SECTION 1 - GENERAL INFORMATION

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate solicitation will not be issued. This solicitation is being issued as a Request for Quotation, which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2022-04.

This is a 100% Small Business set aside and the applicable associated NAICS code for this acquisition is 812210 with a size standard of $8M Dollars. The Solicitation number is:

FA2543-22-Q-0005

The Government intends to issue a single award BPA as a result of this solicitation, to the offeror that can clearly demonstrate the most efficient approach to meet the government’s needs. The award is anticipated to be a Firm Fixed-Price BPA on the bases of tradeoff analysis.

SECTION 2 - SUPPLIES OR SERVICES AND PRICES

Buckley Garrison, Space Force Base has a need for Mortuary services as described in the attached

PWS.

SECTION 3 – SPECIFIC INFORMATION

Inspection and Acceptance

Inspection and acceptance of deliverables will be performed by the Contracting Officer Representative (COR)

Place and Period of Performance

Performance is required at the contractor’s facility and the anticipated period of performance is 12 month base (May 31, 2022 to May 30, 2023) and 4 subsequent optional ordering periods of 12 months each.

(a) Definition. “Contracting officer's representative” means an individual designated in accordance with subsection 201.602-2 of the Defense Federal Acquisition Regulation Supplement and authorized in writing by the contracting officer to perform specific technical or administrative functions.

(b) If the Contracting Officer designates a contracting officer's representative (COR), the Contractor will receive a copy of the written designation. It will specify the extent of the COR's authority to act on behalf of the contracting officer. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid , is in excess of the simplified acquisition threshold , as defined in FAR 2.101, on the date of award of this contract , and does not contain the clause at 52.215-2, Audit and Records-Negotiation.

(1) The Comptroller General of the United States , or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor ’s directly pertinent records involving transactions related to this contract .

(2) The Contractor shall make available at its offices at all reasonable times the records, materials , and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract . If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data , regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e)

WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (DEC 2018)

(a) Definitions. As used in this clause—

“Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.

“Document type” means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.

“Payment request” and “receiving report” are defined in the clause at 252.232-7003 , Electronic Submission of Payment Requests and Receiving Reports.

(b) Electronic invoicing. The WAWF system provides the method to electronically process vendor payment requests and receiving reports, as authorized by Defense Federal Acquisition Regulation Supplement (DFARS) 252.232-7003 , Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall—

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.sam.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/

(e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor shall use the following information when submitting payment requests and receiving reports in WAWF for this contract or task or delivery order:

(1) Document type. The Contractor shall submit payment requests using the following document type(s):

(i) For cost-type line items, including labor-hour or time-and-materials, submit a cost voucher.

(ii) For fixed price line items—

(A) That require shipment of a deliverable, submit the invoice and receiving report specified by the Contracting Officer.

(B) For services that do not require shipment of a deliverable, submit either the Invoice 2in1, which meets the requirements for the invoice and receiving report, or the applicable invoice and receiving report, as specified by the Contracting Officer.

(iii) For customary progress payments based on costs incurred, submit a progress payment request.

(iv) For performance based payments, submit a performance based payment request.

(v) For commercial item financing, submit a commercial item financing request.

(2) Fast Pay requests are only permitted when Federal Acquisition Regulation (FAR) 52.213-1 is included in the contract.

(3) Document routing. The Contractor shall use the information in the Routing Data Table below only to fill in applicable fields in WAWF when creating payment requests and receiving reports in the system.

(4) Payment request. The Contractor shall ensure a payment request includes documentation appropriate to the type of payment request in accordance with the payment clause, contract financing clause, or Federal Acquisition Regulation 52.216-7, Allowable Cost and Payment, as applicable.

(5) Receiving report. The Contractor shall ensure a receiving report meets the requirements of DFARS Appendix F.

5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (OCT 2019)

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with [insert any additional requirements to comply with AFI 31‐101, Integrated Defense, and DODMAN5200.02_AFMAN 16‐1405, Air Force Personnel Security Program] citing the appropriate paragraphs as applicable.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

(f) Failure to comply with these requirements may result in withholding of final payment.

