Information shall include Contractor name, contract title, type of contract, name of activity, installation or location where accident occurred, date and time of accident, names of personnel injured, extent of property damage (if any), extent of injury (if known) and brief description of accident (to include Type of construction equipment used, PPE used, etc.). Preserve the conditions and evidence on the accident site until the Government investigation team arrives on site and Government investigation is conducted.
• Definition of a Recordable Injury or Illness. Any work related injury or ill ness that results in:
(1) Death, regardless of the time between the injury and death, or the length of the illness;
(2) Days away from work;
(3) Restricted work;
(4) Transfer to another job;
(5) Medical treatment beyond first aid;
(6) Loss of consciousness;
(7) A significant injury or illness diagnosed by a physician or other licensed health care professional, even if it did not result in a. through f. listed above.
• Monthly Exposure Report. This report is a compilation of employee-hours worked each month for all site workers, both prime and subcontractors, and shall be recorded daily on the Contractor Production Report.
• The Contractor's workspace may be inspected periodically for OSHA and Navy violations.
Abatement of violations will be the responsibility of the Contractor and/or the Government as determined by the Contracting Officer. The Contractor shall provide assistance to the Safety Office escort and the federal or state OSHA inspector if a complaint is filed.
