STATEMENT OF WORK (SOW)

Ship/Activity: SSN 741 Title: Rental Forklift Place of Performance: Puget Sound Naval Shipyard and Intermediate Maintenance Facility, Bremerton, WA Period of Performance: 4/9/2018 – 10/9/2018

1. BACKGROUND.

In support of material handling operations, Puget Sound Naval Shipyard and Intermediate Maintenance Facility (PSNS & IMF) requires 6,000 lbs. capacity forklifts for the movement of materials throughout the shipyard, such as through buildings, along piers and down in dry docks.

Delivery Location is: Delivery location will be Naval Base Kitsap Bangor, Service Pier building 7114.

2. REFERENCES.

a. 40 CFR 63.6625(i) or (j)

b. 29 CFR, Part 1926, Subpart H, Materials Handling, Storage, Use, and Disposal

c. Industrial Truck Standard Development Foundation, ITSDF B56 standard

3. SALIENT CHARACTERISTICS

The Contractor shall weigh vehicle(s) prior to delivery and mark (4 inch high black lettering) with the individual weight in pounds on each side of vehicle(s). OEM weight markings are acceptable

a. The contractor shall provide (6 each) - 3-ton forklifts (herein referred to as vehicle), equipped as follows:

· Rated Capacity: 6,000 lbs. (minimum)

· Load Center: 24 inches

· Transmission: Automatic

· Tires: Solid or Foam filled

· Mast Type: Nested channels to provide maximum visibility of the attachment and load 2.

· Mast Extended Height: 175” (minimum)

· Lowered Mast Height: 90” (maximum)

· Turning Radius: 90” to outside edge (maximum)

· Overall Width: 52” (maximum)

· Fork Length: 48” pallet type

· Forks shall have side-shift capability

· Fork Thickness: 2”

· Fork Spread: 44” maximum (to outside edge)

· Fuel: Diesel

· Parking Brake: Hand / foot operated

· Lights: Brake, front and rear drive lights, work-lights

· Dash Instruments: Battery discharge, oil pressure, fuel gauge, water temperature, and engine hour meter.

· Lifting Attachment Points: See section d

b. The contractor shall provide 2 each – 7-ton forklifts (herein referred to as vehicle):

· Rated Capacity: 6,000 lbs. (minimum)

· Load Center: 24 inches

· Transmission: Automatic

· Tires: Solid or Foam filled

· Mast Type: Nested channels to provide maximum visibility of the attachment and load 2.

· Mast Extended Height: 260” (minimum)

· Lowered Mast Height: 148” (maximum)

· Turning Radius: 135” to outside edge (maximum)

· Overall Width: 82” (maximum)

· Overall Length: 145” (maximum)

· Fork Length: 96” pallet type

· Forks shall have side-shift capability

· Fork Width: 6” (maximum)

· Fork Thickness: 2.5”

· Fork Spread: 73.9” maximum (to outside edge)

· Fuel: Diesel

· Parking Brake: Hand / foot operated

· Lights: Brake, front and rear drive lights, work-lights

· Dash Instruments: Battery discharge, oil pressure, fuel gauge, water temperature, and engine hour meter.

c. The contractor shall provide 1 each - 10-ton forklift (herein referred to as vehicle) equipped as follows:

· Rated Capacity: 20,000 lbs. (minimum)

· Load Center: 24 inch (minimum)

· Transmission: Automatic

· Tires: Solid (foam filled)

· Mast Type: Nested channels with a minimum 28” of viewing space between mast towers with minimal obstructions (i.e. chains or flex hoses)

· Mast Extended Height: 281” (minimum)

· Mast Lowered Height: 176” (maximum)

· Turning Radius: 162" to outside edge (maximum)

· Overall Width: 96” (maximum) From outside tire to outside tire

· Length to Load Face: 178” (maximum)

· Fork Length: 96” pallet type

· Forks shall have side-shift capability

· Fork Spacing: 3” minimum spread (to inside edge), 89.4” maximum spread (to outside edge)

· Fuel: Diesel

· Parking Brake: Hand or foot operated

· Lights: Brake, front and rear drive lights, work-lights.

· Dash Instruments: Battery discharge, oil pressure, fuel gauge, water temperature, and engine hour meter.

· Operator Cab: Forklift shall be supplied with a fully enclosed cab equipped with two doors, heater, defroster, fan and wipers.

· Lifting attachment points: See section d

d. The contractor shall provide 1 each - 17-ton forklift (herein referred to as vehicle) equipped as follows:

· Rated Capacity: 34,000 lbs. (minimum)

· Load Center: 48 inch

· Transmission: Automatic

· Tires: Solid (foam filled)

· Mast Type: Nested channels to provide maximum visibility of the attachment and load. 29” minimum viewing space between mast towers with minimal obstructions (i.e. chains or flex hoses).