AFFARS 5352.201-9101, OMBUDSMAN (OCT 2019)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.

Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

SECTION 7 – INSTRUCTIONS, CONDITIONS, AND NOTICES TO OFFERORS

Instruction to offerors

The Air Force is inviting offerors to propose an approach that ensures reliable top-quality mortuary service to serve the Buckley SFB community in accordance with the Performance Work Statement (PWS) provided in this Solicitation.

Quotes are due Wednesday, 11 May 2022 at 11 AM, MST time to zachariah.mckie@spaceforce.mil and claudia.evans@spaceforce.mil. Quotes must be well written, complete and address all requirements of the solicitation. The quotes must have 3 separate volumes/files.

1) Technical Volume: Offerors must provide a complete technical approach that clearly outlines the vendors ability meet the government needs outlined in the PWS

Offerors are expected to provide a detailed plan to upgrade the current courtroom. This volume should not exceed 15 pages (diagrams and tables excluded). The technical volume must provide a clear and complete explanation of the propose solution to include a detailed plan of how to get from current stated to implemented solution and must include:

a) A detail list of all services proposed,

b) A complete list of tasked to be accomplished by the contractor to implement the proposed approach with an accurate execution timeline

c) A list of the capabilities and limitations of the offerors facility and services as it related to the PWS requirements

d) Any optional capabilities that may be purchased at the government discretion; these optional upgrades must be priced as a separate optional CLIN

e) Offer must clearly demonstrate their capability to meet each and all the PWS requirements and the performance level expected from them

f) Any applicable support and warrantees included in the proposed solution

2) Past Performance: Offeror must provide information on 3 projects completed within the last 3 years and of similar size, scope and complexity to Buckley’s current need. Offeror must provide for each project 1) brief description of the effort and how it relates to Buckley’s current need, 2) Points of contact for the customer and 3) A description of the offeror’s role for said project. The government will utilize multiple sources of information to evaluate Past Performance including, but not limited to PPERS and CPARS. Note: If an offeror lacks past performance, the government will not rate them negatively, instead they will receive a rating of “Neutral”

3) Price: Price volume should be the only volume to include pricing data; this volume should clearly indicate which capabilities are optional/ upgrade and have the corresponding price for each. The price and technical Volume terminology to reference the capabilities should be consistent and correlate so the government can easily refer to the technical volume to gain further understanding on the scope and capability of each optional upgrade. Pricing detail provided in this volume should be sufficient to determine its fair a reasonableness allowing comparison to commercial published prices for this type of equipment and services.

SECTION 8 – EVALUATION FACTORS FOR AWARD

Evaluation

52.212‐2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (NOV 2021)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

i. Technical Capability

ii. Past performance

iii. Price

Technical is the most important factor and when combined with past performance, are significantly more important than price.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options that will included for award to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

Technical Capability will be evaluated by how well the proposed products or services meet the Government objectives. The adjustable ratings to be used for technical evaluation are listed in the table below:

Quotes with unacceptable ratings are not eligible for contract award.

Factor 2: Past Performance

Past performance will be evaluated for recency, relevance, and quality based on the information received in the quote, Performance Information Retrieval System (PPIRS), Contract

Performance Assessment Reporting System (CPARS) reports, the past and present efforts provided by the offeror on the Past Performance List of References PPQs, and/or data independently obtained from other government and commercial sources in order to establish a single performance confidence assessment rating as follows:

Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or N/A.

In establishing the Past Performance Assessment rating, greater consideration may be given to information on those contracts deemed most relevant to the effort described in this RFQ. The purpose of the past performance evaluation is to allow the Government to assess the offeror’s ability to perform the effort described in this RFQ based on the offeror’s demonstrated present and past performance/experience. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror will not be evaluated favorably or unfavorably on past performance.

Factor 3: Price

The Government will evaluate price for fair and reasonableness in accordance with FAR PART

13.106. The price evaluation will document and conducted on the bases of proposed total evaluated prices for all CLINS the government will included in the award, to include optional CLINS. The total evaluated price will be obtain by adding all CLINS chosen for award to include optional CLINS in the award..
