STATEMENT OF WORK

1. SCOPE:

1.1 This Statement of Work (SOW) covers troubleshooting and repair of the Monarch Lathe 20-C Alpha ACV.

2. BACKGROUND:

2.1 Location: Building 240 at Portsmouth Naval Shipyard in Kittery, ME.

2.2 Shipyard Point of Contact: The point of contact for this work will be Monique Cinfo, 207-438-2873.

3. REQUIREMENTS:

3.1 General: Unless indicated otherwise in this SOW, the Contractor shall provide all equipment, tools, materials/parts, and other items/services necessary to perform the work specified herein. Used herein, “as needed” defines machine conditions that do not meet OEM specifications. Upon completion, the lathe shall meet or exceed original design characteristics. The work will take place 0700-1530 Monday through Friday.

3.1.1 The contractor shall rebuild the lathe.

3.1.2 The Contractor shall provide replacement parts required to repair the unit. In the event the repair costs exceed agreed upon cost the contractor shall notify Portsmouth Naval Shipyard for resolution but the contractor is not allowed to exceed that figure without written approval.

3.1.3 A full operational test shall be performed by the contractor and viewed by a technical point of contact and other Portsmouth Naval Shipyard personnel as required.

4. QUALITY ASSURANCE PROVISIONS:

4.1 Personnel Safety and Health Requirements: All machine parts, components, mechanisms, and assemblies furnished on the unit shall comply with all specific requirements of “OSHA Safety and Health Standard (29CFR1910), General Industry”, that are applicable to the equipment itself. Covers, platforms, guard rails, belt guards, and safety devices shall be provided for all parts of the equipment that present a safety hazard. The safety devices shall not interfere with the operation or maintenance of the equipment. The safety devices shall be removable to facilitate inspection, maintenance, and repair of the part. Additional safety and health requirements shall be as specified in other paragraphs of this specification.

4.2 OSHA Approved: The equipment specified herein shall be in compliance with the applicable OSHA regulations and be listed by one of the OSHA accredited laboratories or approved in accordance with CFR Title 29, Chapter XVII, Part 1910 and installed in accordance with applicable NEC/NFPA requirements. Approval shall be as specified under the “Approval” and “Acceptance” criteria in the OSHA regulations Subpart “O”, Machinery and Machine Guarding para. 1910.212 and Subpart “S” Electrical, para. 1910.303 and para. 1910.399. After equipment delivery and installation and prior to testing, the Contractor shall provide a signed written Manufactures Certification (paragraph 3.3.6.8) that equipment has been manufactured and installed to OSHA CFR 1910.399 (per definition of ‘acceptable’).

5. HAZARDOUS ENERGY CONTROL:

5.1 Contractors and non-PNS government agencies (NPGAs) are hereby informed that their employees or representatives present on property under Portsmouth Naval Shipyard cognizance must adhere to Federal OSHA requirements for hazardous energy control IAW29 CFR 1915.89 and must follow Hazardous Energy Control (HEC) procedures as identified in NAVSEA’s Occupational Safety, Health, and Environment Corporate Manual (OSHECM) Chapter 250 (latest revision), Hazardous Energy Control (HEC) when performing work under PNS contract. All HEC work to be performed by Contractors and NPGAs requires that PNS Technical Points of Contact

6. SECURITY:

6.1 The lthe is within the Controlled Industrial Area (CIA) of the Portsmouth Naval Shipyard. No person not known to be an American citizen of good standing and repute shall be eligible for access to Portsmouth Naval Shipyard. Proof of citizenship may be required.

By the contractor’s representative’s signature below the contractor’s representative certifies that the Company whose name is listed above understands and agrees to the following requirements for any task or work the contractor performs aboard the Portsmouth Naval Shipyard (NAVSHIPYD PTSMH) that involves Hazardous Energy Control (HEC).

29 CFR Part 1915.89, which became effective and enforceable on 31 October 2011, provided a new standard addressing hazardous energy control in shipyard employment. Part 1915.89 applies to private-sector contractors performing servicing operations in shipyard employment, and states the Navy’s HEC Program is in effect in Naval Shipyards. Private-sector contractors performing work that involves HEC aboard NAVSHIPYD PTSMH are responsible for ensuring the safety of their employees who may perform tasks or work involving HEC.

The contractor’s representative’s signature below certifies that the contractor understands and shall comply with all requirements for work involving HEC as provided in 29 CFR Part 1915.89 and the Naval Sea Systems Command (NAVSEA) Occupational Safety, Health and Environmental Corporate Manual (OSHECM) Chapter 250 (latest revision), including Lockout/Tags-Plus (LOTP). WARNING: CONTRACTOR’S FAILURE TO COMPLY WITH THE HEC REQUIREMENTS REFLECTED IN 29 CFR PART 1915.89 AND OSHECM CHAPTER 250 MAY CAUSE SERIOUS INJURY OR DEATH TO THE CONTRACTOR’S EMPLOYEES. UNDER NO CIRCUMSTANCES SHALL THE PORTSMOUTH NAVAL SHIPYARD ASSUME ANY LIABILITIES THAT RIGHTLY ARE THE RESPONSIBILITIES OF THE CONTRACTOR.
