Unity Essential Support Plans for Code 134.4's ISQ LT Single Quadrupole GC-MS (S/N: ISQ1609537) and Trace 1310 (S/N: 715002229), Statement of Work (SOW)
1. OBJECTIVE: This Statement of Work (SOW) covers the scheduled maintenance/inspections and unplanned parts/repairs for Code 134.4's ISQ LT Single Quadrupole GC-MS and Trace 1310 per Quote#: 21108049 I -3.
2. SCOPE OF WORK: Provide multi-year, sole source, Unity Essential Support Plans for the preventative maintenance and service needed to maintain Code 134.4's ISQ LT Single Quadrupole GC-MS (S/N: ISQ1609537) and Trace 131 O (S/N: 715002229).
3. BACKGROUND:
3.1. Current Condition. All equipment is currently fully functional with no known major defects identified.
3.1. Task Statement. Service is for an extended period to cover a total of five (5) years for each instrument. Unplanned parts and labor shall also be covered under this contract.
4. PLACE OF PERFORMANCE: The work will take place in the Materials Test Lab in Bldg. 20, located at Portsmouth Naval Shipyard in Kittery. The work will take place 0600-1530 Monday through Friday.
5. PERIOD OF PERFORMANCE: The period of this service contract shall be for one ( 1) base year starting with the date of award of the contract with four ( 4) option years. Contract must be awarded by the dates specified in the table below:

6. REQUIREMENTS:
6.1. General. Unless indicated otherwise in this SOW, the Contractor shall provide all personnel, equipment, tools, materials/parts, per diem/transportation, and other items/services necessary to perform scheduled inspection/.maintenance, and unplanned repairs as described herein. The Government shall furnish all manuals and technical data to the Contractor if available.

6.2. Scheduled Inspection/Maintenance Calls. The Contractor shall provide yearly recommended preventative maintenance required for the equipment by the original equipment manufacturer (OEM) per their standard checklist and provide a copy of the completed checklist to the government.
6.3. Planned Preventative Maintenance. The Contractor shall be responsible for furnishing all repair/replacement parts (required due to normal wear and tear such as pads, filters, fluids/oils, etc.). The Contractor is also responsible for the disposal of replaced and expendable items at no additional charge to the Government. If the Contractor finds something wrong with the unit during the scheduled maintenance/inspection that requires additional work, the Contractor shall follow the procedures explained in paragraph 6.4. Unplanned Parts/Repairs.
OEM parts are required during maintenance. The Contractor may substitute functionally acceptable equal parts, when OEM parts are unavailable, only after approval from the technical points of contact. The price for Unity Ess~ntial Support Plans is a firm fixed price. ·
6.4. Unplanned Parts/Repairs. The Contractor shall identify any additional parts/repairs needed to bring a unit up to standards found during the performance of a scheduled inspection/ maintenance call to the Contracting Officer (CO), and the Primary & Secondary Points of Contact (POC) for that instrument for approval before proceeding. If these parts/repairs are not covered under any of the scheduled inspection/maintenance, the Contractor shall provide the CO an unplanned parts/repairs report. The Contractor shall proceed with the Unplanned Parts/Repairs only after approval from the Contracting Officer.
7. PERSONNEL SKILLS QUALIFICATION: Service technician shall be knowledgeable in design, operation and safety features of the ISQ LT Single Quadrupole GC-MS an authorized field service representative for the OEM, Thermo Electron North America LLC.
8. SCHEDULE REQUIREMENTS: Within 14 days upon award of a contract per this Statement of Work, the contractor shall establish a work performance schedule that is acceptable to the Government. Contractor shall contact the Primary and Secondary POCs, via email, to schedule any on-site preventative maintenance visits.

9. SAFETY REQUIREMENTS: Work Site Operational Safety Environment - Contractors and non- PNS Government Agencies (NPGAs) shall be responsible to read and understand the requirements of OSHECM Chapter 250 and can see an electronic copy of OSHECM Chapter 250 (latest revision) by using the following web site:

The Contractor shall provide and require their personnel to wear personal protection equipment (PPE) (e.g., hearing protection, eye protection, gloves, steel-toed shoes or boots, aprons, masks, face shields, reflective vests, safety harnesses for fall protection in accordance with OSHA CFR 1926 (construction) and CFR 1915 (shipyard) standards. The contractor is required to follow all appropriate lockouUtag out requirements as specified by OSHECM Chapter 250 and CFR 1915.89. Contractor shall conform to all applicable safety and occupational health standards published in the Federal Register or any other Occupational Safety and Health Administration (OSHA) Public Laws and ensure employees observe safety, traffic control, and fire prevention requirements in effect at the site.
Hazwdous Energy Control Contractor Compliance Certification Document, Attachment 3 of this contract must be signed and returned to the Government prior to working on the shipyard
10. SECURITY REQUIREMENTS: All personnel assigned by the contractor to perform work or site visits must be US citizens. Contractor will be required to submit a visit request listing information of personnel needing access aboard Portsmouth Naval Shipyard. Visit request must include the full name of the persons, nationality, birth date and Social Security Number. Visit request may be in the form of a letter on company stationary with signature from company security manager or authorized company representative and faxed to the Pass and ID office at (207) 438-2669. Company visit requests shall be submitted annually and as often as necessary to update employees of the company who need continued access to the Shipyard. Criminal history and/or falsification of information provided by an employee of the company may be subject to denial for unescorted access up to and including general access to the Shipyard. All contractor vehicles are subject to inspection prior to allowing entry onto this installation.
