GENERAL: This is a combined synopsis/solicitation (solicitation number N6328521Q0049) for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR)
Subpart 12.6 Streamlined solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation.
The Naval Criminal Investigative Services (NCIS) has a requirement for a Commercial-Off-The-Shelf solution to support a Training Records Management System, as outlined in the Statement of Work (SOW).
In accordance with FAR Part 12 and 13, the NCIS, Acquisition Division (Code 14B) is issuing a commercial solicitation for the acquisition of such services. Required specifications are listed in Schedule of Supplies/Services within this solicitation and in the Statement of Work (SOW). A Request for Quote
(RFQ) is being requested and a separate written solicitation will not be issued.
This solicitation document and incorporated provisions and clauses are those in effect through Federal
Acquisition Circular (FAC) 2021-05, Effective March 10, 2021 and Defense Federal Regulation
Supplement (DFARS) Change 02-24-2021. It is the responsibility of the contractor to be familiar with the applicable provisions and clauses, which can be accessed in full text at www.acquisition.gov.
CONFIDENTIAL INFORMATION: The Freedom of Information Act (FOIA) and its amendments have resulted in an increasing number of requests from outside the Government for copies of contract qualifications and quotes/proposals submitted to federal agencies. If a Quoter’s submission contains information that he/she believes should be withheld from such requestors under FOIA on the grounds that they contain “trade secrets and commercial or financial information” (5 USC 552(b)(4)), the Quoter should mark its submission in the following manner:

STATEMENT OF WORK
FOR
NAVAL CRIMINAL INVESTIGATION SERVICE (NCIS), TRAINING ACADEMY WEAPONS
QUALIFICATION AND TRAINING
RECORDS MANAGEMENT SYSTEM
1. BACKGROUND
The Naval Criminal Investigative Service (NCIS) is the federal law enforcement agency charged with conducting investigations of felony-level offenses affecting the Navy and Marine Corps. NCIS is comprised of approximately 2,000 personnel in over 40 countries around the globe. The organization is comprised of approximately 90% civilian and its cadre of federal special agents, about half its total http://www.acquisition.gov/ personnel, is approximately 98% civilian. The NCIS mission is to investigate and defeat criminal, terrorist, and foreign intelligence threats to the United States Navy and Marine Corps, wherever they operate, ashore or afloat.
The NCIS Training Academy’s mission is to serve as the premier provider of training for NCIS employees.
The Academy provides the tools, resources and support to help new agents realize their full potential within
NCIS and is responsible for providing relevant, timely, substantive, and cost effective training programs while supporting the NCIS strategic plan by providing the tools to build a highly skilled workforce. The
Academy has responsibility over all basic, advanced, and in-service training programs for the entire agency.
The in-service training (IST) program managed by the Training Academy is set up to ensure that NCIS staff not only possess, but maintain the technical knowledge, experience, and developmental job skills required to fulfill their duties.
The Naval Criminal Investigative Service is currently unable to accurately track Agent certifications or accurately maintain legally defensible training records and its current internally developed Field Training
Agent Software (FTEMP) is no longer useable agency wide in the current NMCI work environment. NCIS currently cannot track training more than 2 years back and our accredited course curriculum is not directly linked to a specific class. Agent Basic Accredited training records exam results and training quality surveys required to be held for 40 years are currently hosted separate individual documents and locations. NCIS is required to track all weapons assigned to Special Agents by serial number and their quarterly qualifications on each weapon for their entire career. The current system is unable to accomplish this and we are out of compliance on weapons qualifications and tracking. The Training Academy is looking for a single enterprise-based system to meet all of these requirements, one that will track all Agents Training certifications for their entire career.
