TATEMENT OF WORK

1. SCOPE OF WORK: The Contractor shall perform maintenance services of Government-owned Digital

Equipment Corporation (DEC) Federal Information Processing (FIP) equipment to include network components located throughout a campus or multi-building facility, in accordance with the requirements set forth herein. Due to the criticality of the FIP equipment to be supported, response time on maintenance calls will be stringently adhered

to. The FIP equipment comprises the Strategic Weapons Facility Pacific’s (SWFPAC) Management Information

System, Office Automation (MIS/OA) systems and Local Area Network.

2. TASKS: The following sets forth the requirement for maintenance services which includes preventive, remedial, and per-call maintenance. The Contractor shall maintain the MIS/OA systems listed in attached hardware equipment list, so as to perform in accordance with the original equipment manufacturer’s (OEM) specification.

The Government reserves the right to inspect the equipment, or any portion thereof, listed in attached hardware inventory list, to insure the equipment is being maintained at the OEM acceptable maintenance level, at any time during contract performance. This inspection shall be at no cost to the Contractor, shall not take place prior to thirty (30) calendar days after contract award, and shall not include any equipment identified by the

Contractor as not being eligible for OEM maintenance, until such time as the identified equipment has been brought up to an acceptable OEM maintenance level. If, at any time, the Government determines the equipment, or any portion thereof, is not being maintained at the OEM acceptable maintenance level, the monthly maintenance charge for that equipment shall cease. The Government shall notify the Contractor in writing that maintenance charges have ceased due to the equipment not being maintained at the OEM acceptable maintenance level, within four (4) calendar days. The Contractor shall have not more than ten (10) calendar days from date of receipt of notification to insure the equipment is brought up to acceptable levels, and shall certify that the equipment meets the OEM specification for maintenance, at no additional cost to the Government. Maintenance charges shall be reinstated upon receipt and verification of the certification by the Government.

3. MAINTENANCE RESPONSIBILITY

a. Responsibilities of the Government

(1) Government Personnel: Government personnel will not perform maintenance or attempt repairs to equipment while such equipment is under the purview of this contract unless agreed to by the Contractor.

(2) Access: Subject to security regulations, the Government will permit access to the equipment which is to be maintained.

(3) Maintenance Facilities: The Government will provide the Contractor a phone for local calls only.

b. Responsibilities of the Contractor

(1) General Responsibilities: The contractor shall provide all necessary maintenance service to include labor, replacement parts, materials, travel to and from the site, and other expenses necessary as specified herein at the prices shown in the purchase order and shall keep the equipment in good operating condition.

Maintenance coverage will be provided by fully trained and competent maintenance personnel. Personnel who are “in training” will not be utilized except as observers and helpers to qualified personnel.

Maintenance coverage shall not include electrical work external to the equipment, or the adding or removing of accessories, attachments, or other devices. It shall not include repair of damage resulting from accident, transportation between Government sites, neglect, misuse, failure of electrical power or air-conditioning or humidity control, or causes other than ordinary use, except that such repair may be provided on a Per-Call basis under the contract, if agreed to by the Contractor.

(2) Personnel and Facilities Clearance: Personnel and facilities clearances are required at the level of

SECRET for the execution of this contract.

(3) Training of Maintenance Personnel: Maintenance personnel shall have had formal training in maintaining the equipment listed in as follows:

(a) Minimum of three years of combined training and field experience in computer maintenance.

(b) Minimum of two years of field experience in computer maintenance.

(c) Minimum of one year of field experience in DEC/Alpha series maintenance.

(d) Minimum of one year specialized experience in Network management and troubleshooting.

4. HARDWARE MAINTENANCE

a. PRINCIPAL PERIOD OF MAINTENANCE (PPM)

The PPM shall be Monday through Friday, excluding Government holidays, 0700 hours to 1700 hours Pacific

Standard Time (PST). All maintenance furnished during the PPM, whether remedial or preventive, shall be at the basic price for the PPM, and shall include all parts and labor.

b. PREVENTIVE MAINTENANCE

(1) Preventive Maintenance is defined as regular servicing for the purpose of maintaining equipment in satisfactory operating condition by providing for systematic inspection for the detection of incipient failures, either before they occur or before they develop into major defects. The first routine service and maintenance call shall take place within thirty (30) calendar days after the date upon which the Government awards the contract. Preventive maintenance shall be performed in accordance with the OEM standards thereafter. Preventive Maintenance shall be performed at a time other than the PPM, or at a time mutually agreed to by the Government and the Contractor. In the event a time cannot be mutually agreed to, the

Government shall determine the time for the preventive maintenance.

(2)The Contractor shall perform preventive maintenance in accordance with the OEM recommended procedures. Equipment shall be maintained in order to sustain a 90% availability level for components, and a 95% availability level for the system.

(3)The level of maintenance which shall be required is that which is set forth in the manufacturer’s manual.

