USS CHUNG-HOON (DDG 93)
HABITABILITY STATEMENT OF WORK
1. INTRODUCTION
1.1 USS CHUNG- HOON intends to purchase materials and services to upgrade the Wardroom, CO and XO’s Cabins, Disbursing Office and Mustang Alley Stateroom.
2. SCOPE
2.1 Requirement is being solicited with the intention of awarding a Firm Fixed Price (FFP) contract for the upgrade and renovation of the Wardroom, CO and XO Cabin, Disbursing Office and Mustang Stateroom of the USS CHUNG HOON using Military and Federal Specifications materials. USS CHUNG HOON requires the contractor to provide a FFP quote for the labor, materials, delivery, and installation per the tasks specified under section 3 “SPECIFIC TASKS”.
3. SPECIFIC TASKS
WARDROOM 02-126-4-L
3.1.1 Deck: Contractor Shall Remove and Dispose Existing in Wardroom: Install new Fire retardant deck flooring; glued down. Pattern: LT/UPOFLOOR XPRESSIONS Manufactured by Shannon Sales Inc.– American Cherry MIL SPEC ASTM E648 Class 1; ASTM E662 <450 - Area: 876 sq. ft. IAW MIL-PRF-32170; underlayment: IAW MIL-3135 Type III or IV.
3.1.2 Side Table: Contractor shall fabricate, deliver, and install new aluminum side table double shelves to accommodate printer; manufactured IAW MIL-PRF-32038, and shall be laminated with high-pressure plastic laminate, IAW MIL-P-17171, Type I. Size: 25” x 30”. Laminate pattern: Biltmore Cherry.
3.1.3 Install Wallcovering Vinyl: Contractor shall install approx. 400 SQFT of new vinyl 27
oz. wallpaper in the Wardroom. Include wall prep material per Fed. Spec: CCC-W-408D, Type 2. Pattern:
Morning Frost.
3.1.4 Bulkhead and Doors Laminate: Contractor shall install new high-pressure plastic laminate lower part of bulkhead, and doors; matching existing counter tops & bookshelf. Pattern - Biltmore Cherry, in accordance with mil spec, MIL-P-17171E, Type IV, Grade I. Size - 740 sq. ft. Wall prep includes removing lower part of the existing trim, removing existing storage doors on the bulkhead;
cover holes with flat sheet metal and paint all trims and cabinets.
3.1.5 Table Covers: Contractor shall fabricate and deliver one Navy blue artificial leather table cover in accordance with Commercial Item Description (CID) A-A-59517, Class 4, Condition A.
Size: 42" x 186" with gold imprint: 20" crest and corner imprint.
3.1.6 Chair Covers: Contractor shall fabricate and deliver 19 custom-made chair covers with 8" printed crest & personalization - CO, XO, EAGLE; Navy blue artificial leather with gold piping in accordance with commercial item description A-A-59517,Class 4, Condition A.
3.1.7 Paint: Paint all trims and cabinets with air-dry enamel clear-coat paint in accordance with MIL-DTL-24607.
3.1.8 Entertainment Center: Contractor shall fabricate custom fitted aluminum entertainment center IAW MIL-PRF 32038; fully laminated with high pressure plastic laminate per MIL- P- 17171E, Type I; with 2/ 20" x 42" side display case, 2 bottom storage/cabinets with shelf & polycarbonate doors 36x20 x 23" per CID-A-A-59502, Type II; 3 layer bookshelves/CD storage shelves 36x23x61 & custom fitted aluminum countertop w/ stainless steel metal capping (24x103x 1 3/8).
3.1.9 Fabricate and Install Corner Couch Extension and Repair & Reupholster Sofa’s:
The contractor shall fabricate and install a corner couch extension IAW MIL-PRF 32038 to match the existing couch approx. Condition A. Size (76 x 30 x 40 upholster with artificial leather like material and reinforces cushions with form (Neoprene foam shall be IAW MIL-R-20092, Type II, Class 5) per CID A-A-59517; and re-upholster existing couch with armrests to match fabricated piece, size: 96 x 30 x
40. Contractor shall refurbish to like new condition, repair any damaged sinuous springs, with additional nutserts as required. Also install a layer of vinyl over top edge of aluminum arms before foam. Vinyl covering shall be IAW A-A-59517. Contractor will provide the ship’s representative with artificial leather like material and vinyl colors and pattern, preferably dark brown Deco, to select from. Seat cushions shall be 6” thick minimum. Contractor to remove and reinstall furniture as required. Relocate sofas to new locations as directed by ships force.