NCIS currently uses a disparate, disjointed set of programs to support and manage the training environment, with some of the biggest problems being:
a. The current NCIS weapons qualification tracking system uses manually entered data, potentially causing erroneous data to be entered. Queries of data is also a manual process and must be conducted, by office and individuals. The weapons inventory information, training required/completed, and qualifications required/completed can be inaccurate depending on the input of the manually entered data.
b. Training records are not able to be queried in an accurate, agency-wide format further back than two (2) years. NCIS needs a centralized system to track all training and certifications covering an individual’s entire career.
c. Managers and supervisors need to have access to completed training, certification, and qualification records of their assigned staff.
d. Training records cannot be deleted or updated to correct errors in the current system, if an entry is made with an error it stays on file for the individual’s entire career.
e. The Field Training Evaluation Program (FTEP) is part of the Special Agent Basic Training which are broken down into: Phase I is the Pre-Basic Orientation Program; Phase II is the formal academic training period; Phase III is the FTEP/on the job training (OJT) evaluated portion; Phase IV begins once the trainee completes Phase III and all evaluations of the standard evaluation guidelines (SEG) are at an acceptable level. The SEGs are required for a Special Agent and are taught in federal law enforcement training programs. Currently, the FTEP management system has no ability to be upgraded and linked/stored with an individual’s training record. The current program was developed in-house and is no longer fully functional across the agency due to Navy Marine Corps
Internet and NCIS inter-operational limitations.
f. NCIS does not have a system or program to track the agency required certifications, associated training, and renewal compliance.
2. SCOPE AND OBJECTIVES
The Training Academy requires an Enterprise Training Records Management System solution that can be installed on NCIS’ systems as a self-hosted service on NCIS servers to provide accurate, legally defensible, detailed training records for NCIS, all on one platform that will track every employee’s career development, from basic training to retirement. The system should also support Special Agent equipment and weapons inventory tracking. In addition, NCIS needs the system to track the Field Training Evaluation Program
(FTEP), Training Academy Federal Law Enforcement Training Accreditation curriculum, all agency training certifications, and provide completely automated weapons qualifications tracking and reminders.
The final solution should be compatible with current NCIS office systems (Exchange 2019, Outlook 2019, Word 2019, etc.) as well as Office365 documents. Additionally, NCIS’ data center operates a VMWare server environment.
3. TECHNICAL SPECIFICATIONS
3.1 Records Management Solution. The Contractor shall provide a final solution with the capability to support the following functions in an enterprise-based solution hosted on the NCIS internal systems. The final solution must be executable software or issued via license usage for the commercial software product.
The Contractor shall grant to the Government all license rights customarily provided to the public in commercial sales of the delivered software products. Licenses delivered shall include all users of support, production, and COOP environments. The Contractor shall provide, in their quote, the required operating systems and server configurations necessary to ensure successful installation and operation of their product(s) on NCIS VMWare servers. DEL. 1
The number of administrative users in the production and development environment is expected to be less than 5 (five) concurrent users. The number of general users is expected to be 1,180.
I. Basic Academy Training - A centralized System supporting a single record for each Agent that aggregates all training data to ensure they’re properly qualified and certified and that integrates with the Field Training Agent, and Weapons Qualification programs. Specifically the Contractor’s solution shall:
i. Manage, score and track observed skills and practical exercises
ii. Automate graduation processing (final grades, certificate conferral, personnel records updates)
iii. Provide full integration with the Field Agent Training and Weapons Inventory and
Qualifications modules (See Section II)
iv. Add and edit personal records
v. Track employment, title/rank, status
vi. Print and email certificates from personal record
vii. Custom tagging of personal records for quick grouping or identification
viii. Attach documents to individual personal records
ix. Create/manage curriculum templates for standardized class creation
x. Track correlation of learning objectives between curriculum and tests
xi. Define class dates
xii. Add and manage training locations
xiii. Send email to students in staff assigned classes
xiv. Assign staff with specific roles in classes
xv. Print and email student attendance summary
xvi. Student metrics for classes
xvii. Record test scores including retests, observers, and comments
xviii. Track student completion status
xix. Manage student departures with action, reason, and reason details
xx. Automatic or manual graduation processing

