STATEMENT OF WORK
US Naval Hospital Bremerton
Medical Gasses
Scope
The effort required hereunder shall be performed in accordance with this Statement of Work and all other terms and conditions set forth herein. The contractor shall fill Leased/Government owned cylinders with medical-grade cylinder gases, furnish medical grade solids, and provide delivery/ pick-up services to the following locations:
U.S. Naval Hospital Bremerton
Branch Health Clinic Everett
Branch Health Clinic Bangor
Puget Sound Naval Shipyard Clinic (PSNS)
The gases/solids include compressed Medical Air USP, Oxygen USP, Carbon Dioxide USP, Nitrogen NF, Nitrous
Oxide USP, Helium USP, Lung Diffusion Mixtures, Liquid Oxygen, Dry Ice, and may include others as stated in this solicitation.
Installation Access
Defense biometric identification system (DBIDS)
(a) In accordance with CNICMEMO dated May 5, 2017, individuals currently using an NCACS credential for
Installation access are required to switch to a DBIDS no later than 14 August 2017. After 14 August 2017, the
NCACS credential will no longer be valid for access to Navy Installations.
(b) NCACS users may visit the local Navy Installation Visitor Control Center to obtain a DBIDS credential. To ensure uninterrupted Installation access, current NCACS credential holders are encouraged to shift to the DBIDS credential as soon as possible as Navy does not have the ability to extend this deadline. There are no fees incurred by the contractor, vendor, or supplier to obtain a DBIDS credential.
(c) Individuals who apply for NCACS credentials during the period of 17 April through 31 May 2017 must also obtain DBIDS credentials. NCACS credentials issued after 17 April 2017 will no longer be accepted without an accompanying DBIDS credential for Navy Installation access.

(c) Vendors, contractors, suppliers and other service providers shall present their pass upon entry at Gate.
Contingency/ Emergency
The contractor will provide written procedures for U.S. Naval Hospital Bremerton and its Branch locations for protocols to accomplish emergency shutdowns or other sudden, unplanned termination of the refilling process.
Contractor will provide 24/7 emergency contact name(s) and telephone number(s) to be used in emergency situations. Contacts from the facility to the contractor concerning emergency situations must be responded to within one hour to confirm receipt of emergency notification to ascertain the nature of the emergency.
Inclement Weather
Upon projection of Inclement Weather (i.e Snow Storm), contractor is to pick-up and replace empty cylinder(s) and or fill bulk tanks the day before.
Government Observed Holidays
Contractor is to pick-up empty cylinder(s) the day before and delivers re-filled cylinders the day after the holiday.
Cylinder Verification
Prior to first filling, and annually thereafter, the contractor must verify, in writing, the accuracy of all gauges on all contractor and Government owned tanks.
Product Specification
All medical gases shall conform to specifications prescribed in the United States Pharmacopeia (USP) and the
National Formulary (NF) or current regulation. All medical gas manufacturers and fillers of medical gases shall be registered with the FDA and all medical gases shall be manufactured, processed, packed, transported, and stored according to the FDA’s Current Good Manufacturing Practice (CGMP) regulations and Title 21, Code of Federal
Regulations, Parts 210, 211 and 201 for labeling.
In addition, the following specifications are incorporated into the solicitation and resulting contracts:
a. Federal Specification BB-A-1034B, Air, Compressed, for Breathing Purposes
b. Federal Specification BB-C-101B, Carbon Dioxide (CO2) Technical and USP
c. Compressed Gas Association (CGA) Publications G-7-Compressed Air for Human Respiration, ANSI/CGA G-7.1- Commodity Specification for Air
All products, equipment, and services provided under the contracts awarded under this solicitation shall be in compliance with all applicable Federal, State, and local regulations.
Delivery Requirements
Contractor shall comply with the security regulations in force at Naval Base Kitsap in order to gain entry for contract performance. Orders for medical gases will be placed weekly via phone call to the contractor. The ordering activity shall notify the contractor of the quantity, size, and type of empty cylinders ready for return and quantity, size, and type of medical gases to be delivered. During normal operations, the contractor will have 7 calendar days after receipt of order to deliver filled cylinders and pick-up empty cylinders. Contractor must be able to deliver gases within 24 hours upon ordering, if needed. FOB point is destination. All transportation charges for cylinders shall be included in the price of gas.