In the event the equipment has been modified and documentation of the modification is not available, maintenance shall be in accordance with the instructions issued by the Government. Equipment shall not be removed from the Government installation for maintenance or repair without prior Government approval and completion of Form DD 1149.

c. REMEDIAL MAINTENANCE(CLIN0002)

(1) Remedial Maintenance shall be on-call and shall be furnished within the PPM. The Contractor shall respond to a remedial maintenance call within four (4) hours within the PPM. If the Contractor does not respond on the same day as a call is placed, without the four (4) hour period expiring, the Contractor shall respond by 0700 hours PST on the next work day. The Contractor shall provide the Government with one or more designated points of contact to receive remedial maintenance calls, and shall make suitable arrangements to enable its maintenance representatives to receive such calls. Contractor response time shall be measured from the time the Government makes a call (or makes a bona fide attempt to make a call, in the event that the designated point(s) or contact cannot be reached) until the Contractor’s maintenance representative(s) physically arrive at the maintenance site with all necessary parts, equipment, etc., required to effect the repair.

(2) If the equipment fails within four (4) hours after completion of remedial maintenance, this four (4) hour period of time will be considered as a continuation of the previous downtime period for the purpose of computing downtime maintenance credits.

(3) The Contractor shall perform remedial maintenance as necessary for proper equipment performance.

Such service is to include inspection, testing, adjustment and other similar work required to assure performance of equipment in accordance with OEM specification and functional capabilities.

(4) A bona-fide attempt is defined as notifying the Contractor Service Department with the equipment/part number during the PPM associated for that system. If contact cannot be made within thirty (30) minutes after the first attempt to make contact, the response time shall start at the end of that time. If contact is made within thirty (30) minutes the response time shall start at time of contact. If the thirty (30) minute limit for contact has expired, the Government will continue its attempt to make contact.

(5) Parts and Shipping: Maintenance parts for service of equipment of SWFPAC servers, printers and back-up storage systems.

5. MALFUNCTION REPORTS

The Contractor shall furnish to the Government a supply of forms which shall be used by the Government to report date and time of notification and a description of equipment malfunction. The Contractor shall complete the

6. RESPONSE TIME CREDIT

The amount of creditable hours (for failure to arrive within the response time designated herein) shall be accumulated for the month and adjusted to the nearest hour. The amount of credit for each hour in excess of the specified response time shall be computed at the rate of $100.00 per hour. The total credit during any month shall not exceed the Total Monthly Charge.

7. SPARE PARTS, TOOLS AND INSTRUMENTS

a. The Contractor shall maintain the OEM approved level of spare parts, tools, and instruments as necessary to properly and efficiently maintain the equipment for the life of the contract. The Contractor shall use only new standard parts or parts of equal quality. This means that the part may be new, off the production line, or refurbished and returned by the manufacturer to the new parts stock with no distinction made between the refurbished part and a part which is newly manufactured and never used. Defective parts which have been replaced shall become the property of the Contractor. The Contractor shall maintain an adequate supply of the most critical spare parts, available within sixty (60) calendar days after contract award. Evidence of the tentative agreements with parts suppliers, in order to meet the requirement stated above, shall be included in the technical proposal.

b. In no case shall the Contractor use any replacement parts in repairing equipment which would result in the

OEM refusing to support the equipment.

Physical repair of parts on-site for items which require substitution/replacement with OEM approved spares, is unacceptable.

8. EQUIPMENT ADDED TO THIS CONTRACT

a. The Contractor shall be required to maintain any new equipment added to systems under this contract and to maintain new systems and equipment added to this contract if the Contractor is already maintaining the same or similar items from the same or other vendors. If used equipment is added to this contract, it will be maintained by the Contractor, if the OEM certifies that the equipment is eligible for OEM maintenance. If the equipment is ineligible for OEM maintenance, but can be brought up to the OEM acceptable maintenance levels, the Contractor shall, upon Government request, prepare an estimate of the cost of bringing the equipment to OEM acceptable maintenance levels. If the Government elects to bring the equipment to the OEM acceptable maintenance level, the Contractor shall be required thereafter to maintain the equipment.

b. The Contractor will be notified in writing by the Government at least thirty (30) days in advance of any addition of equipment to be maintained under this contract.

c. The maximum proportion of additional equipment under this contract will be limited to twenty-five (25) percent above the original contract amount.

9. DELETION FROM CURRENT SCHEDULE

a. Deletion of equipment/systems from the current schedule will entail a downward adjustment of the per month charges for the equipment. The Government will give thirty (30) days written notification to the

Contractor after which the new rate will be in effect.

b. The maximum proportion of deleted equipment under this contract will be limited to twenty-five (25) percent below the original contract amount.

10. DISCONTINUANCE OF MAINTENANCE SERVICE ON EQUIPMENT UNDER THIS CONTRACT

The Government reserves the right to discontinue maintenance service on any equipment/systems, covered by this contract at any time. The Government will give the Contractor at least thirty (30) days advance notice of its intention to discontinue maintenance service on any given item or system. The maintenance charges for equipment on which maintenance is discontinued shall cease on the effective date of maintenance discontinuance.