II. Field Training Agent – The Contractor shall provide a centralized Field Training Agent system that integrates with the Basic Academy and Training Records Management Systems. Specifically, the Contractor’s solution shall:
i. Provide a valid and reliable job-related evaluation system to be utilized during the FTEP utilizing a standardized and systematic approach to the documented measurement of the trainee’s development and performance
ii. Support the setup, manage, grade and track the agency competencies and rating standards
iii. Support the setup and maintain program-rating scales
iv. Provide real-time reporting, accessible from any field office through the current
DON/NCIS systems, 24 hours a day, with a help-desk option. If access becomes unavailable, an off-line versions or system is available in order to ensure training information can be created/uploaded/merged when the system resumes availability
v. Provide management with real time compliance tracking data
vi. Document and track remediation actions
vii. Automatically append evaluations to agent training records
viii. Assign performance evaluation reports to students in a training class, incorporating class-level, activity-level, or test-level grades
ix. Assign performance evaluation report to an individual from their personal record
x. Provide the ability to provide overall conduct and performance report with Pass/Fail evaluation
xi. Provide automated email notifications for reports pending approval
xii. Provide automated email notifications for reports sent back for editing
xiii. Provide records maintained on personal record
xiv. Display and manage field training plan versions
xv. Close a field training plan as successful for trainee who has demonstrated proficiency without submitting all required training reports
xvi. Close a field training plan as unsuccessful for trainee who has completed the FTEP but not successfully completed assignments
xvii. Provide a view list of field training plan trainee assignments in the portal
xviii. Provide a view of organization training performance reports in the portal
xix. Print all items in a training performance report

III. Weapons Inventory and Qualifications Tracking – The Contractor shall provide a centralized
Weapons Qualification Tracking system that integrates with the Basic Academy and Training
Records Management programs. Specifically, the Contractor’s solution shall:
i. Manage special agent assigned gear and weapons
ii. Track inventory information
iii. Track certifications, qualifications, and expirations that are currently in the various systems and will require the system be National Archives and Records Administration (NARA) compliant
IV. Training Records Management – The Contractor shall provide a centralized Training Records
Management system that integrates with the Field Training Agent, Basic Academy and Weapons
Qualification programs. Specifically, the Contractor’s solution shall:
i. Track and view individual training history
ii. Track certifications and qualifications with expiration dates
iii. Track classroom, in-service and online training
iv. Store comprehensive “hire-to-retire” records
v. Maintain legally defensible training records showing training attempted, field training agent
(FTA)/rater comments/remarks, and end rating so when an employee is fired or subpoenaed, training records will be a basis for action
vi. Provide access for all applications with just one user profile using either via a single sign-on (SSO)
3.2 Configuration and Installation. The Contractor shall support installation onto NCIS systems that include configuration of the COTS product(s) to satisfy the functional requirements as identified above. The expectation is for requirements to be satisfied by configuration of the standard COTS software. The
Contractor shall ensure that the software is properly installed within the appropriate support and production environments. This includes ensuring that the application is functioning in accordance with user requirements per as outlined above. The Contractor shall provide a weekly status of these activities up to full operational capability. DEL. 2
3.2.1 The Contractor shall be capable of incorporating any required software baseline changes during implementation into its proposed COTS software solution in order to minimize customization.
3.2.2 The Contractor shall ensure administrative users are capable to install software pushes and security updates as needed to maintain operation and full functionality throughout the base and optional periods.
3.2.3 The Contractor shall provide product manuals in support of the solution. Product manuals will include system manuals that will support implementation, administration, and user operation, specific to the system.
Software products that are delivered will be specified in the Software Version Description (SVD). Product manuals shall be delivered as electronic media. DEL. 3
3.3 Support Sustainment. The Contractor shall provide an On‐Line Help (e.g., self‐help, self‐service) capability to assist users in performing the functions with the programs capability.
4. DELIVERABLES
All information, records, data, files, documents, and work papers obtained or created in support of this
SOW shall become and remain the property of the Government and shall be returned or disposed of in accordance with the COR’s instructions at the conclusion of performance. The Contractor shall provide the
Government or other agencies, access to any and all program information and data prepared or obtained as part of this contract or in support of any tasking of this SOW.