3.1.10 Cold Table with Stand: The contractor shall fabricate and install cold table with stand custom fitted heavy gauge insulated stainless steel top with drain tube size 58.5" x 25.5" x 10" deep; fitted for a heavy gauge aluminum cabinet with stand IAW MIL-PRF 32038;fully laminated with high pressure plastic laminate IAW MIL-P- 17171E, Type I; size: 59" x 26" x 38" ht with sneeze guard fabricated in stainless steel per IAW MIL-PRF-32038 with clear polycarbonate top and shield IAW CID A-A-59502, Type II. size: 59"x26"x 21". The contractor to remove and dispose existing cold table and install new cold table as required.
3.1.11 Galley Door: The contractor shall cut out 20" x 20" and frame the hole on upper part of the existing galley door, connect both upper and lower doors, then laminate afterwards with high pressure plastic laminate IAW MIL-P- 17171E, Type I.
3.1.12 Wardroom Cabinet: The contractor shall remove and dispose of existing wardroom cabinet, keep counter top in place and cut all supports to level the deck.
3.1.13 Display Case: The contractor shall remove existing bookshelves and cover with .090 gauge flat aluminum. Remove doors and replace with polycarbonate doors 36x2 0x 23" per CID-A-A- 59502, Type II; from existing storage and convert as display case for miniature ship trophy.
3.1.14 Whiteboard Cabinet: The contractor shall fabricate custom fitted aluminum white board cabinet 54" x 42" with doors IAW MIL-PRF 32038; fully laminated with high pressure plastic laminate IAW MIL-P- 17171E, Type I.
3.1.15 Metal Trash Bins and Holders: The contractor shall fabricate custom fitted aluminum trash bins and holders with flap doors IAW MIL-PRF 32038; fully laminated with high pressure plastic laminate IAW MIL-P- 17171E, Type I; sizes 36" x 20" x 14", 36" x 20" x 11", 36"x20"x8"
Executive Officer Stateroom (01-158-4-L)
3.2.1 Remove and Dispose of Existing Carpet and Install 100% Wool Carpet: The contractor shall deliver and install a 100% wool carpet in the EXECUTIVE OFFICER's cabin; size:120
sq. ft. in accordance to Fed. Spec. DDD-C-95 Class 4; 44 oz woven. MIL- STD-1623 - fire retardant.
Removal, including removal disposal of existing carpet. replacement of base vinyl cove.
Commanding Officer Cabin (02-146-1-L)
3.3.1 Remove and Dispose Existing Carpet and Install 100% Wool Carpet: The contractor shall deliver and install a 100% wool carpet in the COMMANDING OFFICER's cabin; size: 312 sq. ft. in accordance to Fed. Spec. DDD-C-95 Class 4; 44oz woven. MIL-STD-1623 - fire retardant. Removal, including removal disposal of existing carpet. Replacement of base vinyl cove. The contractor will provide the ship’s representative samples of carpet, pattern and colors to select from.
3.3.2 Repair & Reupholster and Fabricate Corner Couch Extension: The contractor shall fabricate and install a corner couch extension IAW MIL-PRF 32038 to match the existing couch;
upholster with artificial leather and reinforce cushions with foam per CID A-A-59517, Class 4; Condition A. size: 88" x 109" x 30". Re-upholster existing couch with aluminum armrests to match fabricated piece.
Neoprene foam shall be IAW MIL-PRF-20092, Condition C The contractor shall refurbish to linew condition, repair any damaged sinuous springs, reinforce arms with additional nutserts as required. Also install a layer of artificial leather over top edge of aluminum arms before foam. Seat cushions shall be 6” thick minimum. Artificial leather covering shall be IAW A-A-59517. The contractor will provide the ship’s representative with foam and artificial leather material color to select from. The contractor to remove and reinstall furniture as required. Relocate couch/sofas to new location as directed by ship’s force.
Disbursing Office (1-84-1-Q)
3.4.1 Deck: The contractor shall prep and install LT/UPFLOOR XPRESSIONS manufactured by Shannon Sales Inc. deck tile (glued down) in DISBURSING OFFICER's Office Size: 120 sq ft.
