STATEMENT OF WORK

1. General: This is a service contract to provide Medical Gas Cylinders. The Contractor shall comply with Federal, State, and Local Laws, plus any Federal Regulations as applicable to the performance of this contract. The Contractor shall not accept any instructions issued by any person employed by the U. S. Government, other than the Contracting Officer (KO) or designated representative all acting within the limits of their authority.

1.1 Description of Services: Contractor shall provide Medical Gas Cylinder Services as defined in this Statement of Work. The contractor shall perform to the standards in this contract.

1.2 Scope: Services include delivery, pickup and rental or lease of different size Medical Gas

Cylinders as specified in Section 1.2.2, including HAZMAT fees. Each delivery shall coincide with a pickup of like (same size) cylinder. The Government will not be responsible for rental or lease of cylinders in excess of the monthly amount specified in Section 1.2.2.

Contractor shall not exceed the quantities requested in Section 1.2.2 without prior written authorization, i.e., (modification) from the Contracting Officer.

1.2.1 For Liquid Oxygen, contractor will install/utilize a telemetry system that will provide

24/7 daily monitor readings. Contractor will deliver bulk liquid oxygen upon request of

Naval Hospital Pensacola (NHP) Materials Management Department (MMD) staff or when telemetry system notifies contractor of low limit readings. Contractor is not authorized to install any type of lock or securing device that will allow contractor access to bulk liquid oxygen cage. Contractor will notify NHP MMD when a driver is dispatched to deliver bulk liquid oxygen. During normal working hours, delivery driver will be escorted by NHP MMD staff to bulk liquid oxygen cage. A delivery ticket and/or manifest will be provided with each delivery. NHP MMD staff will unsecure bulk liquid oxygen cage and allow contractor access to cage. In the event of an emergency delivery during weekends, holidays and non-working hours; contractor must contact NHP MMD

1.3 Period of Performance: The period of performance shall be for one (1) Base Year of 12 months and two (4) 12-month option years. The Period of Performance reads as follows:

1.4 General Information

1.4.1 Hours of Operation: The contractor is responsible for conducting business, between the hours of 0700-1600 Monday thru Friday except Federal holidays or when the

Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings.