5. GOVERNMENT FURNISHED RESOURCES
5.1 The Contractor may be provided Government Furnished Information (GFI), such as software (e.g.
laptops) to support access to DoD and Navy Marine Corps Intranet (NMCI) networks. The Contractor shall acknowledge receipt of all GFP/GFE/GFI in the GFP, Status and Management Report, and notify the
Government of any concerns and risks identified upon receipt of GFP/GFE/GFI. The Contractor shall maintain a master GFP/GFE/GFI record showing the disposition and version/status of the items held.
6. SECURITY REQUIREMENTS.
The Contractor shall be a United States-based company, licensed, chartered, or incorporated in the United
States.
All Contractor personnel shall be United States citizens and be capable of meeting minimum background checks prior to performance of the effort. Government personnel at NCIS Headquarters will facilitate in obtaining installation access, if needed. Contractor personnel must supply full name, personal identifiable information, contact phone numbers, and email addresses to the NCIS Technical POC at least five (5) business days prior to arrival in order for NCIS to conduct a background check and approve access to the site. Contractor personnel will be required to meet NCIS’ security requirements to be granted access to
NCIS systems. This included successful completion of the DISA Cyber Security Challenge (certificate required to be submitted before work is started). The Cyber challenge is located at https://cyber.mil/training/cyber-awareness-challenge/
All access and documentation shall be returned to the Government at the completion of the support services or upon termination or resignation of employment. Compliance with all NCIS communication regulations pertaining to access to e-mail and Internet activities is mandatory, as applicable. Failure to comply with security requirements is grounds for termination.
Access to the NCIS HQ is a privilege that may be granted, denied, or withdrawn by NCIS. Denial or withdrawal may occur as a result of any type of misconduct or incident determined contrary to the best interest of NCIS’ mission, protection of property, welfare of personnel and for any other infraction determined justifiable for denying access.
8.1 The Contractor shall comply with the following:
a. No employee or representative of the contractor will be admitted on site unless they furnish satisfactory proof of United States citizenship or, if an alien, that their residence and employment within the United
States is legal.
b. All Contractor employees must possess a valid picture identification card issued by a State or Federal agency.
c. Contractor employees shall conduct themselves in a proper, efficient, courteous and business-like manner.
d. Contractor employees entering NCIS sites shall comply with all security requirements in effect during the contract periods and shall be subject to such checks as may be deemed necessary. The Contractor should contact each installation prior to beginning work on-site to ensure the Contractor understands and can comply with individual installation security requirements.
7. NMCARS 5237.102-90 Enterprise-wide Contractor Manpower Reporting Application (ECMRA)
(a) DOD contracting activities awarding or administering contracts shall incorporate the following
Enterprise-wide Contractor Manpower Reporting Application (ECMRA) standard language into all contracts which include services, provided the organization that is receiving or benefiting from the contracted service is a Department of Defense organization, including reimbursable appropriated funding sources from non-DOD executive agencies where the Defense Component requiring activity is the executive agent for the function performed. The reporting requirement does not apply to situations where a
Defense Component is merely a contracting agent for another executive agency. Contracted services excluded from reporting are based on Product Service Codes (PSCs). The excluded PSCs are:
(1) W, Lease/Rental of Equipment;
(2) X, Lease/Rental of Facilities;
(3) Y, Construction of Structures and Facilities;
(4) D, Automatic Data Processing and Telecommunications, IT and Telecom- Telecommunications
Transmission (D304) and Internet (D322) ONLY;
(5) S, Utilities ONLY;
(6) V, Freight and Shipping ONLY.
(b)The standard language to be inserted is:
“The contractor shall report contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for Interview and Interrogation Training Support via a secure data collection site. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the help desk, linked at https://www.ecmra.mil.
PROCUREMENT APPROACH: The Contractor’s quoted item descriptions shall reflect the characteristics and level of quality that will satisfy the Government’s need as described in the Schedule of
Supplies/Services. A single firm-fixed price contract will be issued and it is the Government’s intent to award on an “all-or-none” basis to satisfy this requirement. The Government reserves the right to issue no award at all resulting from this solicitation.
