REQUEST FOR QUOTE N6227119Q1073

This COMBINED SYNOPSIS/SOLICITATION is prepared in accordance with the format in FAR 13 and supplemented with additional information included in this notice. This RFQ intends to result in a Firm Fixed Price (FFP) Purchase Order.

The award will be evaluated on a technically-acceptable-lowest-price basis.

This is a Brand Name requirement to Institut National D’Optique for a THz illumination source. Third part items will not be considered for award. The NAICS associated with this purchase is 334519. Preferred method of payment is Wide Area

Work Flow (WAWF).

While discussions are not anticipated, the Government reserves the right to do so.

EVALUATION:

Procedures in FAR 13.106 are applicable to this procurement. While price will be a significant factor in the evaluation of offers, the final contract award will be based on a combination of factors. A combination of price and Minimal Technical

Requirements/Salient Characteristics as in the above Statement of Requirements, delivery date, a determination of responsibility, on a technically acceptable lowest price basis.

System for Award Management (SAM). Quoters must be registered in the SAM database to be considered for award.

Registration is free and can be completed on-line at http://www.sam.gov/.

INSTRUCTIONS FOR SUBMISSION OF QUOTATION:

The technical quote shall be submitted no later than 11:30 AM PST on 04/11/2019 and shall be valid for 60 days.

All vendor questions shall be submitted via email no later than 1:00PM PST on 4/16/2019.

Contractor’s proposal shall be valid for sixty (60) days and include Volume 1, and 2. Each document should contain the following language on every page: SOURCE SELECTION SENSITIVE - SEE FAR PART 2.101 AND 3.104, preferably submitted combined in one PDF file, not to exceed 20 pages and 2MB).

Volume I - Technical Submission

Volume 1 shall include the offeror’s name, CAGE code, DUNS number and business size

Technical proposal to include delivery time and product specifications in accordance with the minimum technical requirements specified in the Statement of Requirements

Submitted in MS Word and / or Excel or PDF format. No more than 20 pages total.

Each page labeled with Contractor’s name and pages numbered in the following format: page ___ of ___.

Volume II – Price Submission - vendors shall provide a proposed schedule of supplies with associated prices at a firm-fixed price in accordance with the CLIN table provided below. Detailed price quote shall break out the cost of all equipment and all shipping fees (if not included).

Contractors are encouraged to include any Federal and/or Educational discounts.

Include all warranty information, terms and conditions and anticipated lead time and delivery date. Quoted items must include all power sources and note items needed to operate the equipment.

1.0 Background/Introduction:

The Department of Physics needs a terahertz (THz) illumination source capable to deliver a near-flat-top rectangular illumination. This specific THz illumination source is required to fulfill the aims of several DoD-Space sponsored projects on

THz sensors development for space applications, currently active at NPS. The THz sensors being developed in the

Department of Physics will be integrated in small form factor spacecrafts by the Space Systems Academic Group. The illumination source is required for materials and sensor characterization, qualification tests and to complete a THz real-time imaging system.

2.0 Minimal Technical Requirements/Salient Characteristics:

The minimum requirements are:

Center frequency: 515 GHz

Illumination surface: Greater than 4x4 inches

Output power: Greater than 1 mW

Dimensions: Smaller than 18x18x18 inches (to fit in an existing vacuum chamber)

Power supply: 110 V AC

The equipment must not require any external signal feeding.

The equipment must be a turn-key (connect to power supply and operate).

3.0 Safety and Environmental:

Contractors shall comply with all applicable Federal, State, and local occupational, safety, and health (OSH) and environmental regulations and requirements. Contractor is fully responsible for the safety and health of their employees, and furnishing of any equipment, training, medical surveillance, or other safety requirements, including personal protective equipment.

Contractors shall complete any site specific safety and environmental training prior to starting the relevant contract tasks.

The Contractor shall provide and maintain work environments and procedures, which will safeguard the public and

Government personnel, property, materials, supplies, and equipment exposed to Contractor operations and activities; avoid interruptions of Government operations and delays in project completion dates; and control costs in the performance of this contract.

Hazardous Materials:

Contractors bringing hazardous materials onsite shall comply with FAR Sections 23.3 and 52.223-3: Hazardous Material

Identification and Material Safety Data, and ensure no Contactor hazardous materials remain onsite after work is completed.

Accident Reporting:

For recordable injuries and illnesses, and property damage accidents resulting in at least $2,000 in damages, the Prime

Contractor shall conduct an accident investigation to establish the root causes of the accident and provide a completed Navy

Contractor Significant Incident Report (CSIR) form within 5 calendar days of the accident. The Contracting Officer will provide any required or special forms.