Non-Repairable Government Property
Disposition of non-repairable Government-owned cylinders is the responsibility of the ordering activity, USNH
BREMERTON.
Marking
In addition to markings required by the aforementioned laws and regulations, markings shall comply with
Compressed Gas Association (CGA) Publications C-7- Guide to Preparation of Precautionary Labeling and Marking of Compressed Gas Containers, dated 03/21/04.
Color Coding
Cylinders shall be color coded in accordance with current Food and Drug Administration (FDA) Regulations.
Valves
Valves shall comply with CGA Safety Bulletin SB-26 and CGA Publication V-1. (CGA Standard for Compressed
Gas Cylinder Valve Outlet and Inlet Connections, dated 12/31/00). Valve protection caps for cylinders designed to receive such caps shall be securely attached to the cylinders in a manner to protect the valves from injury during transit and delivery to the purchasers receiving area.
HYDROSTATIC TESTING
Retest Date
Contractor will be required to retest any Government-owned cylinder when requested by ordering activity, regardless of scheduled retest data.
Method Government-owned cylinders which are eligible for extended hydrostatic retest period, testing by modified hydrostatic method, or visual inspection in lieu of hydrostatic testing, shall be serviced by the most economical means which will comply with the DOT regulations (49 CFR 173.34) unless otherwise specified by the ordering activity. When modified or reduced DOT test requirements are contingent upon past usage of the cylinders, a written statement from the ordering activity indicating that the cylinders have been used exclusively for a specific gas may be considered acceptable evidence of the cylinders eligibility for modified testing or inspection. The Government shall pay for the hydrostatic testing of Government-owned cylinders at the prices outlined in the contract.
Cylinder Sizes
However not limited to the cylinder sizes shown below; the listed sizes indicate the product capacity of those currently being used. All capacities are approximate, and the Government shall be billed based on cylinder size delivered. In accordance with the Federal Food, Drug and Cosmetic Act, the actual contents of the container must appear on the label.

Individual Agency Ownership of Cylinders
Government-owned cylinders shall remain the property of the individual ordering (owning) agency and shall not be exchanged for other cylinders, either Government-owned or contractor-owned, without authorization of the ordering agency.
Lost or Damaged Contractor-Owned cylinders
If a cylinder is lost or damaged beyond repair while in the Government’s possession, the Government shall reimburse the contractor after 30 calendar days (time period of 30 days is set in case the Government locates missing cylinder). The Government shall pay the contractor the replacement value, less the allocable rental paid for that cylinder. If damaged, the cylinder will become the possession of the Government after payment has been made to the contractor.
Subject to availability of funds and a review and determination by the contracting officer, The Government shall pay the contractor the replacement value, less the allocable rental paid for that cylinder.
Method of Shipment
Empty Government-owned cylinders that are picked up from an ordering facility for re-fill or servicing shall be transported according to the DOT Regulations, using normal commercial practice to prevent damage to cylinders.
PRICE OF SERVICES FOR GOVERNMENT-OWNED AND CONTRACTOR-OWNED CYLINDERS
Services Included in the Price of Gas (Government-Owned Cylinders):
The offeror’s price for the gas shall include the furnishing, at no additional cost, of all services which are required at each and every filling of a cylinder to comply with applicable regulations, specifications, and normal good practices.
Such services shall include, but not be limited to: Tags indicating cylinder is “Full,” “In Use,” or “Empty,” inspection, testing, evaluation, and cleaning services required at each and every filling; and attachment of
Government-furnished warning tags, when required. In addition to services listed in this paragraph, the contractor is responsible for all maintenance and testing of cylinders.
Striping and painting: Includes all lettering, stenciling, affixing of decals, metal lettering and/or number stamping when required by applicable regulation.
Measurement and conversion computation, atmospheric gases and hydrogen:
Density and data volume measurement equivalents printed in CGA Publication P-6 Standard Density Data, Atmospheric Gases and Hydrogen, dated 03/08/00, shall be used when necessary to convert measurement of these gases from one form to another; for example, from cubic feet to gallons.