PAYMENT TERMS: NET 30. Invoices must be submitted via the Wide Area Work Flow (WAWF) in accordance with DFARS 252.232-7006. Invoices MUST be submitted via the WAWF to be processed for payment. See Statement of Work for more details on technical specifications and installation.
REQUIRED DELIVERY/PERIOD OF PERFORMANCE: The Contractor shall ensure delivery of final
COTS product within 90 days of contract award. Performance shall start at commencement of contract through for a period of 12 months with support and sustainment of the product executed via four (4) 12-month option periods with the option to invoke FAR 52.217-8 Option to Extend Services for up to 6 months.

TECHNICAL ACCEPTANCE: To ensure technical acceptance:
a. The Quoter must clearly demonstrate that the quoted solution meets the solicitation requirements.
Quotes shall provide detailed item specifications, literature, and/or brochures that provide information demonstrating the item quoted meets the solicitation. A mere statement that the
Quoter will meet the Government’s requirement is NOT sufficient.
b. The Quoter must also clearly demonstrate they can provide the configuration and/or installation of the procured solution onto NCIS systems.
c. The Quoter must clearly state whether they will meet the required delivery timeframe. Quoters who cannot meet the required delivery timeframe will not be considered technically acceptable.
d. The Quoter shall provide a URL-based self-guided demonstration of the product that the
Government can evaluate for functionality, completeness of solution, and how the product meets the requirements. A demo will NOT be used to enter production data, nor will any data entered in to the demo need to be retained for more than 60 days past submission deadline.”

52.212-2 ADDENDUM: EVALUATION – COMMERCIAL ITEMS
EVALUATION. Award shall be made to the responsive and responsible Quoter who has the lowest price and is technically acceptable. The Government intends to make a single Firm Fixed Price award to satisfy this requirement. The Government intends to evaluate quotes and award a contract without discussions with vendors. Award will be based on the initial evaluation of quotes received in response to the solicitation.
Therefore, Quoters are cautioned that their initial quotes should contain their best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions with vendors if later determined by the Contracting Officer to be necessary. The Government may reject any or all quotes if such action is in the public interest; and may waive informalities and minor irregularities in quotes received.
Pricing submitted for the base period and option periods will be evaluated. See clause 52.217-8 which authorizes the Government to require continued performance of any services within the limits and at the rates specified in the contract. For the purpose of evaluation only, the Government will evaluate this potential option to extend services by adding six (6) months of the Quoter’s final option period price to the
Quoter’s total evaluated price. Quoters are notified that the Government may choose to use the Option to
Extend Services clause at the completion of any performance period.
TECHNICAL. The following ratings/definitions will be utilized:

Acceptable Quote meets the technical requirements of the solicitation.
Unacceptable Quote does not meet the technical requirements of the solicitation.
The Government may also use information such as Contractor Performance Assessment Reporting System
(CPARS), DoD PPIRS and the FAPIIS data available from Government sources to evaluate a Quoter’s past performance.
PRICING. The price quote should avoid ambiguities of inclusion, exclusion, or content.
The Government will evaluate Quoter’s quoted prices, but this factor will not be given an adjectival rating.
Quotes will be evaluated on completeness, accuracy, and reasonableness. Due to budgetary limitations, in order to be considered acceptable the quoted price including all option periods must not exceed a total price of $230,000.00. Evaluation of the quoted prices for the services will be based on a comparison of quoted prices received in response to the solicitation and may also be based on a comparison to the Independent
Government Cost Estimate (IGCE) along with any other relevant price analysis techniques deemed appropriate.
Reasonableness. For the price to be reasonable in its nature and amount, it should not exceed that which would be incurred by a prudent person in the conduct of a competitive business.
Reasonableness takes into account the context of a given source selection, including current market conditions, general economic conditions, and other factors that affect the ability of a Quoter to perform the contract requirements. Normally, adequate price competition establishes a fair and reasonable price.