· Mast Extended Height: 222” (minimum)

· Mast Lowered Height: 145” (maximum)

· Turning Radius: 205" to outside edge (maximum)

· Overall Width: 100” (maximum)

· Length to Load Face: 214” (maximum)

· Fork Length: 96” pallet type

· Forks shall have side-shift capability

· Fork Spacing: 9” minimum spread (inside to inside edge), 94” maximum spread (outside to outside)

· Fuel: Diesel

· Parking Brake: Hand or foot operated

· Lights: Brake, front and rear drive lights, work-lights.

· Dash Instruments: Battery discharge, oil pressure, fuel gauge, water temperature, and engine hour meter.

· Operator Cab: Forklift shall be supplied with a fully enclosed cab equipped with two doors, heater, defroster, fan and wipers.

· Lifting Attachment Points: See section d

d. Lifting attachment points shall meet the following requirements:

· Lifting attachment points shall have an identified Working Load Limit (WLL) marked at each point. Alternatively, the lifting attachment point’s WLL shown in operations manual, or clearly identified in writing on OEM letter head, would be acceptable.

· All lifting attachment points shall be clearly above the vertical center of gravity and symmetric about the vehicle(s) fore and aft centerline.

· Lifting attachment points shall accept common rigging shackles.

· Lifting gear shall only contact the vehicle at the lifting points. It is unacceptable to remove vehicle components to accommodate lifting gear.

· Use of spreader beams, other special lifting gear or unusually complex lifting arrangements are not allowed unless evaluated and approved by PSNS & IMF Lifting & Handling Department (Department 700 Code 740).

· Vehicle component assembly lift points, used during equipment fabrication, shall not be used for lifting the entire assembled vehicle unless specifically identified in the OEM operations manual.

· Vehicle tow point, hitch, or draw bar shall not be used as a lifting attachment.

· Ensure retrofitted, repaired, or modified lifting accessories on provided vehicle(s) comply with reference 29 CFR, Part 1926, Subpart H, Materials Handling, Storage, Use, and Disposal. Additionally, size attachment points based on either pair of diagonally opposed legs of a four-point lifting assembly carrying the entire load for worst-case scenario of load distribution with respect to center of gravity.

· Size lifting attachments for three-point lifts such that any two points carry the entire load based on worst-case scenario load distribution with respect to center of gravity.

· Ensure the working load limit of each lifting attachment point is not exceeded by sling angle tension when lifted. Minimum sling angle with respect to horizontal shall be 60°.

· Orientation of lifting attachment points shall be such that when loaded, the direction of force applied to each sling shall be aligned with the plane of the padeye or lug within ±5°.

f. Inspect and provide each vehicle in a safe and operational condition, free of rust, cosmetic defects, or damage.

· Provide vehicle(s) that meets, or exceeds, the safety requirements of Industrial Truck Standard Development Foundation, ITSDF B56 standard related to the elements of design, operation, and maintenance, the standardization relating to principal dimensions to facilitate interchangeability, and establishment of test methods and test procedures, for powered and non-powered industrial trucks.

· Accomplish the following prior to delivery and provide written certification of accomplishment upon delivery:

· Provide new condition maintenance free battery.

· Inspect all hoses and belts for cracks, wear, etc. and replace items that suspect to failure.

· Replace the engine oil with new or perform an engine oil analysis per 40 CFR 63.6625(i) or (j) on existing engine oil to validate effectiveness.

· Operationally test the vehicle(s) to include all manufacturer safety features and performance criteria.

· Ensure vehicle(s) is/are crane ready according to paragraph 3.3.

· Inspect the vehicle(s) provided, at the time of delivery and before acceptance, with the Contracting Officer’s Representative (COR) or other authorized government representative.

· Document in writing the condition of the vehicle to ensure it meets contract requirements. Provide a signed copy of the inspection document to the government representative.

· Correct any discrepancies noted or replace the delivered vehicle(s) with a compliant unit.

· Inspect the vehicle(s), upon completion of the period of performance and at the time of pick-up, or prior to the return of the vehicle(s) to the contractor for maintenance, repair or replacement during the period of performance, with the authorized Government representative and document any new conditions or damage. Provide a signed copy of the inspection document to the government representative.

g. Provide a copy of the manufacturer Operation and Maintenance manual(s), containing detailed operating instructions specific for the vehicle provided to the Contracting Officer’s Representative or designated Government employee.

h. Perform any required scheduled maintenance, in accordance with the manufacturer’s maintenance standards, on vehicles provided during the period of performance.

· Provide the planned scheduled maintenance schedule, with the contractor’s proposal, and notify the Contracting Officer’s Representative four days in advance of performing maintenance on the vehicle(s), to permit coordination with project workload.

· Provide a replacement vehicle, within 24 hours, for any unscheduled repairs required.

