PERFORMANCE WORK STATEMENT

USNS CARSON CITY CROATIA

1. The contractor is responsible for providing HOTEL ROOMS, equivalent of AAA 3 Diamond rated facility or higher, as identified below for ship crew personnel. The following STAR ratings - eeee (4 Star), eeeef (4 ½ Star), and eeeee (5 Star), are acceptable ratings for Croatia location. Please refer to http://www.aaa.com/diamonds/diamond-awards

At any time 24 hours per day there shall be at least one restaurant within four blocks of the hotel facility that is open for meals. The hotel facility shall have security policies that require guests to verify their identity before being granted access to rooms and shall require that visitors to guests' rooms surrender an ID at the front desk and be escorted for room access. Each room shall be single occupancy, of western style, and have at least 250 sq. ft. of living space. Each room shall be equipped with a TV, phone, refrigerator, in-room safe, and working smoke detectors. Multiple (more than one (1)) microwaves must be provided in a central location for crew usage. Each room must have an adequate ventilation system, with air conditioning and heating. The hotel facility shall have a full service restaurant serving western style food three meals daily. The restaurant shall be open by 6:00 am each morning so the crew may eat breakfast. The hotel facility shall have ice machines and beverage machines available for guest usage.

2. Onsite functional washers and dryers (two (2) or more of each) shall be available for use. Facility shall be within a 20 mile radius of Viktor Lenac, Croatia shipyard. Hotel internet access shall be available, or crew shall have access to an on-site Business Center. Hotel shall provide free local telephone calls. Hotel rooms shall include daily cleaning, changing of linen, and replenishment of toiletries. Facility shall not be under construction or major renovation during the performance period.

3. The period of performance will commence on 1 April 2018 and end on/about 12 May 2018. The actual number of days/rooms used may be more or less than those amounts in this initial requirement.

4. Prior to the ship crews’ arrival, the Government will provide the hotel facility a list of crewmembers names and check-in/check-out dates. The hotel will only check into a room those personnel whose name is on the list provided. The Government will promptly provide updated name list to the hotel during performance. Prior to checking in unlisted crewmembers, the hotel shall immediately verify with the Government. The Government will not be obligated for any room charge for crewmembers not on the list provided.

5. All rooms should not be adjacent to any entertainment establishments e.g. Restaurant, Bar, Pub, Discotheque and Casinos.

6. Rooms for Master and Chief Engineer – the rooms shall be located at different wing or floors from the ship crew.

7. The Government is not liable for charges such as restaurant, phone, Internet access, mini-bar, or room damages, etc. These charges, if incurred, are the responsibility of each crew member. In no case will the Government pay for any of these charges. It is the Hotel’s responsibility to arrange for payment of these items by the crew member.

8. Hotel must meet local Force Protection requirements. Hotel shall have security cameras, if available. (See HEIGHTENED VIGILANCE information below)

TWENTY-TWO (22) HOTEL ROOMS FROM 1 APRIL 2018 – 11 MAY 2018, WITH CHECK OUT 12 MAY 2018 (41 NIGHTS/902 ROOMS) TWO (2) ADDITIONAL HOTEL ROOMS FROM 1-2 APR 2018 WITH CHECKOUT 3 APRIL 2018 (2 NIGHTS/4 ROOMS)

THREE (3) ADDITIONAL HOTEL ROOM FROM 9 11MAY 2018, WITH CHECKOUT 12 MAY 2018 (3 NIGHTS/9 ROOMS)

TOTAL ROOMS: 915

WITH THREE (3) 10 DAY OPTION PERIOD EXTENSIONS

OPTION 1: TWENTY-FIVE (25) HOTEL ROOMS FROM 12 MAY 2018 – 21 MAY 2018, WITH CHECK OUT 22 MAY 2018 (10 NIGHTS/250 ROOMS)

NOTE:

1) Ship's Master (or his designated representative) shall conduct an on-site inspection of proposed HOTEL ROOMS PRIOR to acceptance.

Heightened Vigilance Contractor has an obligation to be alert at all times in order to contribute to the protection of the ship and its crew. Contractor shall immediately report to the ship's command duty officer, or nearest available Navy official or Host Nation (HN) representative, any signs of suspicious activity in or around the site of performance. Suspicious activities include, but are not limited to, unauthorized personnel or vehicles in the vicinity of the site of performance, unusual equipment or movement of persons or supplies, and any other activity that may be perceived as a threat to United States forces. Contractor also has an obligation to report any such activity perceived outside the site of performance, if such activity could reasonably have an impact on the site of performance or could be perceived as a threat to United States forces.

Contractor will fully partner with U.S. Navy representatives in all matters that could impact the effectiveness of Force Protection services that are provided for or arranged under this contract to protect U.S. Navy Ships, Units and Personnel. Upon request the Contractor will identify to Navy Force Protection Personnel and NCIS representatives the host country Law Enforcement, Military, Security counterparts, or other persons utilized by the contractor for the purpose of providing Force Protection or other services to U.S. Navy Ships and forces. Contractor will arrange introductions to these host counterparts if requested and make available for review any agreements pertaining to Force Protection arrangements or other services with the Host counterparts.
