Statement of Work Calibration and Corrective Maintenance Contract

1.0 GENERAL INFORMATION

The contractor shall provide all personnel, equipment, tools, materials, supervision, and services necessary to perform periodic maintenance and repairs as defined by the manufacturer for those items specified as government furnished property listed below in Appendix 1. The contractor shall perform to the standards in this contract unless otherwise specified in writing.

1.1 SCOPE

The contractor shall provide all labor, calibration testing equipment and parts necessary to perform maintenance actions mandated by the original manufacturer specifications and periodicity (maintenance schedule).

1.2 QUOTATION/OFFER

The contractor shall submit a firm fixed price quotation/offer that covers all repairs, utilizing the manufacturer specifications.

1.3 PROPOSED CONTRACT DURATION

The contract will be for a period of one-year from the date of award with 2 option years, which may be exercised unilaterally by the government.

1.4 CALIBRATION

This activity should be the procedure for mechanical and electronic adjustments required by the original manufacturer to ensure consistent, standardized performance or output.

1.5 CONTRACTOR PERSONNEL QUALIFICATIONS

The contractor shall utilize only fully qualified technicians that have gone through all/any mandated original equipment manufacturer or a comparable third-party service school to perform maintenance and repairs. If requested, documentation supporting this information shall be provided to the Bio-medical point of contact.

1.6 PREVENTIVE MAINTENANCE (See Appendix 1)

Any and all parts necessary to perform the preventive maintenance and repairs will be included. Any additional costs associated for repair parts will be in written form with description and approved by the Biomedical point of contact before initiation.

2.0 RECALLS

The Contractor shall schedule all recall maintenance with the Biomedical point of contact for contract services to avoid disrupting patient care and to notify the department of arrival, services, parts and maintenance required.

2.1 OPERATIONAL IMPROVEMENT

Equipment operational improvement without modifications shall be provided in written form and approved by the

Biomedical point of contact for contract services.

2.2 ACCOUNTABILITY

Contractor representative must check in / check out with the Biomedical Repair Division prior to commencement and upon completion of work. Base access will be the responsibility of the field service provider and or contractor.

2.3 CONTRACTING STICKER

To show completion of services rendered to our customer clientele the service provider will affix a color coded, heavy duty, self-adhesive, calibration sticker to each piece of equipment maintained.

Sticker Details:

- Date of Calibration/Preventive maintenance/ Inspection

- Due date of next Calibration

3.0 CORRECTIVE MAINTENANCE (REPAIRS)

The items that require maintenance or repair, to bring the unit back to working condition per original manufacturer specifications are required to be reviewed and approved by the Bio-medical contracting point of contact. A written outline of the fault and the corrective maintenance to resolve the concern will be approved by Biomed point of contact before commencing any action in the form of a field service report. The contractor will provide all parts needed for this action.

3.1 Ensure that the original design and functional capabilities will not be changed, modified or altered unless authorized in writing by the manufacturer and the Biomedical Repair Division. If any action requires a contractual change to cover expenses, only the Contracting Officer has the ability to authorize this change. Any contractual changes will be issued in the form of a modification to the contract.

3.2 REPAIR PARTS

The contractors’ quotation/offer must include the price for all parts that are needed for maintenance and repair

4.0 AVAILABILITY AND TRAVEL

The contractor shall deliver ll maintenance and repair services between the hours of 7:30AM through to 5:00PM, Monday to Friday. Any other hours will be at agreed upon between the Bio-medical point of contact and the contractor without additional cost.

4.1 TRAVEL

Unless specified elsewhere in the contractor, all travel cost, including those required to obtain access to government facilities (Rapid Pass), are the responsibility of the contractor.

5.0 DOCUMENTATION

Service event documentation. Upon completion of each service or maintenance visit the field service technician must submit a signed service report to the Bio-medical point of contact for services within 72 hours of completion. The service report shall indicate all problems addressed, devices checked, parts replaced or repaired with identification, parts ordered and any issues resolved. Each service report must at a minimum document the following data legibly and in complete detail.

5.1 REPORTING DETAILS

Will consist of the following information listed below.

- Name of contractor.

- Name of technician.

- Control number for the contractor or log number for the report.

- Description of the work performed in detail.

- Equipment identification: Manufacturer, make, model, serial number, electronic control number or (ECN)

- Itemized description of services performed with labor time.

- Action taken and or recommendations for consideration.

- Pending actions or resolutions to any problems.

- Total billing for services provided should be clearly indicated.

- Signature of the field service technician that conducted the maintenance.
