SCOPE OF WORK
(Project name)
U.S. NAVAL HEALTH CLINIC, CHARLESTON
1. GENERAL DESCRIPTION: The Contractor is to provide all labor, tools, materials, equipment, supervision, inspections, commissioning, certification and quality control necessary to provide a turnkey patient Wi-Fi at Naval Health Clinic Charleston (NHCC).
2. CONTRACT REQUIREMENT:
a. Provide and Install Fiber Optic cabling by coming from SCE&G electrical pole adjacent to the dental health building aerially and connecting to existing fiber conduit (provided by NHCC) at the dental health building.
b. Provide and Install an Adtran device in the Communications room at the main Naval Health Clinic building where we will terminate our fiber.
c. Provide and Install Multiple Cisco Switches where all Cat5/6 cabling will terminate. Run Cat5/6 cabling to all designated Access Point placements and hang Access Points in proper designated areas.
d. Perform finish work i.e. patch/paint as needed where (Feature of work here)project has impacted the facilities.
e. All old system (Feature of work here) must communicate with new system or be replaced (where applicable)
f. Provide (Feature of work here).
g.e. Test (Feature of work here) to Design of specifications.
h.f. Provide and install all software needed to program (Feature of work here)complete a turnkey product. Software shall be provided with parallel access independent of programmed version for future updates or settings i.e. alleviate any chance the program is lost.
i.g. Program must have operational access at 3 locations. (buildings 2417, 2418 and 2419) j.h. Provide 16 hours of program training for 8 users.
k.i. Any wall penetrations created shall be sealed with appropriate (FM approved) fire stop caulking.
l.j. Existing cable trays and raceways shall be used where possible.
m.k. Every effort shall be made to route new wiring through existing penetrations. Any deviation from this must be coordinated with the Facility Manager.
3. GENERAL REQUIREMENTS AND PROCEDURES: All workmanship shall meet the standards specified herein and shall be accomplished in conformance with approved and accepted standards of industry equipment manufacturers, federal standards and applicable building and safety codes.
a. All work shall be free of missing components or defects that would prevent it from functioning as originally intended and/or designed. Corrective or repair/replacement work shall include operation checks and cleanup of the jobsite.
b. During and at completion of work, debris shall not be allowed to spread unnecessarily into adjacent areas nor accumulate in the work area itself. All such debris, excess material and parts shall be cleaned up and removed at the completion of the job and at the end of each day, work is in progress.
c. All work is to be accomplished in accordance with NHCC safety instructions and guidelines.
d. All work shall be accomplished in accordance with NHCC Infection Control Policies and Procedures.
e. All work shall be planned, designed, constructed, and tested in accordance with all Uniform Facilities Criteria (UFC) documents, (state applicable references here) specifically, referenced codes and standards therein. Installation must meet National Electric Code and all industry standards in this regard.
f. The contractor must have appropriate current state licensing.
g. Regular work hours shall be between 7:30 a.m. and 4:30 p.m., Monday through
Saturday Excluding Sundays, and Government holidays.
h. All work locations and times shall be coordinated with NHCC IT Manager and NHCC
Facility Manager in order to maintain all building Life Safety features, egress, and patient flow to and from the building.
i. Contract personnel will not be authorized to park in the patient parking lot.
Contractor is responsible for any and all damages incurred during construction/installation. Work Trailers/Conex Box: Contractor will be required to have the following information on their trailer/conex box: Company name, POC, phone number for contact. Contractor is required to keep area around their trailer/conex box cleaned up.
j. Contractors are liable for any enforcement actions, fines, and/or penalties resulting from their failure to comply with applicable environmental/regulatory requirements.
k. Any work that produces noise and/or vibration or any other issue that impeded work of the clinic staff or patients shall coordinated through the Facility Manager.
l. Contractors will wear badges at all times.
4. SITE SAFETY HEALTH OFFICER (SSHO): Contactor shall provide a Site Safety and Health Officer
(SSHO) at the work site at all times to perform safety and occupational health management, surveillance, inspections, and safety enforcement. The assignment of the SSHO does not relieve the Contractor from the requirements governing safety responsibility. EM385-1 shall be followed.
5. INSPECTION AND APPROVAL: All material and workmanship shall be subject to Inspection and approval of NHCC IT manager and NHCC Facility manager.
6. GENERAL ADMINISTRATIVE REQUIREMENTS:
Submittals: Submittals shall be provided electronically with a least one follow-up paper copy for record purposes.
The following items shall be provided within 21 calendar days of project award. Government review will occur within 7 days after receipt.
a) Schedule of Work
b) List of Proposed Subcontractors
c) Health and Safety Plan
e) Quality Control Plan
f) Accident Prevention Plan
g) Infection Control Plan
6. EXAMINATION OF PREMISES: The contractor shall verify all field conditions, dimensions, quantities, current condition and all work necessary to complete the job in accordance with the contract. For current conditions drawings, if they exist, contact Naval Health Clinic Facility Manager at (843) 794-6943 or Naval Health Clinic Contract Maintenance Manager at
(843) 794-6948.
7. COMMENCEMENT, PROSECUTION AND COMPLETION OF WORK: The Contractor shall complete work within (show the estimated number of days for duration of project here)120 calendar days after the date of Contract award. The Contractor is required to prosecute the work diligently and all workmanship shall be of the highest quality.
8. MINOR MATERIALS AND WORK: Minor materials and work not specifically mentioned herein but necessary for the proper completion of the specified work shall be furnished without additional cost to the Government.

Submission of a bid proposal by contractor constitutes acknowledgement and acceptance of all items indicated in this Statement Of Work. Contractor agrees to entire scope of work as written unless specifically removed or changed in his proposal. Items omitted and/or changed from this Statement of Work shall be clearly and individually addressed on a separate sheet(s) of paper titled "DEVIATION OF STATEMENT OF WORK ITEMS"