· Perform lockout/tag-out of vehicle(s) prior to performing maintenance. Notify the Contracting Officer’s Representative if vehicle(s) maintenance must be performed in a condition not requiring lockout or tag-out conditions.

i. Replace inoperative or unsafe vehicles, upon notification by authorized Government representative, within 24 hours if repairs cannot be accomplished to return the vehicle to full operating status within 8 hours of a service call.

· Supply a manufacturer’s technical representative or ASME certified mechanic to respond on-site within 8 hours of Government notification of a problem on a 24hrs/day, including weekends and holidays.

· Provide the Contracting Officer’s Representative the name, telephone number/pager and/or other contact information to coordinate contact with the contractor’s representative for repair of defective or inoperative equipment.

j. Coordinate scheduled pick-up of the vehicle(s), at the end of the period of performance, with the Contracting Officer’s Representative. If the contractor is unable to accomplish pick-up on this date, immediately notify the Contracting Officer’s Representative and provide an alternate date and time of pick-up that is mutually agreeable.

4. DELIVERABLE

a. Prior to contract approval, provide the vehicle(s) make/model along with OEM lifting point identification per section d outlining lifting points to PSNS & IMF Lifting and Handling Department (Department 700 Code 740).

b. Certification that all pre-delivery checks of section 3.6 were performed and all unsatisfactory items were corrected.

c. Emergency point of contact to coordinate repairs as described in above paragraph 3.11.1 .

5. GOVERNMENT-FURNISHED ITEMS – PROPERTY, MATERIAL, EQUIPMENT, SERVICES

a. The government will provide crane lifting and handling services as required for the vehicle(s).

b. The government will provide lifting and handling forklift attachments detailed below:

· NAVAIR Swab and Scrapper Assembly - towing capacity 500 lbs.

· NAVAIR Catapult Cylinder Strong-backs and Lifters - 5,000 lbs. capacity at 24” load center.

· Morse Model 285XGR Single Drum Barrel Lifter - 2,500 lbs. capacity.

· Lift-O-Matic Dual Drum Lifter - 2,000 lbs. capacity

· Hercules 5410 Barrel Lifter - 1,500 lbs. capacity

· Wesco Single Poly Drum Lifter - 1,000 lbs. capacity

· Wesco Single Drum Lifter - 1,000 lbs. and 1,500 lbs. capacity

· Caldwell Single Hook Beam 10S-2-20 - 4,000 lbs. capacity

· Caldwell Single Hook Beam 10S-5-24 - 10,000 lbs. maximum capacity (Not be used in excess of 6,000 lbs. at 24” load center).

· Caldwell Single Hook Beam 10S-7.5-36 - 15,000 lbs. capacity (Not be used in excess of 6,000 lbs. at a 24” load center)

· Forklift Boom Extension - maximum capacity 4,000 lbs. at 36” load center (Load center is based on the distance to the center-of-gravity of the load from the backrest of the forklift forks).

· OWTS Small Roto-bins - maximum capacity 1,300 lbs.

· Two Cubic Yard Roto-bins - maximum capacity 2,400 lbs.

· Additional forklift attachments may be provided by the government with approval from the Puget Sound Naval Shipyard and Intermediate Maintenance Facility Rigging Engineering Branch (PSNS&IMF, Department 700, and Code 715).

c. The government will provide all required employees for vehicle operation.

d. The government will provide all fuel and personnel/equipment vehicle refueling.

e. The government will perform a pre- and post-operational safety inspection of the vehicle(s) as part of daily use. If the vehicle is defective, needs repair or maintenance or found to be otherwise unsafe it will be removed from service and the contractor notified.

f. If the vehicle is involved in an accident or mishap it will be removed from service and the contractor will be notified. Contractor will inspect and provide a condition found report to the Contracting Officer for any damage as a result of accident.

6. QUALITY ASSURANCE.

The Government’s quality assurance will be based on the following:

Government will assess compliance through coordination between the Contracting Officer

7. SECURITY REQUIREMENTS.

Contractor representative(s) will require base access to the Puget Sound Naval Shipyard Controlled Industrial Area (PSNS CIA). Vendor shall notify the Contracting Officer, para. 9.1, Ten days prior to arriving at Puget Sound Naval Shipyard and Intermediate Maintenance Facility, Bremerton, WA.

9. ENVIRONMENTAL AND SAFETY.

Comply with all federal, state and local laws, regulations and best practices in relation to establishing and maintaining a compliant safety program.

10. ADDITIONAL CONSIDERATIONS.

Government Supported Work Hours: All deliveries will be scheduled for accomplishment between the hours of 8:00AM and 3:00PM, Monday through Friday unless one of these days is a federal holiday.

All technical clarifications/deviations will be coordinated through the Contracting Officer.

The Contractor shall inform the Contracting Officer of the specific time and date of forklift delivery.