Pattern: American Cherry MIL SPEC ASTM E648 Class 1; ASTM E 662<450 Area: 876 sq ft IAW MIL- PRF-32170; underlayment: IAW MIL-3135 Type III or IV.
3.4.2 Desk: The contractor prep and install new high pressure laminate for existing desk in accordance with mil spec, MIL-P-17171E, Type I and new stainless steel fitted metal caps with radius edges. Pattern: MAMBO; sizes: U-shape table
Mustang Alley Stateroom (01-314-1-L)
3.5.1 Deck: The contractor shall remove and dispose existing and deck prep in the MUSTANG ALLEY Stateroom. Install LT/UPFLOOR XPRESSIONS manufactured by Shannon Sales Inc. deck tile (glued down) Size: 237 sq ft. Pattern: American Cherry MIL SPEC ASTM E648 Class 1; ASTM E 662<450 IAW MIL-PRF-32170; underlayment: IAW MIL-3135, Type III or IV.
4. WORK LOCATION
4.0 Coordinate renovation and installation schedule with USS CHUNG HOON. The Technical Point of Contact (TPOC): Michael Grimes (CHUNG HOON'S SUPPO).
4.1 Work will be performed onboard USS CHUNG-HOON, Pearl Harbor, HI. Base Access shall be coordinated with USS CHUNG HOONG TPOC: LT Michael Grimes (CHUNG HOON'S
SUPPO).
5. WORK HOURS/SCHEDULE
5.1 Regular working hours shall consist of an 8-hour period, between 0700 am and 1800
Monday through Friday, excluding Government holiday(s). No regular work on Saturday(s) and Sunday(s).
5.2 Work outside regular working hours requires USS CHUNG-HOON TPOC: LT Michael Grimes’ (CHUNG HOON SUPPO) approval.
5.3 Specific work schedule and sequence of work will be coordinated closely with USS CHUNG HOON TPOC: LT Michael Grimes (CHUNG HOON'S SUPPO).
5.4 Contractor can begin work upon agreed up schedule.
6. SECURITY REQUIREMENTS
6.1 USS CHUNG HOON will provide a security briefing to all Contractors’ personnel prior to commencement of work. The Contractor shall provide pictured ID laminated cards for all Contractor personnel. All Contractors’ personnel will be required to possess visitor badges while on-site.
6.2 USS CHUNG HOON shall provide escorts for Contractors if escorts are required for personnel entering, departing, and working on restricted areas.
6.3 Contractor's employees performing services shall comply with all of USS CHUNG HOON's rules and regulations applicable to safety, security, and procedures governing site entry and exit USS CHUNG HOON’s Security point of contact is USS CHUNG HOON TPOC: LT Michael Grimes (CHUNG HOON's SUPPO).
7. BASE ACCESS
7.1 Commander, Navy Installations Command (CNIC), has established the Navy Commercial Access Control System (NCACS), a standardized process for granting unescorted access privileges to vendors, contractors, suppliers and service providers not otherwise entitled to the issuance of a Common Access Card (CAC) who seek access to and can provide justification to enter Navy installations and facilities. Visiting vendors may obtain daily passes directly from the individual Navy Installations by submitting identification credentials for verification and undergoing a criminal screening/background check. Contractor employees would be responsible for picking up daily passes.
Alternatively, if the vendor so chooses, it may voluntarily elect to obtain long- term credentials through enrollment, registration, background vetting, screening, issuance of credentials, and electronic validation of credentials at its own cost through a designated independent contractor NCACS service provider.
Credentials will be issued every five years and access privileges will be reviewed and renewed on an annual basis. Further information regarding NCACS can be found at http://cnic.navy.mil/CNIC_HQ_Site/index.htm (under"Popular Links").
8. Ship’s Availability Dates and Special Notes.
Most of the items to be installed from 4 August 2017 – 1 September 2017, remaining items to be completed by 30 Oct 2017. Contractor shall be required to work weekends and second shift as required to complete project on time due to ship’s scheduled. Contractor is to provide POAM and schedule with representative and attend status meeting as required. NAVSEA stand items are applicable where required and lead testing is required prior to hot work. Contractor shall comply with requirements of NAVSEA TS9090-310D and have a NAVSEA approved QA manual. Contractor shall comply and manufacture furniture with materials and construction details according to type and standard drawing as referenced.
