engineering division prior to departure for service history documentation requirements.

1.2.3 Work Verification and Maintenance Records: The contractor shall only respond to service calls placed by biomedical engineering division personnel. For equipment located in the Naval Hospital, Bldg. 2268, the contractor shall check in at the supply building for a temporary vendors badge, and then to biomedical engineering division prior to performing any work. Special tools, hardware, software, parts, test equipment, and equipment maintenance literature shall be provided by the contractor.

1.2.4 Part Availability: Maintenance actions shall not be delayed for more than twenty-four

(24) hours due to awaiting parts. All replacement parts shall be new and of first rate quality.

1.2.5 Point of Contact: The contractor shall provide a point of contact for all maintenance that is required. This information shall include the name, address, and telephone number of contractor service personnel.

1.2.6 Credentialing: The contractor shall provide documentation indicating that technician is certified/qualified to perform such maintenance and or repairs to the manufacturer original specification(s) of said equipment.

1.3 Period of Performance: The period of performance shall be for one (1) Base Year of 12 months and two (4) 12-month option years. The Period of Performance reads as follows:

Base Year 01 October 2019 – 30 September 2020

Option Year 1 01 October 2020 – 30 September 2021

Option Year 2 01 October 2021 – 30 September 2022

Option Year 3 01 October 2022 – 30 September 2023

Option Year 4 01 October 2023 – 30 September 2024

Recognized Holidays: Contractor is not required to perform services on these holidays.

1.4 General Information

1.4.1 Hours of Operation: The contractor is responsible for conducting business, between the hours of 0730-1600 Monday thru Friday except Federal holidays or when the

Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings.

1.4.2 Place of Performance: The work to be performed under this contract will be performed in building 2268 on the first floor room G1016 (morgue).