Completeness and Accuracy. The Government will review the pricing tables for completeness and accuracy. A determination will be made as to whether the Quoter properly completed the pricing tables. The Quoter’s quote will be checked for mathematical correctness to include the following:
1) Checking arithmetic in all computations; and
2) Ensuring all prices are summarized correctly

52.204-24 REPRESENTATION REGARDING CERTAIN TELECOMMUNICATIONS AND
VIDEO SURVEILLANCE SERVICES OR EQUIPMENT (OCT 2020)
The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it "does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument" in paragraph (c)(1) in the provision at 52.204-26, Covered Telecommunications
Equipment or Services--Representation, or in paragraph (v)(2)(i) of the provision at 52.212-3, Offeror
Representations and Certifications-Commercial Items. The Offeror shall not complete the representation in paragraph (d)(2) of this provision if the Offeror has represented that it "does not use covered telecommunications equipment or services, or any equipment, system, or service that uses covered telecommunications equipment or services" in paragraph (c)(2) of the provision at 52.204-26, or in paragraph (v)(2)(ii) of the provision at 52.212-3.
(a) Definitions. As used in this provision-
Backhaul, covered telecommunications equipment or services, critical technology, interconnection arrangements, reasonable inquiry, roaming, and substantial or essential component have the meanings provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video
Surveillance Services or Equipment.
(b) Prohibition.
(1) Section 889(a)(1)(A) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019
(Pub. L. 115-232) prohibits the head of an executive agency on or after August 13, 2019, from procuring or obtaining, or extending or renewing a contract to procure or obtain, any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. Nothing in the prohibition shall be construed to--
(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or
(ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles.
(2) Section 889(a)(1)(B) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019
(Pub. L. 115-232) prohibits the head of an executive agency on or after August 13, 2020, from entering into a contract or extending or renewing a contract with an entity that uses any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. This prohibition applies to the use of covered telecommunications equipment or services, regardless of whether that use is in performance of work under a
Federal contract. Nothing in the prohibition shall be construed to--
(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or
(ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles.
(c) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management
(SAM) (https://www.sam.gov) for entities excluded from receiving federal awards for "covered telecommunications equipment or services."
(d) Representations. The Offeror represents that--
(1) It [] will, [] will not provide covered telecommunications equipment or services to the Government in the performance of any contract, subcontract or other contractual instrument resulting from this solicitation.
The Offeror shall provide the additional disclosure information required at paragraph (e)(1) of this section if the Offeror responds "will" in paragraph (d)(1) of this section; and
(2) After conducting a reasonable inquiry, for purposes of this representation, the Offeror represents that--
It [] does, [] does not use covered telecommunications equipment or services, or use any equipment, system, or service that uses covered telecommunications equipment or services. The Offeror shall provide the additional disclosure information required at paragraph (e)(2) of this section if the Offeror responds "does" in paragraph (d)(2) of this section.
(e) Disclosures.
(1) Disclosure for the representation in paragraph (d)(1) of this provision. If the Offeror has responded
"will" in the representation in paragraph (d)(1) of this provision, the Offeror shall provide the following information as part of the offer:
(i) For covered equipment--
(A) The entity that produced the covered telecommunications equipment (include entity name, unique entity identifier, CAGE code, and whether the entity was the original equipment manufacturer (OEM) or a distributor, if known);
(B) A description of all covered telecommunications equipment offered (include brand; model number, such as OEM number, manufacturer part number, or wholesaler number; and item description, as applicable);
and
(C) Explanation of the proposed use of covered telecommunications equipment and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b)(1) of this provision.
(ii) For covered services--
(A) If the service is related to item maintenance: A description of all covered telecommunications services offered (include on the item being maintained: Brand; model number, such as OEM number, manufacturer part number, or wholesaler number; and item description, as applicable); or
(B) If not associated with maintenance, the Product Service Code (PSC) of the service being provided; and explanation of the proposed use of covered telecommunications services and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b)(1) of this provision.
(2) Disclosure for the representation in paragraph (d)(2) of this provision. If the Offeror has responded
"does" in the representation in paragraph (d)(2) of this provision, the Offeror shall provide the following information as part of the offer:
(i) For covered equipment--
